We look forward to working with you on this event as well as your future health and wellness needs. The following services and rates shall be provided to your company effective May 12, 2008 through October 31, 2008. It is understood that either party may terminate this Letter of Agreement at anytime without cause or penalty upon 30 days prior written notice. The health fair is scheduled for: Wednesday, May 21, 2008 Blood Screenings 7:30 a.m. to 8:00 a.m. Greeley Health Fair 9:00 a.m. to 1:00 p.m. & 5:00 p.m. to 7:00 p.m. To be held at: 4H Building Island Groove Wednesday,June 4, 2008 Blood Screenings 7:30 a.m. to 8:00 a.m. Fort Lupton Heath Fair 8:00 a.m. to 12:00 p.m. To be held at: Grater Shed Wednesday, June 4, 2008 Longmont Health Fair 2:00 p.m. to 6:00 p.m. To be held at: South County Complex Skin Cancer Screenings Melanoma is the most serious form of skin cancer. If diagnosed and removed early, it is almost 100 percent curable. Skin cancer screenings should be part of everyone's preventive health care. Our Cvn501I Agenda, 2008-1582 ilk Or7 ' qualified medical personnel can provide this service as part of a company health fair or as a stand-alone service. Cost: $165 per hour- May 21' and June 4th (Depending on availability) Weld County will cover the cost and/or collect money for this screening from employees. Hearing Screening 24 million Americans (one in ten)have hearing loss. You can't reverse hearing loss. However, you don't have to live in a world of less distinct sounds. A hearing screening can provide early detection of hearing loss. This screening is performed by a CAOHC certified professional. An audiogram will be used to screen a person's degree of hearing loss and type of hearing loss present. This screening is used as a tool to evaluate one's hearing and is not intended to be used as a diagnosis. Cost: $12 per person - June 4`h Only Weld County will cover the cost and/or collect money for this screening from employees. Health Fair Exhibits The following educational services will be provided to employees at the health fair: * Heart Healthy Living * Germs Cost: $100 for two booths, per day—May 21" & June 4`h *** a one time travel fee of$75 will be added for the South City locations Weld County will cover the cost and/or collect money for this screening from employees. Nutrition Demonstration Our R.D. will be present to make sample size smoothies for participants. A general nutrition booth with handouts and displays is included. Cost: $275 per day—May 21"Only Weld County will cover the cost and/or collect money for this screening from employees. Summary& Referral Summary and Referral is a service available to your health fair participants who wish to review the results of their individual Wellness Profile and lab work and/or have questions about making healthier lifestyle choices. This service will be provided by our Registered Nurse and will be available at the health fair, approximately three presentations will be held per hour with time for questions. Cost: $65 per RN/hr Weld County will cover the cost and/or collect money for this screening from employees. Billing: Billing for all screenings will be sent directly to the company.
