This Agreement updates a previous agreement for the Weld County General Hospital and Aims Community College to make available their health awareness programs at various Senior Nutrition congregate meal sites in Weld County. MEMORANDUM OF AGREEMENT WELD COUNTY DIVISION OF HUMAN RESOURCES SENIOR NUTRITION' PROJFCt ! 7 7957 WELD COUNTY GENERAL HOSPITAL j 1 AIMS COMMUNITY COLLEGE -- This Agreement is made this 7th day of October _ 1981 by and between the Weld County Division of Human Resources Senior Nutrition Project, having its office in Greeley, Colorado, hereinafter designated as the "PROJECT" and Weld County General Hospital, a not for profit agency, and Aims Community College, a not for profit agency hereinafter designated as the "PROGRAM". WITNESSETII: WHEREAS, the parties desire to work together for the express purpose of providing supportive services to the senior citizens of Weld County, and WHEREAS, to implement these goals, the parties desire to establish an Agreement for the use of the Health Awareness Program of the Weld County General Hospital and Aims Community College for the Weld County Senior Nutrition Project; NOW THEREFORE, in consideration of the implementation of these goals, the parties hereto covenant and agree as follows: 1. The "PROGRAM" will provide qualified part-time instructors to teach the Health Awareness Program at designated nutrition sites. 2. The "PROGRAM" will provide financial support for mileage for instructors, instructional materials, audio-visual materials, equipment, publicity and hand- outs. 3. Aims Community College will waiver tuition fees and award college credit to all participants who qualify and satisfactorily complete the Health Awareness class. 4. The "PROJECT" will recruit participants, coordinate the scheduling of classes at nutrition sites, and assist with publicity and secretarial services. MEMORANDUM OF AGREEMENT Page Two S. Coordination and planning meetings between PROJECT and PROGRAM staff will occur regularly to insure quality services. 6. This Agreement may be terminated by either party upon a thirty (30) day written notice. 7. This Agreement is effective from September 15, 1981, until September 14, 1982.
