RE: APPROVE AMENDMENT #1 TO TASK ORDER CONTRACT FOR HEALTH CARE
PROGRAM FOR CHILDREN WITH SPECIAL NEEDS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with Amendment#1 to the Task Order Contract
for the Health Care Program for Children with Special Needs between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and the Colorado Department of
Public Health and Environment, commencing October 1, 2008, and ending September 30, 2009,
with further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Amendment#1 to the Task Order Contract for the Health Care Program
for Children with Special Needs between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Department
of Public Health and Environment,and the Colorado Department of Public Health and Environment
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 27th day of August, A.D., 2008.

SUBJECT: Amendment#1 to the HCP Program
Task Order Contract
Enclosed for Board review and approval is Amendment#1 to the task order contract between the
Weld County Board of Commissioners on behalf of the Department of Public Health and
Environment (WCDPHE) and the State of Colorado through the Colorado Department of Public
Health and Environment(CDPHE) for the Health Care Program for Children with Special Needs
(HCP).
This amendment will provide continuation funding in the amount of$182,009 for the time period
October 1, 2008 through September 30, 2009. Of this amount, $81,904 is pass through funding
from the federal government, and the balance of$100,105 is State of Colorado funding. These
funds will be used to facilitate the development and enhancement of community-based systems
of care for children with special health care needs. Some of the services provided will be case
finding, community involvement, and interagency collaboration for children and their families
who are determined eligible for such services. Eligible children are those who have or are at risk
for a chronic physical, developmental, behavioral, or emotional condition. I recommend your
approval of this amendment.

THIS AMENDMENT,made this 1" day of August,2008.by and between the State of Colorado,acting by and
through the DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT,whose address or principal place
of business is 4300 Cherry Creek Drive South,Denver,Colorado 80246,hereinafter referred to as the"State";
and,BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY,(a political subdivision of the state of
Colorado).for the use and benefit of the Weld County Department of Public Health and Environment,whose
address or principal place of business is 155 North 17`s Avenue,Greeley,CO 80631,hereinafter referred to as the
"Contractor".
FACTUAL RECITALS
Authority exists in the Law and Funds have been budgeted,appropriated,and otherwise made available and a
sufficient unencumbered balance thereof remains available for payment;and
Required approval,clearance,and coordination has been accomplished from and with all appropriate agencies;and
The parties entered into a Master Contract,dated December 18,2006 with contract routing number 08 FAA 00052.
Pursuant to the terms and conditions of the Master Contract,the parties entered into a Task Order Contract,dated
August 01,2007 with contract encumbrance number PO FLA HCP0800446 and contract routing number
08 FLA 00446 referred to herein as the"Original Task Order Contract,whereby the Contractor was to provide to
the State the following:
In partnership with other community organizations,shall facilitate the development and
enhancement of community-based systems of care for children with special health care needs; and to
ensure that all families have access to direct care,enabling and population-based services that
address the issues and health problems delineated in the MCH performance measures.
The purpose for this amendment is described below:

NOW THEREFORE,it is hereby agreed that
I. Consideration for this amendment to the Original Task Order Contract,08 FLA 00446 dated August 01,
2007,with contract encumbrance number PO FLA HCP0800446,consists of the payments which shall be
made pursuant to this amendment and the promises and agreements herein set forth.
2. It is expressly agreed by the parties that this amendment is supplemental to the Task Order Contract,
contract routing number 08 FLA 00446,referred to as the"Original Task Order Contract,"which is,by this
reference incorporated herein,that all terms,conditions,and provisions thereof,unless specifically
modified herein,are to apply to this amendment as though they were expressly rewritten,incorporated,and
included herein.
3. It is agreed the Original Task Order Contract is and shall be modified,altered,and changed in the following
respects only:
A. This Amendment is issued pursuant to paragraph 5 of the Original Task Contract identified by
contract routing number 08 FLA 00446. This Amendment is for the renewal term of October 1.
2008 through and including September 30,2009. The maximum amount payable by the State for
the work to be performed by the Contractor during this renewal term is One Hundred Eighty
Two Thousand Nine Dollars,($182,009.00)for an amended total financial obligation of the State
of THREE HUNDRED SIXTY FOUR THOUSAND EIGHTEEN DOLLARS ($364,018.00).
This is an increase of One Hundred Eighty Two Thousand Nine Dollars($182,009.00)of the
amount payable from the previous term. The revised specifications to the original Scope of Work
and the Budget,for this renewal term are incorporated herein by this reference and identified as
"Exhibit D"and"Exhibit E",respectively. The Original Task Order Contract is modified
accordingly. All other terms and conditions of the Original Task Order Contract are reaffirmed.
4. The effective date of this amendment is upon approval of the State Controller or October 1,2008,
whichever is later.
5. Except for the"Special Provisions",in the event of any conflict,inconsistency,variance,or contradiction
between the provisions of this amendment and any of the provisions of the original agreement,the
provisions of this amendment shall in all respects supersede,govern,and control.The"Special Provisions"
shall always be controlling over other provisions in the agreement or amendments.The representations in
the Special Provisions concerning the absence of bribery or corrupt influences and personal interest of State
employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT FISCAL YEAR
ARE CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING APPROPRIATED,BUDGETED,
AND OTHERWISE MADE AVAILABLE.

IN WITNESS WHEREOF,the parties hereto have executed this amendment on the day first above written.
*Persons signing for Contractor hereby swear and affirm that they are authorized to act on Contractor's
behalf and acknowledge that the State is relying on their representations to that effect and accept personal
responsibility for any and all damages the State may incur for any errors in such representation.

