Bridge 87/42.5 Replacement
Project Description: Replacement of existing steel bridge on WCR 87 over the South Platte River near
Masters, Colorado. HNTB Corporation has completed the design of the new bridge with construction
scheduled for July 2008.

The following change is hereby made to the Contract Documents:
Contract Time Extension: The original Agreement for Professional Services is extended until such time
that construction is completed including all required construction documentation or until such time that
Weld County notifies HNTB in writing that services are no longer required.
Change to Contract Scope: Engineering Construction Services shall be added to the Contract. A detailed
description and cost breakdown of these services is attached.

General
The work consists of construction management services required to support Weld County (OWNER) in
Construction Administration for the Weld County Road 87/42.5 Bridge replacement over the South Platte
River.
Parties involved include:
Prime Consultant/Engineer of Record (CONSULTANT): HNTB Corporation
Resident Project Representative (RPR): Yeh &Associates, Inc.
Materials Testing: Yeh &Associates, Inc.
Contractor(CONTRACTOR): American Civil Constructors
1. Project Coordination
A. Project Management. The CONSULTANT will coordinate all the work tasks being accomplished
described here in to ensure project work completion stages are on schedule.
B. The CONSULTANT will prepare monthly invoice progress reports.
2. Construction Administration
A. Project Management. The CONSULTANT will coordinate all the work tasks being accomplished as
described herein to ensure project work completion stages are on schedule. Includes on-site
attendance every other week at weekly construction meetings.
B. CONSULTANT/Resident Project Representative
Field inspection in conformance with CDOT protocol, including CDOT Local Agency Manual and
CDOT Construction Manual, during the construction period of July 15, 2008 to November 15, 2008.
Construction is anticipated to occur 12 hrs/day, 5 days/week with resident project representative
on site an average of 6 hrs/day,5 days/week (an additional 2 hrs/day assumed for drive time).
Duties, Responsibilities, and Limitations of Authority of CONSULTANT/Resident Project
Representative:
Through observations of CONTRACTOR's work in progress and field checks of materials and
equipment by the RPR,CONSULTANT shall endeavor to determine if CONTRACTOR's work is in
accordance with contract documents. However, CONSULTANT/RPR shall not,during such visits
or as a result of such observations of CONTRACTOR's work in progress,supervise,direct,or have
control over the CONTRACTOR's work nor shall CONSULTANT/RPR have authority over or
responsibility for the means, methods,techniques,sequences,or procedures selected by
CONTRACTOR,for safety precautions and programs incident to the CONTRACTOR's work in
progress,for any failure of CONTRACTOR to comply with Laws and Regulations applicable to
CONTRACTOR's performing and furnishing the Work,or responsibility of construction for
CONTRACTOR's failure to furnish and perform the Work in accordance with the Contract
Documents.
The duties and responsibilities of the RPR are limited to those of CONSULTANT in the Agreement
with the OWNER and in the Contract Documents,and are further limited and described as
follows:

