Bridge 27/66A Replacement
Project Description: Replacement of existing steel bridge on WCR 27 over the Greeley Canal #2. The
new bridge will be a reinforced concrete structure approximately 40' wide by 60' long. Short Elliott
Hendrickson Inc. (S.E.H.) has completed the design of the new bridge. Construction bidding is scheduled
for September, 2008.

The following change is hereby made to the Contract Documents:
Contract Time Extension: The original Agreement for Professional Services is extended one full year for
upcoming construction. The anticipated construction services completion date is June 1, 2009.
Change to Contract Scope: Engineering Construction Services shall be added to the Contract. A detailed
description and cost breakdown of these services is attached.
CHANGE TO CONTRACT PRICE:

Change Order No.1 Costs $88,934.31
Revised Contract Amount $259,040.69
CHANGE TO CONTRACT TIME:

Dear Mike:
This letter supplements and amends the Agreement for Professional Services between Weld County,
Owner, and Short Elliott Hendrickson Inc. (SEH°) dated 9/12/07 and addresses providing services in
connection with the design and construction administration of a replacement bridge for Weld County
Bridge 27/66A, BRO C030-030, Project Code/SA# 14655, and related improvements (hereinafter called
the "Project").
The County has requested that SEH provide services through the re-construction of the bridge. The scope
of services to be provided is shown in the attached material, but generally consists of: full time
construction inspection provided by Mike Shrout, material testing as outlined in the attached CDOT Form
250, and shop drawing review and other technical assistance(included in the existing contract).
You will pay us a fee for these services,currently estimated to be$88,934.31, in accordance with the
Agreement. This brings the total authorized fee to$259,040.69 when the design engineering fee of
$170,106.38 is included.
This letter, the Supplemental Letter Agreement dated April 9, 2008, and the Agreement for Professional
Services dated 9/12/07 along with Exhibits A and B represent the entire understanding between you and
us in respect of the Project and may only be modified in writing signed by both of us. If it satisfactorily
sets forth your understanding of our agreement, please sign the enclosed copy of this letter in the space
provided below and return it to us.

structures, asphalt pavement, and wrote a large percentage of the CDOT
Form 266s that measured work, allowing payment to the Contractor.
8 Mile Overlay, I-25, SH 52 to SH 66. Design and
Construction/Assistant Project Engineer. This was a night paving
project. Michael performed reviews of the traffic control, performed
asphalt inspection,and wrote most of the CDOT Form 266s that
documented work, allowing payment to the Contractor.
Bridge Replacement with Detour,SH 392 E of Windsor. Design and
Construction/Assistant Project Engineer. This was a bridge
replacement over a canal. Michael was de facto Project Engineer and
performed reviews of traffic control, performed inspection of the detour
and new bridge, performed asphalt inspection,and wrote nearly all of the
CDOT Form 266s that documented work,allowing payment to the
Contractor.
8 Mile Overlay,I-76,Lochbuie To Hudson.Design and
Construction/Assistant Project Engineer. This was a resurfacing
project. Michael performed reviews of the traffic control, performed
asphalt inspection, and wrote most of the CDOT Form 266s that

Bridge Replacement with Detour, US 36 W/O Last Chance.Design
and Construction/Assistant Project Engineer.This was a bridge
replacement over a draw. Michael performed reviews of traffic control,
performed inspection of the detour and new structure, performed asphalt
1 inspection,and wrote nearly all of the CDOT Form 266s that documented

Brief description of the problem/issue:
Weld County received federal funds for the replacement of Bridge 27/66A. The design is now complete, and
R.O.W./Easements acquisition is ongoing. The project is scheduled to be bid out in September 2008.
Construction is scheduled to be completed in May, 2009. Public Works is recommending that Short Elliott
Hendrickson Inc., the consultant who completed the design, be utilized for construction services. The
construction services will entail daily inspection, materials quality control, and documentation. This work
requires an inspector who is familiar with CDOT regulations and paperwork requirements.
What options exist for the Board:
The cost for the work was negotiated and the proposed Change Order No.1 amount is $88,934.31. Weld
County's portion of this amount is 20% ($17,786.86), and the federal funds cover the remaining 80%. The
Board could either accept or not accept the Change Order No.1. Not accepting this change order could result in
the bridge replacement project not meeting completion deadlines, and Public Works would be required to seek
an alternate means of accomplishing the construction services work.
Recommendation to the Board:
I recommend approval for the Change Order No.1. Approval will allow the project to proceed in a timely
manner, and ensure inspection by a well-qualified individual. The proposed cost is reasonable when compared
to similar completed projects and industry standards.
