WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Grant Application for the Weld Worksite
Wellness Works Program from the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, to the Colorado Department of Public Health and Environment,
commencing July 1, 2007, and ending June 30, 2010, with further terms and conditions being as
stated in said application, and
WHEREAS,after review, the Board deems it advisable to approve said application,a copy
of which is attached hereto and incorporated herein by reference.

Enclosed for Board review and approval is the completed grant application for the
Weld Worksite Wellness Works (WWWW): A coordinated approach to worksite
wellness in Weld County grant from the Weld County Department of Public Health and
Environment. The total amount of the request for Weld County is $165.245. The
funding period is from July 1, 2007 to June 30, 2008, with the anticipation of an
additional two years of funding.
The Weld County Department of Public Health and Environment will use this funding
for two employees (1.5 FTE) to provide heart disease risk reduction education to the
residents of Weld County through community activities, comprehensive worksite
wellness programs targeting small businesses, and assistance for the Weld County Safety
and Wellness Committee.

The Weld County Department of Public Health and Environment is pleased and excited to
submit this grant application to address the risk factors for cardiovascular disease in Weld
County. Heart disease is the leading cause of death in Weld County and funding from this grant
will provide prevention education and activities through community groups and small
businesses.
This agency has established itself as a resource for health education in many areas: tobacco
prevention and cessation, diabetes prevention, abstinence and relationship education, traffic
safety and injury prevention. Tackling the tough issue of lifestyle changes to decrease and
prevent cardiovascular disease using evidence-based strategies in the community and small
businesses will build on past successes and expand current programs.
The Weld County Department of Public Health and Environment has a history of successful
collaboration with other agencies. We appreciate the opportunity to apply for funding to
improve the health of Weld County residents.

WC government.

• Implement worksite wellness programs in a minimum of 20 small businesses
• Hire a Community Wellness Coordinator who will provide leadership for the
worksite wellness activities in WC government as well as be a resource for the
small business community.
• Conduct a minimum of 1 healthy cooking class and 1 nutrition education
program per month in the community that encourages hands-on participation
by WC residents.
• Provide awareness about cardiovascular disease and risk factor modification
through at least 12 media sources(include one well-known community
member as a spokesperson)
• Develop a resource that will be available to all WC residents that will highlight
the 15 best walks around WC.
• Provide for screening of employees and community residents for BMI, Blood
Pressure (BP) and Cholesterol at least annually.
• Collaborate with chronic disease grants currently being funded and
administered through WCDPHE to leverage resources to cover a county-side
effort on chronic disease prevention.
15. Evaluation Strategy: Several methods of evaluation will be used, both qualitative and
quantitative, including but not limited to the number of community residents that participate
in nutrition education and physical activities, data from the Behavioral Risk Factor
Surveillance Survey(BRFSS), Health Risk Assessment and screening data over a three year
period, participation rates of employees at worksites, number and variety of worksite
activities, number of small businesses that participate in worksite wellness activities and
before and after comparison of health care costs for small businesses and WC government.

