Thank you for having Banner Occupational Health Worksite Wellness at North Colorado Medical Center
provide a Lunch 'n Learn Presentation for employees at Weld County Government. We look forward to
working with you on this event as well as your future health and wellness needs.
The following services and rates shall be provided to your company effective August 15, 2008 through
October 30, 2008. It is understood that either party may terminate this Letter of Agreement at anytime
without cause or penalty upon 30 days prior written notice.
Lunch 'n Learns
The Healthy Meeting Lunch 'n Learn is scheduled on the following dates:
Thursday, October 2, 2008
11:30pm- 1:00pm
*Please note: We will arrive approximately 30 minutes early to set-up for the event.

Group presentations are offered on-site to increase awareness and knowledge of a health topic.
Educational programming can be tailored to your companies health & wellness needs. Presenters will
allow time for question and answer at the completion of the presentation.
Cost: $97.50 + Food Cost
