The project director will receive the proper forms and information for administering this grant at the preaward conference. Please understand that any future grants that you might request from the Division of Criminal Justice for LEAA funds must be received by this office no less than 75 days before the State Council on Criminal Justice will act on your application. Forensic/Photographic Lab Upgrade GRANT PERIOD: From 7/14/78 To 9/30/78 In accordance with the provisions of Title I of the Omnibus Crime Control and Safe Streets Act of 1968 (P.L. 90-35]) , as amended, the Division of Criminal Justice hereby awards a grant to the above-named grantee. Federal Grant Award $ 26,481 Initial Federal Drawdown $ -0- Subgrantee Contribution $ 1,471 Initial Legislative Drawdown $ -0- State Match/Buy-In $ 1,471 Total Drawdown $ -0- Total Project Budget $ 29,42.1 The approved budget by category is based upon your Action Grant Application, DCJ Form 5, which was approved by the Council on Criminal Justice. It is necessary that you base your activities on this approved application and not on the application originally submitted since changes may have occurred as a result of processing requirements. Your finance officer must have a copy of the approved Action Grant Application in order to prepare the necessary quarterly reports for this project. The grantee must secure prior written permission from the Division of Criminal Justice if there is to be an increase in any budget category total that exceeds ten percent, if a new line item is to be added, or if an existing line item is to be deleted. The grantee is to keep copies of all documents and correspondence that relate to the project in a file bearing the project title and number. All expenditures must be supported by documentation. Only actual expenditures will be permitted. This grant is subject to the general conditions and the conditions applicable to fiscal administration set forth in the Action Grant Application, DCJ Form 5 Form 9A. This Statement of Grant Award duly executed and returned to the Division of Criminal Justice in conjunction with the approved grant application will serve as the contract between the Division of Criminal Justice and the grantee. Quinn, Director DIVISION OF CRIMINAL JUSTICE GRANT AWARD ACCEPTANCE THIS AWARD IS SUBJECT TO SPECIAL CONDITIONS AS FOLLOWS PURSUANT TO THE AUDITING AND FISCAL RESPONSIBILITIES OF THE DIVISION OF CRIMINAL JUSTICE: This grant is subject to an audit. The grantee must maintain all project records as will facilitate an effective audit for three years or beyond the three-year period if an audit is in progress and/or the findings of a completed audit have not been resolved satisfactorily. If it appears that the project will not be completed within the time specified in the Statement of Grant Award, the grantee must submit a written request for an extension of the grant period to the Division of Criminal Justice. This request must be submitted prior to the expiration date of the project. (Unless the request is approved by the Division of Criminal Justice prior to the authorized completion date of the project, reimbursement for expenditures beyond the specified completion date will not be allowed. ) Requests for proposal or invitations for bid issued by the grantee or subgrantee to implement the grant or subgrant project are to provide notice to prospective bidders that the LEAA organizational conflict of interest provision is applicable in that contractors that develop or draft specifications, requirements, statements of work and/or RFP's for a proposed procurement shall be excluded from bidding or submitting a proposal to compete for the award of such procurement. See: LEAA Guideline Manual M7-1000. lA Chapter 3, Paragraph 49e. Special Procedures for Equipment Grants: 1. This Statement of Grant Award is your authority to initiate an order for the equipment specified in the approved project budget. 2. A cash warrant for the federal share will be sent to you, upon receipt by the Division of a copy of the invoice(s) for the equipment to be purchased. 3. In order to eliminate delays , phone calls, and time consuming cor- respondence, please reconcile the invoice(s) to the project budget on a separate sheet of paper. This is necessary because many vendors identify equipment items by their numbers on the invoices. You are in the best position to know, i .e. , that--1 each-$295.86-- is the typewriter in the project. budget. 4. A cash drawdown request for the federal amount in the approved grant budget or the appropriate federal share computed by the match ratio of the approved application, whichever is less , should be submitted in the form of a separate letter from the project director. SPECIAL CONDITIONS: 1. That the Weld County Sheriff's Department develop and implement an EEO Program that satisfies LEAA-EEO guidelines by May 15, 1979. 2. All funds must be obligated by September 30, 1978 and expended by December 31, 1978. However, the Council has attached the following Special Conditions: That the Weld County Sheriff's Department develop and implement an EEO Program that satisfies LEAH-EEO guidelines by May 15, 1979. Pursuant to State Council on Criminal Justice policy, the operational project director and the operational finance officer must attend the pre-grant award conference. This conference will be held in Room 110 of the State Centennial Building (1313 Sherman Street, Denver) on August 10, 1978 at 10:00 A.M. for discussion of: subgrant administration, financial reporting, accounting procedures, program implementation and program evaluation. Attendance at this conference is mandatory before funds are awarded and distributed to this project.