16. Project Title: Weld Worksite Wellness Works (WWWW): A coordinated approach to
worksite wellness in Weld County.
Abstract:
This grant will focus on three areas of integration: 1) community activities; 2) small group
worksite wellness; and 3) a community wellness coordinator. Community activities will include
classes, media related to risk factor reduction, a walking guide that will highlight the 15 best
places to walk in WC, and experts who will be contracted to disseminate healthy messages about
chronic disease prevention. Examples include participatory healthy cooking classes monthly,
integrated Health Promotion media, and celebrity spokespersons like the "Cooking
Cardiologist". Worksite wellness will include an assessment of the small group worksite, and
then a team of employees who will take ownership to target interventions. The Community
Wellness Coordinator will be the local resource for worksite wellness, while enhancing and
coordinating the WC Government health and wellness worksite plan. This person will be the
link that leverages resources with need and interest at the worksite through a technical assistance
role.
PART III. APPLICATION NARRATIVE
a. The Weld County Department of Public Health and Environment (WCDPHE) has documented
success with grants related to heart disease, nutrition and physical activity, tobacco prevention
and other health topics. This agency also has developed long-standing community partnerships
that result in enhanced collaborative projects, especially when working with low-income and
Hispanic populations.
b. The CWCCI program is an example of a grant funded program that has done an outstanding
job of education and screening for breast and cervical cancer in women, as well as referring them
for treatment, if needed. The grant from the Office of Health Disparities expanded our efforts in
diabetes prevention and education. This grant will provide the resources for similar work in
reducing chronic disease risk factors.
c. Grants awarded to WCDPHE specifically target disparate populations: Steps to a Healthier
Weld County, CWCCI and diabetes prevention have successfully integrated into the community
to contribute to the healthier messages that keep wellness and prevention in the spotlight. Efforts
are targeted in different locations including schools, faith-based organizations and health care
providers. In spite of the expansive square mileage of Weld County, staff are still able to
provide outreach to all areas of WC, from the very rural ranching area of New Raymer in the
northeast to the fast-growing southwestern corner of the county in the Mead, Dacono, Frederick
and Firestone area.
d. This grant will enable WCDPHE to expand successful existing programs. The Steps to a
Healthier Weld grant is working with the larger WC employees on worksite wellness,
establishing Greeley as a"Well City" under the umbrella of the Wellness Councils of America
(WELCOA). Steps targeted the larger employers because they needed to reach 20% of the
• workforce in Greeley. However, 80% of the members of the Greeley/Weld Chamber of
Commerce are small businesses, the group that will be one focus of this grant. This grant would
also help bring more public awareness to the risk factors for cardiovascular disease and provide
WC residents with the knowledge and tools to make behavior changes to reduce their risk
factors.
It is estimated that employers spend $3500/year/worker on health insurance premiums alone and
health care costs have doubled since 1990 and are projected to double again by 2012.5 Behind
these rising health care costs is the fact that 70% are attributed to preventable illnesses.
Cardiovascular disease is the number one cause of premature and permanent disability in the
U.S. labor force with an estimated direct/indirect cost of$403.1 billion in 2006. 75% of adults
have at least one major risk factor.6
The WC Safety and Wellness Committee has been operating for 13 years on a budget of$5000
per year. WC Government has experienced a sharp increase in health insurance premiums,
increases in the number of sick days utilized by employees and higher turnover rates. This grant
will provide the impetus to move towards a comprehensive worksite wellness program that
reduces health care costs by decreasing risk factors for chronic diseases.
NEEDS STATEMENT
a. Weld County (WC) covers a rural area of 3,992 square miles and is the third largest county in
Colorado. The population has increased dramatically over the past ten years. In 1995 the
population was 148,000: in 2005 the population was estimated to be 229,000, equally divided
between females and males. The median age was 31.3 years. 66% of the population is between
the ages of 18 and 65, which are key workforce years.
In 2005: 28% of the people were Hispanic; 69% of the people were White non-Hispanic.
English is the primary language spoken in 80% of the households; Spanish is spoken in 18%
In 2005 the median family income was $57,009, slightly lower than the state ($62,470).
12% of people were living in poverty; since 2000 the poverty rate has been increasing.
The percentage of adults with private or public health insurance decreased from 86% in 1998-
1999 to 78% in 2005.° This decrease may reflect the increasing difficulty for small businesses to
provide health insurance as a benefit because of increasing costs.
According to the 2005 Weld Behavioral Risk Factor Surveillance Survey(BRFSS) obesity is
increasing: 61% of adults were overweight or obese: 40% of the 18 to 24 year olds; 63% of the
25 to 54 year olds; 73% of those 55 to 64 years of age. In comparison, 41% of adults were
identified as overweight or obese in 1990-1991.
The age distribution of WC Government employees is older than that of the average Weld
County resident. 44% of the people employed by WC are 45 years of age or older, compared
with only 30% of WC residents.
In other ways, the nearly 1500 WC employees mirror WC residents: 420 employees (29% of the
workforce) completed the Health Risk Appraisal (HRA) tool called the Personal Wellness Profile
(PWP) during 2005. Of these, 67% were identified as overweight or obese, and 41% were
considered to have high coronary risk. 31% had cholesterol levels over the recommended levels,
and 28% had elevated blood pressure levels. While none of the employees reported being
hospitalized during the last year, 18% of those who completed the survey reported they had
missed 5 or more workdays because of illness.
The Personal Wellness Profile also tries to determine readiness to change and asks which health
action opportunities would be of interest to the individual. 47% of those surveyed indicated they
were in the contemplative, planning or action stage. People in these stages need more
information and additional tools and methods for making changes. The 43%who said they were
in the maintenance stage need encouragement with positive support and reinforcement with:
74% indicated an interest in"Better Nutrition," 67% in "Weight Management," 64% in
"Improving Fitness" and 62% in"Protecting Your Heart."
b. The Weld Worksite Wellness Works (WWWW): A coordinated approach to worksite wellness
in Weld County will promote awareness of the importance of comprehensive worksite wellness
throughout this large, mostly rural, county. Cardiovascular disease is the leading cause of death
of the residents of Weld County, and promoting awareness and educational programs about the
significant statistics that surround this fact is crucial to modifying and controlling risk factors
that negatively affect the quality of life of community residents. The CDC recognizes that
worksites are a unique and successful location to help people modify their lifestyles and live
more healthfully. Through a customized assessment, the community and small group worksites
will enjoy events and educational programming that builds on media about both the obesity and
physical inactivity epidemics. The long- range goal is to promote personal responsibility for
making better and healthier choices for current and future health to prevent and/or delay chronic
disease. Building on the success of large-group worksite wellness programs that have been
seeded by the Steps to a Healthier Weld County federal grant, this project is intended to target
worksites with less than 50 employees (80% of the Chamber of Commerce members). The
target population was chosen due to the sparse resources that these small groups can devote to
the design and implementation of comprehensive worksite wellness programs. Because they are
comprised of a high percentage of disparate employees, these small group worksites can benefit
from awareness and educational programs that will help develop a healthier workforce for the
entire county. Leveraging resources with the Steps grant, the tobacco grant, CWCCI, and the
Diabetes Disparities grant will promote appropriation of grant money to cover a broader scope of
work within a wider geographic area in the rural Weld County

