RE: APPROVE TASK ORDER CHANGE ORDER LETTER#1 FOR MATERNAL AND CHILD
HEALTH PROGRAM AND HEALTH CARE PROGRAM FOR CHILDREN WITH SPECIAL
NEEDS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Task Order Change Order Letter#1 for
the Maternal and Child Health Program and the Health Care Program for Children with Special
Needs between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment, commencing
October 1, 2006, and ending September 30, 2007, with further terms and conditions being as
stated in said task order change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order change
order letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Task Order Change Order Letter#1 for the Maternal and Child Health
Program and the Health Care Program for Children with Special Needs between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Weld County Department of Public Health and Environment, and the Colorado
Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said task order change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 24th day of January, A.D., 2007, nunc pro tunc October 1, 2006.

SUBJECT: Task Order Change Order Letter#1 to
MCH/HCP Program
Enclosed for Board review and approval is Task Order Change Order Letter#1 between the
Colorado Department of Public Health and Environment(CDPHE) and Weld County for the
MCH/HCP (Maternal and Child Health/Health Care Program for Children with Special Needs)
program.
The original task order included $1,000 for WCDPHE to be a pilot site for TBI (Traumatic Brain
Injury) Surveillance. Due to the resignation of the HCP regional coordinator, this segment of the
contract was suspended. The resumption of these activities will be evaluated at a later date. This
change order letter will reduce funding for the 2006-2007 contract period of October 1, 2006
through September 30, 2007 by $900 for a new total amount of$321,125.
I recommend your approval of this change order letter.

This Task Order Change Order Letter is issued pursuant to paragraph F. of the Master Contract identified as
contract routing number 05 FAA 00054 and paragraph C. 10. of the Task Order identified as contract routing
number 07 FLA 00394 and contract encumbrance number 07 FLA MCH0700394. This Task Order Change Order
Letter is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and
BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY,(a political subdivision of the State of
Colorado)for the use and benefit of the Weld County Department of Public Health and Environment. The
Task Order, as amended, if applicable, is referred to as the"Original Task Order". This Task Order Change Order
Letter is for the current term of October 1,2006 through September 30,2007. The maximum amount payable by
the State for the work to be performed by the Contractor during this current term is decreased by Nine Hundred
Dollars,15900.001 for an amended total financial obligation of the State of Three Hundred Twenty One
Thousand One Hundred Twenty-Five DOLLARS, ($321,125.00).The maximum federal funding source is
decreased by Nine Hundred Dollars,1S 900.001 for an amended total amount of Two Hundred Twenty Two
Thousand Two Hundred Fifty Dollars,15222,250.00). The revised Budget is incorporated herein by this
reference, made a part hereof, and attached hereto as "Exhibit I". The first sentence in paragraph C. of the
Original Task Order is modified accordingly. All other terms and conditions of the Original Task Order are
reaffirmed. This change to the Task Order shall be effective upon approval by the State controller, or designee, or
on February 1,2007,whichever is later.
Please sign, date, and return all 4 (four)originals of this Task Order Change Order Letter by January 22,2007, to
