RE: APPROVAL OF CONTRACT BETWEEN WELD COUNTY DEPARTMENT OF SOCIAL
SERVICES AND ROBERT D. HARTLEY, M.D. FOR CHILD ABUSE CONSULTA-
TION SERVICES.
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, a contract for child abuse consultation services
between Weld County Department of Social Services and Robert D.
Hartley, M.D. , 1650 16th Street, Greeley, Colorado, has been
presented to the Board of County Commissioners of Weld County,
Colorado. A copy of said contract is attached hereto and incor-
porated herein by reference, and
WHEREAS, the term of said contract is from January 1 , 1979
through December 31, 1979, and
WHEREAS, the Board deems it advisable and in the best
interests of Weld County to approve said contract.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Com-
missioners of Weld County, Colorado that the contract for child
abuse consultation services between Weld County Department of
Social Services and Robert D. Hartley, M.D. , be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that paragraph 3 and
paragraph 6 of said contract may be modified provided said modi-
fications are not substantial changes and further that they comply
with state requirements.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 3rd day of
January, A.D. , 1979.

It is hereby agreed that:
2. This contract shall be effective from January 1 , 1979 and continue in
force until December 31 , 1979 and may be subject to renewal.
3. COUNTY AGREES TO PURCHASE AND CONTRACTOR AGREES TO FURNISH Consultation

SERVICE AT A COST OF $ 40.00 PER XXXIMMDIPX#XUXfM FOR SERVICES RENDERED
AT WELD COUNTY CHILD ABUSE TEAM-MEETINGS.
Contractor cannot assign provision of this service to a subcontractor.
4. County agrees:
a. To pay Contractor after satisfactory completion of services, the cost of
the above service upon receipt of the billing form provided by County
0;7WBJED06MMIX Voucher procedure.
b. To determine the eligibility for services and dispense information re-
garding rights to fair hearings prior to referral to Contractor.
c. To assure that the service described herein has been accomplished and a
report received prior to payment to Contractor.
5. Contractor agrees:
a. To hold the necessary license(s) which permits the performance of the
service to be purchased herein.
b. To comply with the requirements of the Civil Rights Act of 1964, and re-
quirements for safeguarding client information in accordance with rules
of the State Department.
c. To provide access by county, state, and federal personnel at reasonable
times to such records as are necessary to document the cost and delivery
of service.
d. To provide the service described herein at a cost not greater than that
charged to other persons in the same community.
6. TERMINATION: EITHER PARTY MAY TERMINATE THIS CONTRACT BY 30 DAYS PRIOR NOTI-
FICATION IN WRITING.
