RE: APPROVE GRANT PROPOSAL FOR DENTAL, VISION, AND HEARING PROGRAM -
NORTH COLORADO MEDICAL CENTER FOUNDATION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Grant Proposal for the Dental,Vision,and
Hearing Program from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on
Aging, to the North Colorado Medical Center Foundation, with further terms and conditions being
as stated in said grant proposal, and
WHEREAS, after review, the Board deems it advisable to approve said grant proposal, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Grant Proposal for the Dental, Vision, and Hearing Program from the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, Area Agency on Aging, to the North
Colorado Medical Center Foundation be, and hereby is, approved.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 19th day of March, A.D., 2008.

Dear Ms. Leffler
The Weld County Area Agency on Aging (WAAA) is submitting this proposal to the North
Colorado Medical Center(NCMC) Foundation requesting a grant in the amount of
$10,000.00. If awarded, the grant will be used to further expand services provided by the
Dental, Vision, and Hearing Program.
Since 1998, the Dental, Vision, and Hearing Program has provided financial assistance for
the eligible sixty and older, low income and at risk senior residents of Weld County who
are in need of dental, vision, and/or hearing aid services.
In fiscal year beginning July 1, 2007, North Colorado Medical Center Foundation awarded
the Weld County Area Agency on Aging Dental, Vision, and Hearing Program $3,900.00.
To date, we are pleased to report that the Dental, Vision, and Hearing Program has served
seventeen Weld County seniors with their vision needs.
With assistance from the North Colorado Medical Center Foundation, we will continue to
reach the ever increasing population of older adults who can benefit from this growing

General Information about Applicant Organization
The Weld County Area Agency on Aging (WAAA) is one of sixteen in the state under
the Colorado Department of Human Services Aging and Adult Services Division. WAAA
shares their mission assist recipients to live safely with maximum personal
independence by planning and promoting an effective, integrated, accessible delivery
system under the Older Americans Act and Older Coloradan Fund.
For over thirty years, WAAA has been a support system for Weld County residents 60
years of age and older and their caregivers. Our primary focus is to help maintain an
individual's independence and dignity in their home and community. The WAAA
supports services for seniors either through contracts with community agencies or by
providing the service directly. These services include adult day care, case management,
home delivered meals, legal assistance, nursing home and assisted living ombudsman
services, peer counseling, rural senior centers, senior nutrition programs, Hispanic
senior outreach, and information and referral to name a few.
Description and Purpose of Program/Project
Since 1998, the Dental, Vision, and Hearing Program has provided financial assistance
for the eligible sixty and older low income and at risk senior residents of Weld County
who are in need of dental, vision, and/or hearing services.
Applicants submit an application; after approval and the selection of a provider of their
choice, authorization is sent to that provider. Once the services are complete, the
provider will bill WAAA at which time it is paid up to a maximum allowable amount. If
the recipient's need is complete and no other services are anticipated, any remaining
funds are reallocated back into the program for other potential recipients. The recipient
is responsible for any balance over the allotted amount and any service(s) rendered or
paid for prior to approval are not covered.
According to the Compass Weld County Assessment, affordable health care and the
availability of affordable providers is a major concern for aging seniors with difficult
choices due to costs. The Dental, Vision, and Hearing Program provides services to
vulnerable populations before a crises situation by providing aid to seniors experiencing
difficulty accessing services. By providing assistance prior to a crises situation, we are
supporting recipients to assume responsibility of their own health by becoming self-
sufficient and begin preventive care.

