Please be advised that the Fire District is in receipt of a letter dated 06/04/08 from Kristie
Fisher whereas it states that they have complied with all of the below sated requirements .
This is satisfactory for the Fire Department approval .
1 . The District prefers that each individual lot have its own access ; accesses shall be
designed to carry a designed wheel load of 18000 pounds (80000 pound GVW)
(H2O loading) . This access shall be specifically for the proposed future house.
2 . The access shall be constructed of a permanent all weather road base.
3 . Addresses shall be posted clearly on the road fronting the property.
4 . Addresses shall be posted clearly on the house.
5 . Buildings that are located more than 150 ' from an approved access road are
subject to alternative requirements .
6 . Because this facility is going to be open to the public there will need to be
available adequate fire flows within 1000 ' of this facility.

Dear Mrs. Fisher,
This letter is in follow up to the site inspection conducted on November 1, 2007 by myself and
Frank Piacentino. During that visit we discussed the outstanding Conditions of Approval of your
USR permit. Since that visit, almost every condition remains uncompleted and no action was
taken or correspondence received by you. Not only is your permit approval contingent upon your
completing the Conditions of Approval but it is also subject to the following according to the
resolution passed by the Board of County Commissioners.
Upon completion of 1., the applicant shall submit a Mylar plat along with all other
documentation required as Conditions of Approval. The Mylar plat shall be recorded in the
office of the Weld County Clerk and Recorder by Department of Planning Services' Staff.
The plat shall be prepared in accordance with the requirements of Section 23-2-260.D of the
Weld County Code. The Mylar plat and additional requirements shall be submitted within one
hundred and eighty (180) days from the date of the Board of County Commissioners
resolution. The applicant shall be responsible for paying the recording fee.
The deadline for completing your Conditions of Approval and recording the plat was October 8,
2007. This case will be brought back before the Board of County Commissioners with a
recommendation that the approval be rescinded if action is not taken immediately to comply with
the Conditions of Approval.
Please provide in writing to the Department of Planning Services a timeline for completion of the
Conditions of Approval no later than Friday December 28, 2007. Should this proposal be
approved and the action plan adhered to the case will not be brought back before the Board of
County Commissioners.

one uncompleted Condition of Approval. The requirement states:
The applicant shall obtain a building permit for a change of use, to insure the
kennel is consistent with building codes as stated in the referral received January
1, 2007 from the Weld County Building Inspection. Evidence of approval shall be
submitted to the Department of Planning Services.
The deadline for completing your Conditions of Approval and recording the plat was October 8,
2007. This case will be brought back before the Board of County Commissioners with a
recommendation that the approval be rescinded if action is not taken immediately to comply with
the Conditions of Approval. Please submit the required building permit application to the
Department of Planning Services no later than Friday, April 25, 2008.
