Home My WebLink About20083072.tiff RESOLUTION RE: APPROVE MEMORANDUM OF UNDERSTANDING FOR QUALISTAR RATING PROJECT AND AUTHORIZE CHAIR TO SIGN - UNITED WAY OF WELD COUNTY WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Memorandum of Understanding for the Qualistar Rating Project between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Family Educational Network of Weld County, and United Way of Weld County, commencing upon full execution, and ending February 28, 2009, with further terms and conditions being as stated in said memorandum, and WHEREAS, after review, the Board deems it advisable to approve said memorandum, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Memorandum of Understanding for the Qualistar Rating Project between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Family Educational Network of Weld County, and the United Way of Weld County be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said memorandum. Griego, Director, Human Sre f ptm COLORADO RE: Memorandum of Understanding between the Weld County Department of Human Services' Family Educational Network and United Way of Weld County Enclosed for Board approval is a Memorandum of Understanding between the Weld County Department of Human Services' Family Educational Network and United Way of Weld County for the Temple Boyne Buell Foundation Qualistar Rating Project. The Application Form was approved at the Board's September 8,2008,Work Session and the September 24,2008,Board meeting. The Grant Award was presented at the October 27, 2008, Board meeting. This Memorandum of Understanding appropriates$1,640.00 of the$8,840.00 to United Way of Weld County from the Family Educational Network. It outlines United Way's responsibilities to prepare FENWC staff to meet the Qualistar Quality Star Rating. Allowed expenditures include: Travel,Notebook and copies,Orientation to the Ratings Training, Getting Ready for Ratings Training and Coaching. This portion of the Grant must be completed no later than February 28, 2009. If you have questions, please give me a call at extension 6510. 2008-3072 Memorandum of Understanding Between United Way of Weld County And Family Educational Network of Weld County This memorandum of understanding dated November 14,2008 between United Way of Weld County(herein referred to as UWWC)and Family Educational Network of Weld County(herein referred to as FENWC)outline the terms and conditions made between the above listed parties for the$1640.00 Temple Hoyne Buell Foundation funds for the Head Start sites of Platteville, Gilcrest,Dos Rios and Madison. This funding is appropriated for the Orientation/Getting Ready for Ratings of the Qualistar Quality Star Rating. The funds will be expended as follows: Travel,Notebook and copies, Orientation to the Ratings Training,Getting Ready for Ratings Training and Coaching. UWWC agrees to: • Provide Orientation to the Ratings and Getting Ready for Ratings training(1 hour preparation and 5 hours of training) for staff members. • Provide 6 hours of coaching in preparation for the Qualistar Quality Star Rating FENWC agrees to: • Have the entire staff fully participate in the Orientation training,Getting Ready for Ratings training, and coaching in preparation for the Qualistar Rating. • Follow through with the recommendations and coaching from the Getting Ready for Ratings training and complete the Qualistar rating no later than February 28,2009. • Pay United Way of Weld County$1640.00 for the Orientation to the Ratings and the Getting Ready for Ratings training and coaching. • Request that Qualistar send to UWWC a copy of the Quality Performance Profile/Quality Improvement Plan (QPP/QIP). i.