In September 2004, the Colorado Department of Human Services, Division of Aging and
Adult Services completed a Strengths and Needs Assessment of Older Adults in the
state of Colorado. Based on the information obtained from the study, over 2,500 Weld
County adults sixty or older stated that they needed but could not afford eyeglasses,
hearing aids or dentures. As the population increases, the baby boomer segment
continues to grow, and with programs such as Medicaid not covering these types of
services, we anticipate the need for our program and others like it will also continue to
expand. Essential partnering providers such as Salud Dental Health Clinic, Sunrise
Dental Clinic, Catholic Charities Northern, WAAA staff as well as private providers,
continue to express ongoing need for dental, vision, and hearing services and provide a
steady flow of referrals to the program.
The need in the community is also evident based on the funding being exhausted prior
to the end of the fiscal cycle; which in turn brings about the need for a waiting list.
Along with the inability to serve everyone because of lack of funds, the U.S. Census
Bureau statistics also suggest that society can anticipate a much greater need as the
population increases. According to the Bureau, in 2006, the estimated percent of all
individuals in Weld County below poverty level is 15.7% and persons 65 years and over
below poverty level is 10.8% that is 3.7% and 2.5% above the state's average
respectively. Also according to statistics, there were 32,985 individuals or 17.4% of
Weld County residents that were uninsured in 2000. The older adult population of
Colorado is projected to grow by 51% from 2000 to 2012 (NRC Inc. 2004). Weld
County residents age 60 and over is estimated to grow by over 9,977 or nearly 74%
between 2006 and 2012 (Colorado Department of Local Affairs;
http://www.dola.state.co.us 2007).
Population Served
The WAAA adheres to all of the Older American Act policies and regulations which
include serving a target population of adults sixty years of age and older with special
emphasis on those seniors who are in greatest economic and social need, low income,
minority, frail and rural. Referrals are received from community agencies such as case
managers, churches, adult protective services, Catholic Charities, providers and other
community technicians. Applicants must reside in Weld County; however, the flexibility
of the program allows recipients residing in southern Weld County to receive services
from a provider they select that may be near their home in Adams County. The same
can be said for recipients living near Boulder, Larimer and even Logan Counties.

Amount Requested
In an effort to meet the growing needs of seniors, Weld County Area Agency on Aging is
requesting $10,000.00 from North Colorado Medical Center Foundation for fiscal year
July 2008 through June 2009. Under the current grant of $3,900.00 from NCMC
Foundation, we have been able to assist twelve disadvantaged seniors with their vision
needs. With an award of$10,000.00, WAAA would be able to increase the number of
seniors in need of assistance by approximately 40% from the current award. All funds
received from NCMC Foundation will be used for direct services for seniors; there will
be no administrative costs. These direct services may include, but are not limited to
dental extractions, dentures, cleanings, exams, frames, lenses, hearing exam, hearing
aids. All expenses, including personnel, supplies, and travel will be covered by monies
received through the Older Coloradan Fund.
Committed Funding Sources
During this current fiscal year 2007-2008 the Dental, Vision, and Hearing Program
receives the majority of its funding from the Older Coloradan Fund, $35,000.00. The
Platte Valley Medical Center in Brighton, Colorado through their Community Health
Investment Program (CHIP) provides 20.5% or $10,000.00 of funding and 8% is
provided from the North Colorado Medical Center Foundation or$3,900.00.
Funding by the Platte Valley Medical Center CHIP is restricted to senior residents living
in the southern area of the county. The NCMC Foundation grant dollars have been
extremely helpful in meeting the needs of northern area senior residents. Unfortunately
during the current fiscal year, our waitlist has already almost reached capacity of thirty
individuals with several more months until the new fiscal year begins.
The WAAA staff, senior organizations and individual seniors continue to advocate on a
state and national level for improved funding levels. Weld County Area Agency on
Aging will continue to receive Older Coloradan Funds as the primary source of funding.
However, with an award from NCMC Foundation, we will be able to reach more of the
target population of needy Weld County seniors. It is through this community effort of
working together with our partners and providers that Weld County senior's benefit.