STATEMENT OF WORK
Weld County Department of Public Health and Environment
To Contract Amendment Dated 08/01/2008 -Contract Routing Number 09 FLA 00544
This Statement of Work is for one of the three Maternal and Child Health(MCH)populations,which covers
Children with Special Health Care Needs(CSHCN).
1. Under this Task Order the Contractor shall provide the core public health services of assessment,policy
development,and assurance on behalf of the Children with Special Health Care Needs(CSHCN)population as
described and defined in"Core Public Health Services Delivered by MCH Agencies",a component of the MCH
Guidelines available on the MCH website: www.mchcolorado.ona and the Health Care Program for Children
with Special Needs(HCP)website:www.hcpcolorado.ore The MCH Guidelines are incorporated and made part
hereof by reference.
2. The Contractor shall provide leadership,in coordination with public and private community partners,in the
development and implementation of the Contractor's Children with Special Health Care Needs Operational Plan.
3. The State shall provide guidance and technical assistance to the Contractor to support the implementation of the
Contractor's Children with Special Health Care Needs Operational Plan for federal fiscal year 2008-2009 and for
the development of the Contractor's Children with Special Health Care Needs Operational Plan for the federal
fiscal year 2009-2010.
4. The Children with Special Health Care Needs Operational Plans are to be based on a community planning
process that includes a review of the health status needs of the CSHCN population and of the health system
resources of the community. These plans are further designed to:
a. Contribute to the accomplishment of the National and State priorities,performance
measures,and outcome measures,as identified in the MCH Guidelines.
b. Provide for the continuation of the core public health services of assessment,policy
development,and assurance on behalf of the maternal and child health populations; and
in implementing the 10 essential services for this population in partnership with the State,
as identified in the MCH Guidelines.
c. Work with public and private community partners to plan for the development and
maintenance of resources that assure access to direct care and services for vulnerable
children and adolescents with special health care needs,such as those who are low-
income,uninsured,underinsured,living in rural or underserved areas,or from ethnic or
cultural minority communities who may experience language or cultural barriers to
services;
d Facilitate outreach and referral efforts to increase enrollment of eligible children and
adolescents,including those with special health care needs,in Medicaid(Colorado Baby
Care/Kid's Care Program)or Child Health Plan Plus(CHP+)and Supplemental Security
Income(SSI);
e. Refer families participating in any and all programs in its agency such as Women,Infants
and Children(WIC);Early and Periodic Screening,Diagnosis and Treatment(EPSDT);
immunizations clinics;Family Planning;HCP etc.to appropriate enabling and direct care
resources and service programs in the community;
f. Collaborate with public and private community partners to plan for the development and
implementation of population-based approaches within a community Medical Home

System for addressing MCH performance measures and priority issues for children with
special health care needs in the community.
5. The Contractor shall receive technical assistance and consultation from State MCH consultant(s)for planning and
implementing their operational plan and program activities in the facilitation of a community medical home
approach for CSHCN.
6. The Contractor shall perform in accordance with the HCP Contract,Performance Measures,and responsibilities
listed in the MCH Guidelines and HCP Guidance and Policy and Procedures. The Guidelines and Policy and
Procedures are periodically updated on the web site and the contractor will be notified by email when
updates/revisions are made. The contractor is responsible for complying with any updates/revisions.
7. The Contractor shall work collaboratively with the State for consultation,technical assistance,oversight and
training on using the Health Care Program for Children with Special Needs/Clinical Health Information Record
of Patients(HCP/CHIRP)database and to ensure that all HCP/CHIRP database users follow the HCP/CHIRP
guidance,policy and procedures available on the HCP web site:www.hcncolorado.org.
8. The Contractor shall follow all HCP/CHIRP Security policy and procedures and have a Data Security Use and
Confidentiality Agreement signed by each user.
9. The Contractor shall provide orientation and ongoing training for HCP and agency staff on using HCP CHIRP
database,HCP Guidance and HCP/TBI policies and procedures related to HCP care coordination,TBI care
coordination,specialty care,CHP+/Medicaid/SSI,family leadership and their community Medical Home
system. The HCP Team Leader shall provide a summary of training and orientation activities in HCP/CHIRP
Community Encounters.
10. The Contractor shall plan,promote and provide consultation for systems building activities to facilitate a
community medical home system using the Medical Home Action Guide document on the HCP website.
11. The Contractor shall convene at least one meeting annually of regional office staff to include contract staff and
community discipline experts. The assigned MCH Consultant and HCP state staff shall be invited to attend these
meetings. The HCP Team Leader shall provide meeting minutes in HCP/CHIRP Community Encounters.
12. The Contractor shall submit documents and reports as defined with instructions for completion and submission
dates available in the MCH Guidelines located on the MCH website:www.mchcolorado.org and the HCP
website:www.hcecolorado.org.
13. The Contractor shall assure attendance of HCP staff to state or regional HCP teleconferences and meetings that
the state may provide for addressing HCP priorities,promoting learning groups and increasing skills for HCP
practitioners.
14. The contractor shall participate in activities in conjunction with the State to ensure that the program planning,
evaluation and monitoring requirements as described in this Task Order and Exhibits are met. Activities
include,but are not limited to,participation in site visits and meetings as requested by the State at the
Contractor's location.
15. The Contractor shall cooperate with the State and provide all requested records regarding recipients for whom
services were provided under this Task Order.
