RE: APPROVE CLINICAL SITE AGREEMENT AND AUTHORIZE CHAIR TO SIGN - AIMS
COMMUNITY COLLEGE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Clinical Site Agreement between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Public Health and Environment, and Aims Community
College, commencing upon full execution,with further terms and conditions being as stated in said
agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the Clinical Site Agreement between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Department
of Public Health and Environment, and Aims Community College be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 14th day of November, A.D., 2007.

Community College CLINICAL SITE AGREEMENT
The faculty of Aims Community College(College)desire to provide learning experiences for its
students in health occupation programs. Weld County Health Department(Site) has agreed to provide for a
component of those necessary learning experiences.
THEREFORE,it is hereby agreed that:
1. The dates and times for the use of these facilities by such students has been mutually determined by the
College and Weld County Health Department for November 17,2007.
2. Weld County Health Department will provide such students with opportunities for observation and/or
practical experience.
3. College agrees to supervise or to arrange for the supervision of all such students assigned.
4. Weld County Health Department will not pay wages to students while participating in clinical experiences,
and faculty and students shall not be considered to be employees of Weld County for any purpose.
5. College shall provide professional liability insurance for students and faculty of the College in the amount
of one million dollars($1,000,000.00)for any one occurrence and a minimum of three million dollars
($3,000,000.00)in the aggregate covering all occurrences during the term of this Agreement. Upon request,the
College will provide to Weld County Health Department acceptable proof of the insurance policy along with a
list of students who are covered by the policy.
6. Students and faculty shall be responsible for payment of all of their medical bills for injuries or sickness
occurring during the training. The student will assume liability for transportation to and from the clinical site,
including home health visits if appropriate.
7. The College will provide Workers Compensation Insurance to cover students participation in any College
professional health care program pursuant to the option granted in C.R.S. 8-40-202(1)(a)(IV).
8. Weld County Health Department may in his/her sole discretion, request the withdrawal of any student, or
faculty member whose conduct or professional competence is deemed to interfere with their obligations to
provide health care or is disruptive to operations. College shall immediately remove any such student or faculty
member upon verbal notification by Weld County Health Department.
