RE: APPROVE COORDINATION AGREEMENT AND LEASE AND AUTHORIZE CHAIR TO
SIGN - THE CHRISTIAN CHURCH OF WINDSOR DISCIPLES OF CHRIST
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Coordination Agreement and Lease
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Social Services,and The Christian
Church of Windsor Disciples of Christ,commencing March 1, 2008,and ending February 28, 2009,
with further terms and conditions being as stated in said agreement and lease, and
WHEREAS, after review, the Board deems it advisable to approve said agreement and
lease, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services,that the Coordination Agreement and Lease
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County,on behalf of the Department of Social Services,and The Christian
Church of Windsor Disciples of Christ be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement and lease.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 19th day of May, A.D., 2008, nunc pro tunc March 1, 2008.

RE: Coordination Agreement and Lease Betweethe Weld County
Department of Social Services and The Christian Church of
Windsor Disciples of Christ
Enclosed for Board approval is a Coordination Agreement and Lease between the Weld County
Department of Social Services(Department)and The Christian Church of Windsor Disciples of
Christ(Church of Windsor). This Agreement and Lease was reviewed at the Board's Work
Session of February 25, 2008.
The major provisions of the Agreement and Lease are as follows:
I. The period of this Agreement and Lease is March 1, 2008 through February 28, 2009.
2. The Church of Windsor will help recruit volunteers to assist clients who are applying for
or are being re-determined for public assistance and will provide space at their facility for
a web-cam site each Tuesday from 9:00 a.m. to 11:00 a.m. The Department will train
such volunteers.
3. The Department agrees to reimburse the Church of Windsor a total of$250 per month for
janitorial and utility costs for such space, which will be appropriate and secure for a web-
cam site.
4. The Church of Windsor agrees to comply with non-sectarian activities and with Title VI
of the Civil Rights Act of 1986 while involved with the web-cam site.

COORDINATION AGREEMENT AND LEASE
THIS COORDINATION AGREEMENT AND LEASE, made and entered into this _29th day of
February 2008, by and between the COUNTY OF WELD, a political subdivision of the State of Colorado, by and
through the Board of County Commissioners of the County of Weld, and sifting as the Board of Social Services for
the Weld County Department of Social Services, herein after referred to as ("Lessee")and The Christian Church of
Windsor Disciples of Christ, hereinafter referred to as("Lessor").
WITNESSETH:
That in consideration of the keeping and performance of the covenants and agreements by Lessee,
hereinafter set forth, Lessor hereby leases unto Lessee one office and work space, Tuesdays from 10:00 AM to
12:01 PM during which time is referred to herein as "Web-Cam Site", located at The Christian Church of Windsor
Disciples of Christ at 530 Walnut Street, Windsor,Colorado,hereinafter referred to as"the Premises".
The parties hereto agree as follows:
1. That there will be no rent charged to the Lessee by the Lessor for use of the Web-Cam Site;
2. That the Lessee will use ordinary care when using the Web-Cam Site;
3. That there will be no sublet of any part of the Web-Cam Site;
4. That the Lessor will not use the Web-Cam Site for no other purpose than conducting an access
point for the business and duties of interviews for public assistance applicants and recipients of
the Lessee;
5. That the Web-Cam Site will be kept clean and in sanitary condition;
6. That the Lessee will pay $100 per month for utilities;
7. That the Lessor will allow the Web-Cam Site to be open each Tuesday from 10:00 a.m. to 12:00
p.m. (noon);
8. That trash removal shall be paid for by Lessor;
9. That no assent, express or implied, to any breach of any one or more of the covenants or
agreements hereof shall be deemed or taken to be a waiver or any succeeding or other breach;
10. That Lessor agrees to indemnify, save, and hold harmless Lessee from any and all loss, injury, or
damage whatsoever, incurred by Lessee as the result of fire, flood, inclement weather, or other
casualty,other than by negligence of the Lessor;
11. That Lessee shall own and maintain the filing cabinets and computer systems including
computers, modems, printers, and associated computer equipment, used and stored at the Web-
Cam Site, as purchased by the Lessee;
12. That Lessor shall have access to and use ordinary care of the DSL Line paid by the Lessee and
shall provide for reasonable security of the Lessee's equipment;
13. That Lessor shall recruit volunteers who have appropriate background checks in accordance of
Colorado Revised Statute (C.R.S.) 26-6-104 to assist clients in the Lessee's paperwork for
assistance to public assistance and to escort such clients to and from the Web-Cam Site and
reception area;

14. That Lessee shall pay for the cost of background checks of volunteers, train such volunteers, and
provide mileage reimbursement, which will be compensated to the volunteer at the same level as
other employees of the Lessee;
15. That volunteers are not agents or employees of the Lessor or the Ministerial Association of
Windsor;
16. That Lessee shall pay for miscellaneous office supplies and forms and related janitorial services
for the Web-Cam Site at a cost of$125 per month;
17. That Lessee shall pay for the cost of appropriate signage;
18. That Lessor shall comply with non-sectarian activities and with Title VI of the Civil Rights Act of
1986 and no person shall, on the grounds of race, creed,color, sex, or national origin,be excluded
from participation in, be denied the benefits of, or be otherwise subjected to discrimination in its
involvement in the Web-Cam Site;
19. That the Lessee shall comply with the Lessor's restrictions that the use of tobacco, in any form,or
alcohol beverage is not allowed on property owned by the Lessor;
20. That the duration of the lease agreement shall commence on March I, 2008, unless sooner
terminated by any of the parties hereto, upon the delivery of thirty (30) days notice of such
termination to the other parties. The lease agreement shall terminate on February 28,2009.
