Dr. Carol Collier hereby files this notice of claim against the Weld County Department of
Social Services,pursuant to Colorado Revised Statues 24-10.109.
I. Claimant's Address: 405 Whitney Harbor,Windsor,Colorado 80550.
II. A concise statement of the factual tasis of the claim: Dr. Collier discovered
on or about June 12, 2008, that she has been reported on the state automated
system for a confirmed reportof I child abuse and that this slander and
defamation of character act has affected her employment. Dr. Collier has now
filed an appeal with the state. Dr. Collier, a licensed psychiatrist, recently
accepted employment with Colorado Mental Health Institute. After accepting
employment she has learned that Weld County Department of Social Services
by placing her name on the state automated system for a confirmed report of
child abuse will result in the loss of employment and may affect her license.
Further it will affect her ability to prpvide for her children. The action of the
Weld County Department of Social Services results in slander and defamation
of character,

IV, A concise statement of the nature the extent of theiniury cl}irned to hive

resulted in slander and defamation of Dr. Collier's character, which has
affected her employment.
V. Dr. Collier request monetary damage in the amount of $7,000,000.
