RE: APPROVE AMENDMENT TO COLORADO WORKS AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - NORTH RANGE BEHAVIORAL HEALTH
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Colorado Works
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
North Range Behavioral Health, commencing November 1, 2007, and ending June 30, 2008, with
further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services,that the Amendment to the Colorado Works
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
North Range Behavioral Health be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 7th day of January, A.D., 2008, nunc pro tunc November 1, 2007.

RE: Amendment to Colorado Works AgreemenbnI Weld
County Department of Social Services and North Range
Behavioral Health—Multi-Disciplinary Youth Assessment Team
(MYAT)
Enclosed for Board is an Amendment to Colorado Works Agreement between the Weld County
Department of Social Services and North Range Behavioral Health. This Amendment was
reviewed at the Board's Work Session held on November 5, 2007.
The purpose of the Amendment is to increase the hours of their Full Time Equivalent(FTE)from
32 Hour to a 40 Hour work week. This increase will result in an additional expenditure of
$5,713, which will result in a total compensation level of$42,848.
All other provisions of the Agreement will remain the same including the term of the Agreement
of July 1, 2007 through June 30, 2008.

AMENDMENT TO COLORADO uORRS AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
AND NORTH RANGE BEHAVIORAL HEALTH
This Amendment, made and entered into the 29th day of October 2007, by and between the Board of Weld '
County Commissioners, on behalf of the Weld County Department of Social Services, hereinafter referred to as
"Social Services", and North Range Behavioral Health, hereinafter referred to as"North Range Behavioral Health."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(PY07-08-CWP-25)effective November
1,2007,to modify North Range's weekly participation in the Multidisciplinary Youth Assessment Team,and the fee
Social services agrees to pay the MYAT contractor for a period beginning November 1,2007 and ending June 30,
2008;and
WHEREAS,required approval,clearance, and coordination have been accomplished from and with
appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and agree as follows:
1. Exhibit A,Roles of Contractor,Item B,will be revised as follows:
North Range,a 40 Hour per week FTE ending June 30,2008.
2. Exhibit C,Overall Cost of Phase VIII,Item A,will be revised as follows:
North Range, a 40 hour per week FTE: $42,848.
3. Effective November 1,2007, for an additional expenditure of$5713.00.
Except as modified herein,the terms and conditions of the Original Agreement,together with all
attachments of the Original Agreement,remain in full force and effect.