RPR is OWNER's agent at the Site,will act as directed by and under the supervision of
CONSULTANT,and will confer with CONSULTANT regarding RPR's actions. RPR dealings in
matters pertaining to the CONTRACTOR's work in progress shall in general be with
CONSULTANT and CONTRACTOR, keeping OWNER advised as necessary. RPR shall
generally communicate with OWNER with the knowledge of and under the direction of
CONSULTANT.
ii) Schedules:
Review the CONTRACTOR'S progress schedule,schedule of Shop Drawing and Sample
submittals,and schedule of values prepared by CONTRACTOR and consult with
CONSULTANT concerning acceptability.
Hi) Conferences and Meetings:
Attend meetings with CONTRACTOR,such as preconstruction conferences,progress
meetings,job conferences and other project-related meetings,and prepare and circulate
copies of minutes thereof.
iv) Liaison:
• Serve as CONSULTANT's liaison with CONTRACTOR, coordinate with
CONTRACTOR's superintendent and assist in understanding the intent of the
Contract Documents.
• Assist CONSULTANT in serving as OWNER's liaison with CONTRACTOR when
CONTRACTOR's operations affect OWNER's on-Site operations.
• Assist in obtaining from CONSULTANT additional details or information,when
required for proper execution of the Work.
v) Interpretation of Contract Documents:
Report to CONSULTANT when clarifications and interpretations of the Contract Documents
are needed and transmit to CONTRACTOR clarifications and interpretations as issued by
Engineer.
vi) Shop Drawings and Samples:
▪ Record date of receipt of Samples and Shop Drawing submittals.
• Receive Samples which are furnished at the Site by CONTRACTOR, and notify
CONSULTANT of availability of samples for examination.
• Advise CONSULTANT and CONTRACTOR of the commencement of any portion of
the Work requiring a Shop Drawing or Sample submittal for which RPR believes
that the submittal has not been approved by Engineer.
vii) Modifications:
Consider and evaluate CONTRACTOR's suggestions for modifications in Drawings or
Specifications and report with RPR's recommendations to CONSULTANT.Transmit to
CONTRACTOR in writing decisions as issued by CONSULTANT.
viii) Review of Work and Rejection of Defective Work:
• Conduct on-Site inspection and testing of CONTRACTOR's work in progress to
assist CONSULTANT in determining if the Work proceeding in general conformance
with the Contract Documents.
• Report to CONSULTANT and/or OWNER whenever RPR believes that any part of
CONTRACTOR's work in progress will not produce a completed Project that
conforms generally to the Contract Documents or will prejudice the integrity of the
design concept of the completed Project as a functioning whole as indicated in the
Contract Documents,or has been damaged,or does not meet the requirements of
any inspection,test or approval required to be made;and advise Engineer of that

part of work in progress that RPR believes should be corrected or rejected or
should be uncovered for observation, or requires special testing,inspection or
approval.
ix) Inspections and Tests:
• Observe,record and report to CONSULTANT appropriate details relative to the test
procedures.
• Accompany visiting inspectors representing public or other agencies having
jurisdiction over the Project, record the results of these inspections,and report to
CONSULTANT.
x) Materials Testing
• Provide and be responsible for materials testing for the project as outlined in the
attached CDOT Form 250.
xi) Records:
All project records shall be in accordance with the CDOT Local Agency Manual.
• Maintain orderly files for correspondence, reports of job conferences,
reproductions of original Contract Documents including all Change Orders, Field
Orders, Work Change Directives,Addenda,additional Drawings issued subsequent
to the execution of the Contract,CONSULTANT clarifications and interpretations of
the Contract Documents,progress reports,Shop Drawings and Sample submittals
received from and delivered to CONTRACTOR,and other Project related
documents.
• Keep a diary or log book for each day that the RPR is present and construction
services are performed on the project, recording CONTRACTOR's hours on the
Site,weather conditions,data relative to questions of Change Orders, Field Orders,
Work Change Directives,or changed conditions, Site visitors,daily activities,
decisions,observations in general,and specific observations in more detail as in
the case of observing test procedures;and send copies to CONSULTANT.
• Record names,addresses and telephone numbers of all contractors,
subcontractors,and major suppliers of materials and equipment.
• Maintain records for use in preparing project documentation.
• Upon completion of the Work and full project close out,furnish original set of all
RPR Project documentation to CONSULTANT. Upon review and concurrence
CONSULTANT to submit to OWNER.
xii) Reports:
• Furnish to CONSULTANT periodic reports (bi-weekly)as required of progress of the
Work and of CONTRACTOR's compliance with the progress schedule and schedule
of Shop Drawing and Sample submittals.
• Draft and recommend to CONSULTANT proposed Change Orders,Work Change
Directives,and Field Orders.Obtain backup material from CONTRACTOR.
• Furnish to CONSULTANT copies of all inspection reports.
• Report immediately to CONSULTANT and/or OWNER the occurrence of any Site
accidents,any Hazardous Environmental Conditions, emergencies, or acts of God
endangering the Work,and property damaged by fire or other causes.
xiii)As-Built Drawings: Provide final "as-built"drawings,as red-lined by RPR in the field.
Electronic CAD"as-built"drawings will not be provided.
xiv)Payment Requests:

procedure for their submission and forward with recommendations to CONSULTANT, noting
particularly the relationship of the payment requested to the schedule of values,Work
completed,and materials and equipment delivered at the Site but not incorporated in the
Work. Upon concurrence CONSULTANT to submit to OWNER.
xv) Certificates:
During the course of the Work,verify that materials and other data required by the
Specifications to be assembled and furnished by CONTRACTOR are applicable to the items
actually installed and in accordance with the Contract Documents,and have these documents
delivered to CONSULTANT for review and forwarding to OWNER prior to payment for that
part of the Work.
xvi)Completion:
• Before CONSULTANT issues a Certificate of Substantial Completion,submit to
CONTRACTOR a list of observed items requiring completion or corrections.
• Observe whether CONTRACTOR has arranged for inspections required by Laws and
Regulations,including but not limited to those to be performed by public agencies
having jurisdiction over the Work.
• Participate in a final inspection in the company of CONSULTANT,OWNER,and
CONTRACTOR and prepare a final list of items to be completed or corrected.
➢ Determine whether all items on final list have been completed or corrected
and make recommendations to CONSULTANT concerning acceptance and
issuance of the Notice to Acceptability of the Work.
xvi)Resident Project Representative shall not:
• Authorize any deviation from the Contract Documents or substitution of materials
for equipment(including "or-equal" items).
• Exceed limitations of CONSULTANT's authority as set forth in the Agreement or the
Contract Documents.
• Undertake any of the responsibilities of CONTRACTOR,subcontractors,suppliers,
or CONTRACTOR's superintendent.
• Advise on, issue directions relative to or assume control over any aspect of the
means, methods,techniques,sequences or procedures of CONTRACTOR's work
unless such advice or directions are specifically required by the Contract
Documents.
• Advise on,issue directions regarding,or assume control over safety precautions
and programs in connection with the activities or operations of OWNER or
CONTRACTOR.Participate in specialized field or laboratory tests or inspections
conducted off-site by others except as specifically authorized by CONSULTANT.
• Accept Shop Drawing or Sample submittals from anyone other than CONTRACTOR.
• Authorize OWNER to occupy the Project in whole or in part.
• Be required to be on-site full time.
C. Submit monthly progress reports including site visit diary entries and inspection logs to OWNER.
D. Process periodic pay requests and submit to OWNER for payment.
E. Review of submittals and RFI's.

Re: Resident Project Representative and Quality Assurance Testing for Weld County Bridge on
WCR 87
Dear Ms:Leiphart:
Yeh and Associates is pleased to submit this proposal to provide the Resident Project Representative
(RPR)and materials testing relative to the above referenced project. Our proposal includes an outline of
the project information provided to us,the proposed scope of services,and our estimated fees.
PROPOSED CONSTRUCTION
We understand that the project will construct a new bridge replacing the existing bridge on Weld County
Road 87,as well as new road approaches and pavement. We understand that the materials to be tested
include 24,495 cubic yards of earthwork, 154 cubic yards of structural backfill,2,545 tons of aggregate
base course(ABC),645 tons.of Hot Mix Asphalt(HMA)pavement,and structural concrete for the bridge
structure;caissons,and gutter as well as other misr.ellaneous concrete. Flow fill will also be tested as part
of the roadway improvements.
RESIDENT PROJECT REPRESENTATIVE
Yeh and Associates will provide one Resident Project Representative(RPR)to perform duties in
accordance with the attached Scope of Services

Estimated Hours are seven-hundred-twenty(720)at$75.00/hour= 554,000.00
One RPR to provide the services delineated in the contract Scope of Services. Our estimate is that ninety
(90)trips will be required at 130 miles per trip®$0.585/mile:

PROJECT OUALITY ASSURANCE TESTING
Based on our understanding of the project,the following quality assurance material testing services may
be required for the proposed construction.
• Field density-compaction testing and laboratory testing of soils and aggregates for embankment,
structural backfill and base course.
• Field and laboratory testing of concrete and flow fill
• Field and laboratory testing of Hot Mix Asphalt(HMA)pavement

