Terracon Consultants, Inc. (Terracon) is pleased to submit this proposal for providing
construction materials testing services for the referenced project. This proposal includes an
outline of the project information provided to us, the proposed scope of services, our
estimated fees and the proposed schedule.
A. PROJECT INFORMATION
As we understand, this phase of the project will consist of road improvements on Weld
County Road 2. The roadway will include class 6 road base with HMA mat and drainage
improvements. The project is located at Weld County Road 2 and I-76, Lochebuie, Colorado.
B. PROPOSED SCOPE OF SERVICES
Our understanding of the required construction materials testing services for this project is
based upon information provided by your firm. The general services estimated for this
project are listed below, followed by the specific scope of services for each service type:
• Field density-compaction testing of soils;
• Field testing and laboratory evaluation of concrete;
• Field testing of asphalt-concrete.

Our services will be provided on an as-requested basis with scheduling by the Contractor or
owner's representative. Since our personnel will not be at the site on a resident basis, it will
be imperative that we be advised when to provide services on the project. Services must be
scheduled a minimum of one (1) working day in advance. Unless other arrangements are
made, scheduling of personnel will be on an as available basis, which may require changes
in personnel assigned to the project. Terracon will not be responsible for scheduling our
services and will not be responsible for tests that are not performed due to a failure to
schedule our services on the project. The detailed scope of services envisioned to complete
the testing is as follows:
Field Density-Compaction Testing: Field density-compaction testing will be performed on
an as-requested basis, during site grading, at the project. Additionally, density-compaction
testing will be performed during slab, miscellaneous site work and pavement subgrade
preparation. The field services will be supported by appropriate laboratory evaluation of
soils used as fill and/or backfill at the project.
The laboratory testing will include laboratory moisture-density relationships (Proctor).
Laboratory material evaluations will be conducted, at a minimum, for each type of soil
encountered during fill placement. The testing will be performed to determine compliance
with project specifications. The Contractor on the site will be informed of our field
observations and test results. Written reports of test results will be prepared on a regular
basis throughout the duration of the project.
Concrete Testing: An Engineering Technician will be provided, on an as-requested basis,
to sample the plastic concrete used during construction and site work as applicable. The
concrete will be tested for slump, air content and temperature at the time of placement.
Sets of four (4), 4-inch by 8-inch cylinders will be molded at each sampling. All samples will
be field cured at the site and returned to our laboratory for moist curing prior to compressive
strength testing.
Compressive strength testing will be conducted in compliance with project specifications.
All test results will be conveyed after testing is completed.
Asphalt Concrete Paving: Field density-compaction testing by nuclear methods will be
provided by an experienced Technician, on an as-requested basis, during the placement of
asphalt concrete at the project. The density-compaction of the asphalt concrete will be
evaluated utilizing information from the paving contractor's asphalt mix design. (Gradations,
Rice, Marshall — etc). Daily reports of test results will be available on-site for your review
and a written report will be prepared at the completion of the project. CDOT table below will
be followed for frequency of tests under "Frequency-Acceptance"

C. COMPENSATION
The cost for our services, including all field work, laboratory testing, engineering analyses
and report preparation has been estimated on the basis of unit rates especially prepared for
this project. Our services will be provided to give the owner a greater degree of confidence
in the work being constructed, however, these services in no way relieve the various
contractors from their responsibility for properly constructing the work. For testing and
observation services, our costs for the project are based on the following number of trips at
the durations indicated:
The time required for construction materials testing on the project will be directly related to
the schedule and performance of the various contractors on the site. While the following
phases of construction have been separated for estimating purposes, actual field testing
services may be performed simultaneously on the various phases. As a result, total fees for

construction materials testing services are based on applicable unit rates and we will invoice
you on that basis.
The estimated fees presented will include only those services outlined in the above section
of the proposal. The fees presented are inclusive of all field work, laboratory work, mileage
and report preparation required to complete standard testing requirements.

services not listed will be billed according to our current fee schedule.

Our estimated project costs are based upon our understanding of the project, information
provided by the individuals listed above, information from the project plans provided and the
State of Colorado. Additionally, the following assumptions were used in preparing our
estimated fees:
• Should additional services be required beyond the scope of work outlined above, our
standard unit rates will be applicable;

• The client and/or contractor is responsible for scheduling all testing and inspection
services with a minimum of 24 hours notice;
• Unit rates will be applicable for on-site duration's in excess of those outlined; and
• Invoices for the project will be submitted on a monthly basis.
This proposal may be accepted by executing both originals of the attached Agreement for
Services and returning one executed copy along with this proposal to Terracon. The
remaining executed copy is for your files. This agreement, including the limitations it
contains, shall constitute the exclusive terms, conditions and services to be performed for
this project. Commencement of services by Terracon will be considered acceptance of this
proposal and the attached Agreement for Services.
We appreciate your consideration of Terracon for this work, and look forward to working as
your geotechnical and materials engineering consultant on this and future projects.

REVIEW/WORK SESSION REQUEST
RE: Terracon Contract Work Order 6 for material testing on rd. 2 and new pay rate schedule for 2008.
DEPARTMENT: PUBLIC WORKS DATE: 6-16-2008
PERSON REQUESTING: Joshua Holbrook, Construction Inspection Supervisor.
Brief description of the problem/issue: Change Order No. 1 - The original agreement for Professional
Services could extended from June 18th, 2008 to June 17th, 2009 as permitted by the contract. This
extension would be in the second year of the four year term limit. The 2007 rate Schedule of the original
contract would be replaced with the attached Exhibit A, 2008 Rate Schedule.
Work Order No. 6 — Public Works is proceeding with WCR 2 Road Project from I-76 to WCR 45. We
have selected Terracon to perform theses services under the On-Call Geotechnical Contract. Terrracon
has submitted a proposal and will not exceed the cost to complete the requested services. These services
are required to ensure materials used during construction meet contract specifications for embankment,
gravel, concrete and asphalt. Attached is Work Order No. 6 outlining the scope of work and cost to
complete the work. Terracon estimates the cost for WCR 2 from I-76 to WCR 45 to be $19,337.00.
What options exist for the Board? (Include consequences, impacts, costs, etc. of options)
BOCC can either approve Work Order No. 6 and the Change Order No. 1 and continue proceeding with
the work or deny approval for Work Order No. 6 and Change Order No. 1.
Recommendation to the Board: I recommend approval of Work Order No. 6 and Change Order No. 1 for
Terracon under the On-Call Geotechnical Contract.
