HomeMy WebLinkAbout20081658.tiff RESOLUTION RE: APPROVE GRANT AWARD FOR WOMEN'S WELLNESS CONNECTION PROGRAM AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Grant Award for the Women's Wellness Connection Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, with terms and conditions being as stated in said award, and WHEREAS, after review, the Board deems it advisable to approve said award, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Grant Award for the Women's Wellness Connection Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said award. Wallace, MD, MPH, Director Depaitnient of lic Health a Environment '' � CAA: COLORADO DATE: June 3, 2008 SUBJECT: Women's Wellness Connection Provider Contract Enclosed for Board review and approval is a grant award letter from the Colorado Department of Public Health and Environment Women's Wellness Connection Program (WWC) indicating the final amount of expansion grant funding for the Weld County Department of Public Health and Environment(WCDPHE). Funding for the WWC provider services is available from federal and state funds. Weld County Department of Public Health and Environment will make available a .2 FTE Public Health Nurse to assist women enrolled in WWC with the cancer screening process and follow up of abnormal diagnostics. The grant will also fund recruitment, education, translation, and navigation services for the program as well as a 1.0 FTE Community Service Specialist to coordinate the administrative aspects of the grant implementation. For providing the services described above during the time period July 1, 2008 through June 30, 2009, the Colorado Department of Public Health and Environment will pay Weld County an amount not to exceed $69,974.48. On behalf of the program, we are pleased to inform you that your grant application has been approved for funding for fiscal year 2008-2009. Congratulations! The process to evaluate your applications was intense and we feel this resulted in all sites having a fair chance at receiving a grant. Included in the process was a technical review for compliance with the Request For Applications (RFA) requirements, an assessment conducted by a volunteer rating/screening team comprised of public health and health care professionals from the health department and outside agencies, as well as a review and recommendation to fund each grant by the Breast and Cervical Cancer Screening Advisory Board. The Women's Wellness Connection received nineteen applications and has awarded nearly $650,000 for the upcoming grant year- In order to keep our awards within a limited expansion grant budget, some changes to original requests had to be made. This included limiting "indirect costs" to 10% maximum for all agencies and specifically removing "training and education" costs for colposcopy. As part of the program's goal for 2008-09, the program will be hosting colposcopy training in October to a select number of providers. If your site had requested colposcopy training and was not funded, please contact Shirley Hass, Nurse Consultant (303-692-2323 or Shirlcy.hass(a>state.co.us) to discuss this additional opportunity for your agency. Your grant award is explained on the following page. Please follow the included instructions on how to proceed with the contract process. Please do not begin work on your project prior to receiving an executed contract or purchase order; the department cannot reimburse you for work done prior to the contract being fully executed.
