RESOLUTION
RE: APPROVE APPOINTMENT OF MONICA MIKAAS PRIVACY OFFICER UNDER HEALTH
INSURANCE PORTABILITY AND ACCOUNTABILITY ACT
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, by Resolution#2003-0902, the Board of County Commissioners approved the
Health Insurance Portability and Accountability Act(HIPAA)Compliance Plan for Weld County for
Dental,Vision, and Flexible Spending Plans, and the HIPAA Compliance Plan for the Departments
of Public Health and Environment, Paramedic Services, and Human Services as health care
providers, and
WHEREAS, said HIPAA Compliance Plans require HIPAA Privacy Officers be appointed,
and
WHEREAS, Donald Warden, Director of Finance and Administration, has recommended
that Monica Mika, Director of Administrative Services, be appointed to replace Donald Warden, as
the Weld County Dental, Vision, and Flexible Spending Plans' HIPAA Privacy Officer.
NOW, THEREFORE BE IT RESOLVED by the Board that Monica Mika be, and hereby is,
appointed the HIPAA Privacy Officer for the Weld County Dental, Vision, and Flexible Spending
Plans' HIPAA Plan.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 17th day of September, A.D., 2007.
