RE: GRANT COUNTY DANCE HALL LICENSE NUMBER 343 ISSUED TO BAYOU HOUSE,
LLC, DBA DOUG KERSHAW'S BAYOU HOUSE AND AUTHORIZE CHAIR TO SIGN -
EXPIRES DECEMBER 31, 2008, AT 12:00 MIDNIGHT
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, Bayou House, LLC, dba Doug Kershaw's Bayou House, presented the Board
of County Commissioners with an application for a County Dance License to hold public dances
at the location described as follows:
33131 Highway 85
Lucerne, Colorado 80646
WHEREAS, said applicant has paid to Weld County the sum of TWENTY-FIVE AND
NO/100 DOLLARS ($25.00) for said Weld County Dance Hall License, and
WHEREAS, having examined said application, the Board deems it appropriate to grant a
Weld County Dance Hall License to Bayou House LLC, dba Doug Kershaw's Bayou House, and
WHEREAS, said license shall be issued upon the express condition and agreement that
the public dances will be conducted in strict conformity to all laws of the State of Colorado, and any
violation thereof shall be cause for revocation of the license.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Dance Hall License Number 343 be, and hereby is, granted to Bayou
House, LLC, dba Doug Kershaw's Bayou House, to conduct dances at the hereinabove described
location.
BE IT FURTHER RESOLVED by the Board that said License shall be in effect through
12:00 o'clock midnight, December 31, 2008, conditional upon the applicant conducting all dances
in strict conformity to all laws of the State of Colorado.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said License.

License fee of$25.00, payable to Weld County Clerk to the Board, P.O. Box 758, Greeley,
Colorado 80632,is hereby tendered and application is hereby made fora license to conduct a public
dance hall,booth or pavilion,where two or more public dances are held,for the calendar year
or portion thereof.
The undersigned hereby makes application for a County Dance Hall License, under the
provisions of Chapter 12, Article V of the Weld County Code,and Title 12, Article 18, C.R.S., and
for that purpose makes the following answers to the questions contained in this application. Every
question must be answered. Any false answer or statement made by the applicant constitutes
perjury and the person making such statements subjects any license issued herein to revocation.

Fort Lupton Substation I have reviewed the application for the dance hall license number 343, at the Bayou
9T STREET
Ft. Lupton, CO 80621 House, located at 33131 Highway 85 in Lucerne, Colorado and investigated the
Ph. (303) 637-2421 incidents requiring law enforcement action at that location.

There were three (3) incidents in 2007 at the Bayou House in which written reports
are documented.
Southwest Complex
4209 WCR 24 'A 1) On 02-07-2007 the Sheriffs Office was sent to the Bayou House when a
Longmont, CO 80504 man showed up after hours, demanding entry into the building to look for
Ph. (720) 652-4215 his girlfriend.
Fax(720) 652-4217 2) On 03-03-2007 a couple left the restaurant and refused to pay for their
meal. The male physically attacked the manager in the parking lot after
being notified the restaurant was calling the police. No charges were filed
North Jail Complex and an agreement between both parties was reached.
2110"O"Street 3) On 08-09-2007 an adult, intoxicated female was contacted in the parking

Ext. 3922 and did not have anywhere to go. She was transported to Island Grove

The Weld County Sheriffs Office responded to three (3) false fire alarms, nine (9)
false burglary alarms, conducted four (4) bar checks, one (1) business check and
various other calls including unwanted persons and suspicious activity.
