CHANGE REQUEST FORM
This Change of Scope Request ("Change Request") is governed by the terms and conditions of the Master Professional Services Agreement
("Agreement") between 3t Systems, Inc., a Colorado corporation ("3t Systems"), and Interagency Oversight Group ("Client") dated and
effective August 21,2007 and the SOW issued there under and effective August 16,2007. This Change Request shall describe the nature of the
requested change to the Tapestry Sysop SOW. 3t Systems understands that the undersigned has the authority to accept and approve the
Services and any modifications to any schedule on behalf of the Client. The terms and definitions of the Agreement shall apply to the SOW
and this Change request and any exhibits or schedules hereto.
Upon completion by 3t Systems of the information below and the return of this Change Request to Client, Client shall have five(5)business
days to accept this Change Request by executing below and returning to 3t Systems. Upon the expiration of five (5)business days without
Client's execution and return,this Change Request,including any delivery timelines and cost estimates contained herein,shall expire and have
no binding effect. Client is hereby advised that whether this Change Request is ultimately accepted by the parties or not, the dependency of
this Change Request and the subject matter referenced herein may necessitate a temporary work stoppage under the Tapestry Sysop SOW
until the resolution of this Change Request.

Problem Description:
The Healthy Marriages project will not be able to be completed by January 31,2008 as stated in Change Request 2 dated
November 27,2007 due to dependencies contingent upon VisionLink's release of Form Builder.
Solution Requested:
The delivery date for the Healthy Marriages project will be changed to March 15,2008 contingent on deliveries and training
provided by VisionLink.
Resource Impact:
Jim Finch's time will need to be scheduled to accommodate the new scheduled completion date.
Schedule Impact:
The delivery date for the Healthy Marriages project will be changed to March 15,2008 contingent on deliveries and training
provided by VisionLink.
Cost Impact:
The additional cost associated with this change is$0.

and the Healthy Marriages project will not be able to be completed by December 31, 2007 as stated in the SOW due to

The delivery date for the JAC project will be changed to December 31,2007 and the delivery date for the Healthy Marriages