c. A well-planned health management program will provide opportunities for identifying health
risks and learning self-care and prevention of high-risk lifestyle patterns. Healthy People 2010
has a goal(7.5)to increase the proportion of worksites that offer a comprehensive employee
health promotion program to their employees. Currently only 34% of companies employing
fewer than 50 people have any type of health promotion programming to their employees.8
Out of 42 studies on the benefits of worksite wellness programs, 30.3 percent reported reduced
employee absenteeism, health costs were reduced an average of 21.8 percent, and cost-to-
benefit-analysis showed a positive return on company investment of an average of$5.64 net
benefit for each$1 spent on the program)
e. Screening for cardiovascular risk factors will be customized to the worksite wellness plan for
each site. It is projected that 50% of the employees of the small businesses participating in the
project will complete Health Risk Appraisals and screenings. This will increase each year, as
more businesses become involved. Including WC government employees, 800 employees
during year I will be screened for CVD risk factors; 1400 employees during year 2; and 1800
employees during year 3. No treatment services will be provided.
f.The Weld County Department of Public Health and Environment has taken the lead in
establishing community coalitions to share resources, coordinate efforts and avoid duplication of
services related to chronic diseases. The North Colorado Health Alliance is an organization
composed of 12 agencies in Weld County that work together to address the health needs of the
uninsured and underinsured residents of Weld County. Shared clinic space,joint projects for
school health clinics, electronic medical records system that links the health department and
community health clinic records, and diabetes prevention classes in Spanish are examples of
projects where the Health Department has taken the lead in collaborating with these agencies.
f. The Health Department has collaborated with many agencies and businesses on chronic
disease issues. For more than 10 years the Health Department has been the lead agency for the
Weld County Diabetes Coalition, coordinating a variety of community outreach activities and
educational programs for professionals. More recently, a very successful physical activity event
was coordinated with the Greeley Tribune to get community members to collectively"Walk
Around the World in 92 Days". The response was so great(700+participants)that two trips
around the world were achieved.
g. There are key partners who will help make this grant successful. Letters of support from the
four entities outside WCDPHE are included.
1) Human Resources/Personnel Office at WC Government: This office oversees the Safety and
Wellness Committee, is the conduit for information about nutrition and physical activity
programs to the employees, and negotiates benefits like health insurance.
2) Chamber of Commerce: 80% of the members of the Greeley/Weld Chamber of Commerce
are small businesses.
3)North Colorado Medical Center(NCMC) Wellness Services provides excellent clinical
services to worksites and Personal Wellness Profiles, also referred to as Health Risk Appraisals.
4)Aims Community College Downtown Location has offered to host events and classes for
businesses and the community in their neighborhood.
5)Health Promotion Department at WCDPHE where the grants are administered. Steps,
Tobacco, Diabetes Disparities, CWCCI and injury prevention will collaborate to leverage scarce
resources and prevent duplication of services.
h. The employees of Weld County Government have already been involved in the planning for
this project through their participation in previous health screenings and health risk appraisals.
Their interest in reducing their risks of cardiovascular diseases sparked the idea of submitting
this grant application. The Safety and Wellness Committee members will be an integral part of
the successful implementation of the programs. Through surveys and discussions with
employees at the small businesses recruited to participate, comprehensive worksite wellness
programs will be established that meet their needs and interests.
i. By working closely with the small businesses, the health education that focuses on skill
development and lifestyle behavior change, in addition to information dissemination and
awareness building will be tailored to the interests and needs of the employees. When
appropriate, materials and classes in Spanish can be provided.
EVALUATION
a.All activities that are part of the comprehensive worksite wellness programs will be evaluated by
number of participants, as well as knowledge change and"satisfaction" surveys completed by the
participants. For WC government,percentage of employees with high cholesterol levels, high
blood pressure and high overall coronary risk can be compared with the data from the 2005
Personal Wellness Profile Executive Summary Report to see if there are any changes. After
three years of funding for this grant, additional Personal Wellness Profile data from employees
of the small businesses, as well as data for absenteeism and health care costs will be collected
from the businesses and evaluated.
The Behavioral Risk Factor Surveillance Survey has sampled extra Weld County residents to
collect data related to nutrition and physical activity for the Steps to a Healthier Weld grant. An
oversample again at the end of this grant in 2009-2010 will help measure the effectiveness of the
community and business programs throughout the county.
b.The Health Data Specialist at the Weld County Department of Public Health and Environment
will provide inkind evaluation through collaboration with the Community Wellness Coordinator
to set up methods to analyze the collected data.

