RE: APPROVE ASSUMPTION OF CHILD PROTECTION AGREEMENT FOR SERVICES AND
AUTHORIZE CHAIR TO SIGN - NORTH RANGE BEHAVIORAL HEALTH
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Assumption of Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and
North Range Behavioral Health, which was previously entered into with Island Grove Regional
Treatment Center, Inc., commencing June 30, 2008, with further terms and conditions being as
stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve the assumption of said
agreement by North Range Behavioral Health, a copy of which is attached hereto and incorporated
herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Assumption of Child Protection Agreement for Services between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and North Range Behavioral Health be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 11th day of February, A.D., 2009, nunc pro tunc June 30, 2008.

Assumption of Child Protection Agreement for Services
between the Weld County Department of Human Services
and North Range Behavioral Health
Enclosed for Board approval is an Assumption of Child Protection Agreement for Services
between the Weld County Department of Human Services and North Range Behavioral Health.
Island Grove Regional Treatment Center, Inc., assigned all of its rights and obligations under the
Agreement to North Range Behavioral Health. North Range Behavioral Health agrees to assume
all obligations, responsibilities, and terms of the Agreement and become the Contractor under the
Agreement.

ASSUMPTION OF CHILD PROTECTION AGREEMENT FOR SERVICES
Whereas, Island Grove Regional Treatment Center, Inc. (the "Contractor"), and the
Board of Weld County Commissioners, sitting as the Board of Human Services, on behalf of the
Weld County Department of Human Services (the "Department"), entered into a Child
Protection Agreement for Services (PY-08-09-CPS-0094) dated June 30, 2008, incorporated
herein by this reference (the "Agreement"), wherein Contractor will provide anger management
and domestic violence services, and;
Whereas, Island Grove Regional Treatment Center, Inc. desires to assign all of its rights
and obligations under the Agreement to North Range Behavioral Health, a Colorado nonprofit
corporation ("North Range Behavioral Health"); and
Whereas, North Range Behavioral Health intends to benefit the Weld County Department
of Human Services by promising to perform all terms and conditions of the Agreement.
Now, therefore, in consideration of the Department's approval and other mutual
covenants and agreements expressed in the Agreement, North Range Behavioral Health does
hereby covenant and agree to assume all obligations, responsibilities, and terms of the
Agreement and hereby become the Contractor under the Agreement.
Further, North Range Behavioral Health submits to the Department the following notice
address pursuant to Section 15 of the Agreement:
North Range Behavioral Health

Greeley, CO 80631
Further, North Range Behavioral Health designates to the Department the following
Contractor Representative pursuant to Section 14 of the Agreement:
Wayne A. Maxwell, Executive Director
Further, North Range Behavioral Health submits to the Department herewith, the proof of
insurance as required in Section 9 of the Agreement.
