RE: APPROVE GRANT AWARD FOR QUALISTAR RATING PROJECT AND AUTHORIZE
CHAIR TO SIGN - TEMPLE HOYNE BUELL FOUNDATION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Grant Award for the Qualistar Rating
Project to the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Family
Educational Network of Weld County, from the Temple Hoyne Buell Foundation, with terms and
conditions being as stated in said grant award, and
WHEREAS,after review,the Board deems it advisable to approve said grant award,a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Grant Award for the Qualistar Rating Project to the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf
of the Department of Human Services, Family Educational Network of Weld County, from the
Temple Hoyne Buell Foundation be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said grant award.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 27th day of October, A.D., 2008.

COLORADO RE: Grant Award for Temple Hoyne Buell Foundation
Qualistar Rating Project for the Weld County Department of
Human Services' Family Educational Network
Enclosed for Board approval is notification that the Application for Temple Hoyne Buell
Foundation Qualistar Rating Project for the Weld County Department of Human
Services' Family Educational Network has been funded. The Application Form was
approved at the Board's September 8,2008,Work Session and the September 24,2008,
Board meeting.
A grant of $8,840.00 has been approved to be used as follows: $1,640.00 for
Orientation/Getting Ready for Ratings and $7,200 for Qualistar to rate six (6)
classrooms. The classrooms targeted by FENWC will be in Gilcrest, Greeley, and
Platteville for a total of six(6)classrooms. Additionally,up to$12,000.00(maximum of
$2,000.00 per classroom)will be awarded for quality improvements. The exact amount
will be determined after the rating has been completed.

Dear Dr. Flaugher:
Congratulations! Family Educational Network of Weld County has been chosen to participate in
the Buell Foundation's Qualistar Rating Project. A grant of$8,840.00 has been approved to be
used as follows: $1,640 for Orientation/Getting Ready for Ratings and $7,200 for Qualistar to
rate six classrooms. Additionally, up to $12,000 (maximum of$2,000 per classroom) will be
awarded for quality improvements. The exact amount will be determined after the rating has
been completed.
This grant has been approved based upon the following terms and conditions:
1. Upon receipt of this contract, Family Educational Network of Weld County will
immediately call Laura Colburn, Director of Qualistar Ratings, (303-339-6816) to
schedule the Qualistar rating to occur no later than May 31, 2009, unless a delay in
scheduling is beyond the control of Family Educational Network of Weld County. If
the rating cannot be scheduled within this time frame, Family Educational Network of
Weld County must provide a written explanation to the attention of Marilie Hart at
the Buell Foundation.
2. Within two weeks of receipt of this contract, Family Educational Network of Weld
County will contact Sheila Watson at 970-353-4300 to schedule Qualistar
Orientation/Getting Ready for Ratings.
3. When completing the Qualistar application, Family Educational Network of Weld
County must include the Buell Foundation in the list of entities that will receive a
copy of the Qualistar report. In the Release of Information section, under"Other,"
please enter the following email address: mhart@buellfoundation.org.
4. Family Educational Network of Weld County must notify Erin Binford with the Buell
Foundation when the Qualistar rater schedules the debriefing meeting to allow Erin
the option of attending. Erin will provide a form to request the quality improvement

grant (maximum of$2,000 per classroom). Items and/or services to be purchased
with the quality improvement grant must address specific Qualistar recommendations.
5. This grant is conditioned upon your acceptance of the terms set forth above, and the
Buell Foundation reserves the right to discontinue, modify, or withhold any payment
under this grant award, or to request a refund of any grant funds, if it reasonably
determines that Family Educational Network of Weld County has not fully complied
with the terms and conditions of this grant.
If the conditions of this grant meet with your approval, please sign and return one copy of this
letter with original signatures to the Buell Foundation within 14 days after its receipt. A check
for $8,840.00 will be issued within three weeks of our receipt of the signed contract.