This letter is in support of the grant application of the Weld County Department of Public Health
and Environment to decrease the risk factors of cardiovascular disease through worksite wellness
and community programs in Weld County. The Personnel/Human Resources Department of the
Weld County Government is excited about the opportunities this will provide for the employees
to learn about making changes in eating habits and increasing physical activity and then to
incorporate this knowledge into behaviors that will improve their health.
As with most large employers, Weld County Government has seen an increase in health
insurance costs over the past few years. The Safety and Wellness Committee provides some
screenings and physical activity programs for the employees, but it is not the comprehensive
worksite wellness program that could lead to significant health improvements.
Employees of Weld County Government have indicated in surveys that they are interested in
learning about cardiovascular risk reduction. Our office will be glad to provide support and
assistance to the Wellness Coordinator to coordinate educational and physical activity events, to

for the Weld County Wellness Program focusing on worksite wellness and
cardiovascular disease prevention. We are interested in partnering by offering the use
of our facilities and expertise in educational programming.
The Downtown Center is located in the heart of downtown Greeley, surrounded by the
City of Greeley offices,the city and county courthouses,the Chamber of Commerce,
The Downtown Development Authority,the Northern Colorado Business Development
Center(Upstate Colorado),Banner Health Administration offices, and small private
businesses which make up most of the downtown businesses. Therefore, our location is
advantageous for both large and small businesses as well as government agencies.
The Downtown Center was created to provide class offerings more accessible to the
East side of Greely. The main campus is on the west side of Greeley and quite large.
The Downtown Center offers a smaller,alternative learning environment that is
accessible by many bus routes, and in walking distance to all the downtown businesses
and residences.
A large population of Aims Community College is adult learners who work full-time.
We cater to the adult learner with classes before and after their work day, lunch-n-
learns,accelerated programs on weekends,online learning, and continuing education.
Along the lines of health and wellness, the Downtown Center currently has classes in
yoga, country swing,ballroom dance, belly dancing, and Caribbean dance. We have Downtown Center

Coordinator grant. This grant would support a full-time FTE who would- part-time-recruit,support
and provide Worksite Wellness assistance to small business in the Weld County area. Services
that could be offered to employers and their employees could include but would not be limited to:
Health Risk Assessments, blood screenings, Lunch 'n Learns, and physical activity&nutrition
programs.
As of provider of health promotion &wellness services in Northern Colorado, I support having a
Wellness Coordinator who will energize the small business community to promote wellness at
their worksite. Small business owners do not have extra time to take part in such an initiative,
which is why funding for an employee dedicated to this mission is so important. As health care
costs in the United States rise, small businesses are keenly aware of the money necessary to pay
for health insurance for their employees.
Greeley, Colorado has just been given the 'approval'to move forth with obtaining a Well City
award from the Wellness Councils of America. All of the large businesses in Greeley are
represented in this initiative, however Worksite Wellness should not only be offered to employees
of large business but also to small businesses. Please consider this as applications are being
reviewed. A Well-Workplace culture in Greeley will soon become the standard. Thank you for

Dear Ms. McConville,
As the President of the Greeley Chamber of Commerce, I would like to express
strong support for the Cancer, Cardiovascular Disease and Pulmonary Disease grant
application being submitted to CDPHE by the Weld County Department of Public Health
& Environment.
The Greeley Chamber is a community partner with the Steps to a Healthier Weld
County Program (federal funding from the U.S. Department of Health and Human
Services' Centers for Disease Control and Prevention) as a supporting business of the
Greeley Well City Initiative.
Furthermore, our visibility within the business community, defines us as the
leading agent for anticipating, communicating, collaborating and acting on issues that
impact the prosperity of our business community. Due to the high rate of chronic
illnesses related to behavior risks in our community, we are dedicated to advancing the
concept of worksite wellness, and WELCOA's Well Workplace process.
Because 80% of our members are considered small businesses, we are
especially excited about the opportunity this grant provides for worksite wellness for their
employees. Please consider funding this grant to the Weld County Department of Public
