Dear Staci Datteri-Frey:
Thank you for having Banner Occupational Health Worksite Wellness at North Colorado Medical Center
provide a blood screening for employees at Weld County. We look forward to working with you on this
event as well as your future health and wellness needs.
The following services and rates shall be provided to your company effective April 4, 2008 through
October 31, 2008. It is understood that either party may terminate this Letter of Agreement at anytime
without cause or penalty upon 30 days prior written notice.
The blood screenings are scheduled for:

Services Provided:
Health Risk Assessment (HRA) with Blood Chemistry Panel and Blood Pressure Screening
The HRA, Personal Wellness Profile, is a measurement of one's current health status. It
encourages individuals' awareness of those health needs and lifestyle practices that determine
personal well-being. The individual report is combined with a Chem 24 blood chemistry panel to
help participants evaluate their current lifestyle, identify health risks and learn where and how to
make improvements. The Chem 24 Chemistry Panel includes measurements of cholesterol, HDL,
LDL, triglycerides, risk ratio, iron, thyroid, glucose, along with several heart, kidney, liver and
muscle enzymes.
This health assessment will determine areas of health risk and/or interests for your employees.
Individual reports will be sent to each participant.
Cost: $36 per person
100-200 people $35 per person
200 people or more $32.25 per person
Weld County will cover the cost and/or collect money for this screening from employees.
Blood Panel and Blood Pressure (no HRA)
Cost: $36 per person
100-200 people $34 per person
200 people or more $31 per person
Weld County will cover the cost and/or collect money for this screening from employees.
Prostate Specific Antigen (PSA) Blood Screening
This is a blood screening for men. The American Urological Association and the Prostate Cancer
Education Council recommend annual prostate screenings for all men age 50 and over(earlier if
positive family history).
Cost: $25 per person
100-200 people $24/per person
200 people or more $23/per person
Weld County will cover the cost and/or collect money for this screening from employees.
Executive Summary& Economic Impact
This is a summary of the results of your group's Health Risk Assessment which outlines your
company's overall health risks. It will enable us to assist you in developing cost effective Wellness
programs that have the greatest impact on your employees.
Cost: $60
Health Risk Assessment packets including blood results will be mailed to the address listed on the
participants Health Risk Assessment or General Consent form approximately 3-4 weeks after the blood
screening.
Weld County is covering the cost and/or collecting money for the Health Risk Assessment (HRA)
with Blood Chemistry Panel and Blood Pressure Screening, Prostate Specific Antigen, and Bone
Density Screenings for each Weld County employee, who chooses to participate. No other
screenings will be offered to employees.
• Billing: Billing for all screenings will be sent directly to the company.

**Please note: It is up to the employer to collect money from individual employees who are
participating in these events. We require one check from the employer for services rendered
It is a pleasure to work with you and look forward to continuing to provide wellness services to Weld
County employees. Health Risk Assessments have been mailed and I have emailed Blood Screening and
and Fasting Instructions already; if you need anything prior to that please let me know.
If you are in agreement with the foregoing, please sign both copies and return them in the enclosed pre-
addressed envelope. They will then be reviewed and signed by a Banner representative and a fully
executed copy will be returned to you for your records.
