RE: APPROVE PURCHASE OF SERVICES AGREEMENT AND AUTHORIZE CHAIR TO SIGN
- LOCHBUIE AFTER GLOWS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on
Aging, and the Lochbuie After Glows, commencing January 1, 2009, and ending June 30, 2009,
with further terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Purchase of Services Agreement between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, Area Agency on Aging, and the Lochbuie After Glows be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 11th day of February, A.D., 2009, nunc pro tunc January 1, 2009.

COLORADO RE: Purchase of Services Agreement between the Weld County
Department of Human Services and the Lochbuie Senior
Citizens Program
Enclosed for Board approval is a Purchase of Services Agreement between the Department and
the Lochbuie Senior Citizens Program. This Agreement was presented at the Board's February
10, 2009, Work Session.
This Purchase of Services Agreement is the last agreement of the series of agreements with
Senior Citizen centers that applied for mini -grants funded for capital improvement projects under
the Older American's Act at a level of $5,000 per each project.
The major provisions of this Agreement are as follows:
1. Procure the necessary labor and materials to complete exterior projects including but not
limited to the completion of concrete walkways, stairwells, repairs and resurfacing of the
driveway and add a structure on the rear of the building to cover the lower level entrance.
2. The grant award will be in the sum of $5,000.00.
3. The terms of this Agreement are valid through June 30, 2009.

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this 1st day of January, 2009 by and between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Services' Area
Agency on Aging, hereinafter referred to as "AAA" and the Lochbuie Senior Citizen Program hereinafter referred to as the "Lochbuie
After Glows."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services, and
WHEREAS, "AAA" considers small rural senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Lochbuie After Glows" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies (under the Older Americans Act) in the sum of $5,000 to "Lochbuie After
Glows."
2. "Lochbuie After Glows" agree to expend the grant monies to procure the necessary labor and materials to complete exterior
projects including but not limited to the completion of concrete walkways, stairwells, repairs and resurfacing of the driveway
and add a structure on the rear of the building to cover the lower level entrance.
3. "Lochbuie After Glows" will be responsible for all applicable state and local permits necessary for performing the specified
work and "Lochbuie After Glows" will be responsible for the satisfactory completion of all sponsored work.
4. "Lochbuie After Glows" agree to maintain documentation of the above expenditures and to seek prior approval from the
"AAA" if it is needed to expend a portion of this grant on expenditures not listed in number two (2) above.
5. "Lochbuie After Glows" understand that that if any workers paid or voluntary are injured in the course of performing the
specified work, "AAA" shall be exempt from liability for injuries to the fullest extent allowed by law.
6. "Lochuie After Glows" understand that all work must be completed and receipts turned in to the "AAA" no later than June
30, 2009.
7. "Lochbuie After Glows" understand that the terms of this agreement are valid through June 30, 2009.
IN WITNESS WHEREOF, the parties hereunto have caused this Agreement to be executed as of the day and year first hereinabove set
forth.