For several years Ms.Burns worked at OK Check
Cashing,an experience she describes as being fun. She
was able to meet a lot of interesting people.She was a
manager,and even went back to school to learn Span-
ish for her job.
In the last few years Ms.Bums has experienced some -
setbacks with her health,and she was in desperate need
of dentures. Being a very social person, and having
always enjoyed the company of others,she began to

Dear Sir or Madam,
This letter enthusiastically supports the Dental, Vision, and Hearing Program that is
administered through the Area Agency on Aging and their application for another grant
to fund the program from the NCMC Foundation. Sunrise Monfort Family Clinic
addresses a wide variety of health care issues and strives to ensure that all
underserved residents in the Greeley/Evans area have access to appropriate,
affordable, comprehensive, quality care.
Several of our patients have received assistance through the Dental, Vision, and
Hearing Program, and the assistance has greatly enhanced their quality of life. Several
patients who have had all of their teeth extracted have been able to receive assistance
through this program for dentures, which has allowed them to again eat a healthy diet
and has boosted their self confidence — they are no longer ashamed to smile or go out
in public. Many patients suffer from diabetes, hypertension, and other physical
conditions that require them to maintain a healthy diet — the dental assistance has
enabled them to again eat nutritious foods. The vision and dental assistance greatly
increases quality of life for our patients as well, allowing them to again be able to drive
or read a book; to be able to hear well enough to again talk on the phone and be an
active participant in group conversations.
The Sunrise Monfort Family Clinic extends its support to the Dental, Vision, and
Hearing Program, and we are excited about partnering with the Area Agency on Aging
once again in order to provide this excellent service to our patients.

BYLAWS
Weld County Area Agency on Aging Advisory Board
ARTICLE I
NAME
The name of this Weld County Government Advisory Board shall be the "Weld
County Area Agency on Aging Advisory Board."
ARTICLE II
PURPOSE
The Weld County Area Agency on Aging Advisory Board organized pursuant to
resolution by the Board of Weld County Commissioners, October 1, 1980, shall
assist the Weld County Board of Commissioners in all matters of planning for
older persons as provided by these Bylaws and shall carry out the objectives and
intent of the Older Americans Act of 1965, as amended.
ARTICLE III
ADVISORY BOARD MEMBERSHIP
Section 1
The Weld County Area Agency on Aging Advisory Board shall have not less than
fourteen(14) nor more than eighteen(18)voting members.
Section 2
The membership on the Weld County Area Agency on Aging Advisory Board
shall be appointed by the Weld County Board of Commissioners, subject to the
following requirements:
a. Members of the Advisory Board shall be comprised fifty percent (50%) of
persons age 60 and older.
b. Other members may include but not be limited to representation of
colleges and universities, elected or public officials, health care providers,
senior centers, County Council on Aging, clergy, local business and
service clubs,the media, and Nutrition Advisory Council members.
c. Providers of services, employees, and members of the Boards of Directors
receiving Title III funds under the Older Americans Act or planning to

receive Title III funds shall not serve as Advisory Board members to the
Area Agency.
Section 3
Advisory Board members appointed in accordance with Section 2 shall generally
be representative in terms of ethnic origin, sex, income level, geographic areas
and population centers of Weld County.
Section 4
Advisory Board members shall be appointed in January and shall simultaneously
serve three (3)year terms with eligibility for reappointment permitted. No person
shall serve more than two (2) terms.
Section 5
In the event of a vacancy or the failure of a member to attend three(3) consecutive
regular meetings of the Advisory Board, the Board of County Commissioners
shall reappoint a qualified individual pursuant to the requirements set forth in
Article III, Section 2.
Section 6
Each member shall be entitled to one (1) vote.
ARTICLE IV
The Advisory Board
Section 1
The Advisory Board shall elect from its members a Chairperson and Vice-
Chairperson. The elected officers shall serve one (1) year terms beginning in
February. No officer shall serve more than two (2)consecutive terms.
Section 2
The time and place of the Advisory Board meetings shall be determined by the
Weld County Area Agency on Aging.
Section 3
The Advisory Board shall meet monthly unless there is no other business to
transact.