Field Density—Compaction Testing:An engineering technician will be provided on a part-time basis or
as requested to perform materials testing services for earthwork. We understand that testing will consist
of moisture-density relationships,one point checks,gradations,Atterberg Limits and compaction testing
with a nuclear density gauge.
Concrete: An engineering technician will be provided on a part-time or as requested basis to sample fresh
concrete used during construction.The concrete will be tested for slump,air content,unit weight,and
temperature at the time of placement. Test cylinders will be molded at the time of placement and
compressive strength testing will be performed at the Yeh lab to determine compliance with the project
specifications. Test results,will be provided in the field for review and compressive strength test results
will be conveyed as available. The testing,frequency will be daily and as scheduled. The areas to be
addressed are shuctu al concrete from the major structural elements,caissons,gutter and flow fill.
Hot Mix Asphalt: Field density-compaction testing by nuclear methods will be provided by an
engineering technician,on an as-scheduled basis,during placement of the Hot Mix Asphalt(IHMA)
pavement. Test samples will be taken to supplement the quality control testing being performed by the
asphalt supplier. A Yeh representative will obtain samples for laboratory tests as required. Daily reports
of test results will be available on-site for review. Cores,if required will be taken by the contractor and
tested for density in the lab by a Yeh engineering technician.
All testing frequencies will be in accordance with the current Colorado Department of Transportation
(CDOT)Field Materials Manual Quality Control Schedule and the form 250 for the project
Engineering.Supervision and Clerical: Engineering,supervision and clerical services will be provided as
required for the project and to prepare formal reports.
Testing Cost: We understand the project will be completed over two years,starting in July of 2008 with a
suspension in the early spring and completion in 2009.We anticipate that one technician will be required
on.an on-call basis,and that the average technician work day will consist of six to eight hours including
travel time.Those hours may increase dining an activity such as a bridge deck pour depending on the
amount of testing required. Our estimated fee is as follows:
Earthwork
One field technician providing three trips with 2-3 density tests per trip as well as sampling soil for lab
testing. We estimate that five(5)trips will be required. 6 hours per trip @$65/hour plus 130 miles per
trip @$0.585/mile:

One field technician providing thirteen trips with a minimum of one test per trip,but more when larger

pours are constructed. We estimate that thirteen(13)trips will be required. 7 hours per trip @$65/hour
plus 130 miles per trip @$0.585/mile: (7 hours is an average,where one sample is required,the time
will be less,and for large pours such as the bridge deck,more hours will be required)

Hot Mix Asphalt
One field technician providing two(2)trips with 2-3 density tests per trip as well as samples for lab
testing. We estimate that two(2)trips will be required. 6 hours per trip @$65/hour plus 130 miles per
trip @$0.585/mile:

The fee and hourly based worksheet estimates were generated with the understanding that Yeh and
Associates,Inc.would be performing the duties of Quality Assurance(QA)on the project. The'fees are
based upon our understanding and requirements of the project,CDOT'sperifications,and our experience
on similar projects. If services performed exceed the work described above,additional costs will be
invoiced at the respective unit rates after written authorization from the client. The unit rate for services
requited beyond the scope presented above will be in accordance with the 2008-2009 Yeh and Associates,
Inc.standard rates.
ENGINEERING MANAGEMENT OVERSIGHT:
We anticipate that Engineering Management and Oversight for both the QA testing and the on-site
manager will require 7 on site visits and a total of 42 hours.
The anticipated cost of this service is based on seven(7)visits at 130 miles per visit,and forty-two(42)
hours at$135.00/hour for a total of:

COLORADO SUBJECT: Bridge 87/42.5— Change Order No. 1
Attached are two original Change Orders for our design engineer's current contract.
HNTB designed the bridge project and our contract allows PW the option to include
construction services if desired. PW does not the ability to inspect a federal grant
project with its in-house resources and requires outside services for this CDOT type of
inspection.
The costs to perform these services are $114,527.15 which includes one full time
inspector, materials testing, and part time management support as needed. A detailed
breakdown of these fees is attached to the change order. PW is recommending
approval for this change order.
