RE: APPROVE AMENDMENT TO COLORADO WORKS AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN -WELD COUNTY SCHOOL DISTRICT 6
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Colorado Works
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Weld County School District 6, commencing July 1, 2007, and ending June 30, 2008, with further
terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,ex-officio Board of Social Services, that the Amendment to the Colorado Works
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
Weld County School District 6 be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 7th day of January, A.D., 2008, nunc pro tunc July 1, 2007.

County Department of Social Services and School District 6—
Multi-Disciplinary Youth Assessment Team (MYAT)
Enclosed for Board is an Amendment to Colorado Works Agreement between the Weld County
Department of Social Services and School District 6. This Amendment was reviewed at the
Board's Work Session held on November 5, 2007.
The purpose of the Amendment is to correct the level of compensation to the agreed upon amount
of$42,848 for a 40 hour per week Full Time Equivalent(FTE).
All other provisions of the Agreement will remain the same including the term of the Agreement
of July 1, 2007 through June 30, 2008.

AMENDMENT TO COLORADO WORKS AGREEMENT FOR SERVICES 1 .
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES ,AFC'
AND SCHOOL DISTRICT 6
This Amendment, made and entered into the 29h day of October 2007,by and between the Board of Weld ,,*:-
County Commissioners, on behalf of the Weld County Department of Social Services,hereinafter referred to as
"Social Services", and School District 6,hereinafter referred to as"School District 6." QS
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(PY07-08-CWP-25)effective July 1,
2007,to modify School District 6's weekly participation in the Multidisciplinary Youth Assessment Team,and the
fee Social services agrees to pay the MYAT contractor for a period beginning July 1, 2007 and ending June 30,
2008; and
WHEREAS,required approval, clearance,and coordination have been accomplished from and with
appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as follows:
I. Exhibit A,Roles of Contractor,Item B,will be revised as follows:
School District 6, a 40 Hour per week FTE ending June 30,2008.
2. Exhibit C,Overall Cost of Phase VIII, Item A,will be revised as follows:
School District 6,a 40 hour per week FTE: $42,848.
Except as modified herein,the terms and conditions of the Original Agreement,together with all
attachments of the Original Agreement,remain in full force and effect.