Section 4
Except as provided herein, a quorum shall be deemed formed when there are
present fifty-one percent (51%) of the then presently constituted voting
membership. However, in no case shall less than five (5) voting members present
constitute a quorum.
Section 5
A majority vote of the members present shall be considered a favorable vote upon
all motions or resolutions considered a favorable vote upon all motions or
resolutions considered by the Advisory Board.
Section 6
The Advisory Board will establish committees to provide advice and assistance to
the Advisory Board and staff in carrying out the objectives of the Area Agency on
Aging. At a minimum, two standing committees will be established, a Committee
on Nutrition and Senior Centers and a Long Term Care Ombudsman Committee.
Other committees may be appointed at the discretion of the Advisory Board.
Membership on committees shall be comprised at a minimum of one person from
the Area Agency Board who will serve as chairman of the committee and liaison
to the board. Other members may be appointed from the board and from the
community at large.
Section 7
Robert's Rules of Order as revised, shall govern the conduct of the Advisory
Board meeting, unless otherwise specified by the Bylaws.
ARTICLE V
DUTIES OF THE OFFICERS
Section 1
The Chairperson shall preside at all meetings of the Area Agency Advisory Board
and may call special meetings when needed. Notice to the members of special
meetings shall no sooner than 48 hours.

Section 2
The Vice-Chairperson shall preside and perform the duties of the Chairperson in
the Chairperson's absence and shall perform all other duties that may be assigned
by the Chairperson.
Section 3
An Executive Committee shall be comprised of the Chairperson, Vice-
Chairperson, and subcommittee chairperson. The Chairperson shall appoint any
other member(s) from the general membership to serve on the Executive
Committee. This Committee shall have the authority to act on those matters
requiring immediate decision and action prior to regular Area Agency on Aging
Advisory Board meetings. However, the continuing effect of such interim action
requires ratification by the Area Agency Advisory Board at the next regular Board
meeting.
ARTICLE VI
ADVISORY BOARD FUNCTIONS
Section 1
The Area Agency Advisory Board shall perform the following functions in an
advisory capacity to the Weld County Commissioners:
a. Develop and submit annually, to the state agency responsible for approval,
an Area Plan on Aging which conforms to federal requirements.
b. In addition to the development and administration of an Area Plan, the
Advisory Board shall carry out directly, to the maximum extent feasible,
the following functions:
(1) Provide leadership and advocacy on behalf of all other persons
within the county.
(2) Determine the need for social services in the region, with special
attention being given to the needs of low-income and minority
elderly.
(3) Inventory the resources available within the region to meet the
needs of the elderly and perform an evaluation of the effectiveness

of the services provided by the public and private agencies within
the county in meeting such needs.
(4) Establish measurable program objectives and priorities for
implementation of the Area Plan in keeping with the objectives
established by the State Division of Services for the Aging.
(5) Plan with existing agencies and providers of services in the county
who are concerned with the needs of the elderly.
(6) Either directly or through contract or grant, provide for an action
program designed to:
Coordinate the delivery of existing services for the elderly.
Pool untapped resources of public and private agencies in order to
strengthen or initiate new services for older persons.
(7) Make periodic evaluation of activities carried out pursuant to the
Area Plan, including the views of older persons participating in
such activities.
(8) Conduct periodic public hearings concerning the needs of the
elderly.
(9) Collect and disseminate information concerning the needs of the
elderly.
(10) Make provision for technical assistance to providers of social
services in the county.
(11) Where necessary and feasible, enter into arrangements consistent
with provisions of the Area Plan under which funds of Title III of
the Older Americans Act, as amended, may be used to provide
legal services to older persons in the region, carried out through
federally-assisted programs or other public or non-profit agencies.
(12) Where possible, enter into arrangements with organizations
providing day care services for children so as to provide
opportunities for older persons to aid or assist on a voluntary or
paid basis in the delivery of such services to children.
(13) Take into account, matters of general policy arising i the
development and administration of the Area Plan, and the views of
recipients of services under the Area Plan.

c. Develop a comprehensive and coordinated system for the delivery of
social services to the elderly in the county.
d. Review and comment on all A-95 type applications forwarded to the
Advisory Board by the Regional Clearinghouse,which pertain to programs
and planning for the elderly.
ARTICLE VII
AMENDMENT OF BYLAWS
Section 1
These Bylaws may be amended at any regular meeting of the Weld County Area
Agency Advisory Board with subsequent approval of the Weld Board of
Commissioners.
Section 2
At least twenty-five (25) days notice in writing shall be given of any proposed
amendments of these Bylaws. The written notice shall also include the proposed
amendments.
