It is that time of year to remind you that the Support and Maintenance Agreement for your
document imaging / management products is due to expire on November 10, 2009. In order
for these products to sustain a high level of quality performance and utilization for your
organization, we strongly recommend that you renew this agreement for an additional term to
avoid a lapse in support coverage.
Please sign the enclosed agreement and provide your purchase order number and return to
our Service Administration Department at the address below. Once we recieve the signed
agreement, you will be invoiced for the amount shown on the contract, plus any applicable
tax.
We hope that the performance of these products and HOV Services/LASON's support has
met your expectations, and as always, we appreciate your business.

2. HOV Services shall diagnose aid repair problems rdatDe to the covered components. T he mstorrer shy! promptly inform HOV SaHces of any pmleis arising from the used these cenporents. The
disposition of diagnosed and/a reported problems conceming their sorely and scheduling fa repar, shal be the derision of the Customer and HOV Services. HOV Services shall perform routine
preventative neidenance t the covered components m a periodic beds spaced equay throughout the trm of his agreement as idenitied in the covered services sedum on page one of tats agreement
swerving to the mmufadurefs spedieatbns, and the Customer's usage regdrements. The ageement may also be liritd to tie number don -site service calls performed during the term d the

3. Special Conditions fa Service Agreements Covering Equpnent and Hardvere
3.1 This ageement does nd cover any supdy [terns such as: Starter Toner (developer), Toner, Glass Flats, Glass Ganders, Papa, Fuse- OR, Bdbs, Ammonia, or any oher supply tem. HOV
Services will not be responsible for bs al log suds ferns a damages i named by not installing such items as required.
3.2 Standard Hadvare Sipped aid Mantenance Agreement HOV Services wit provide nairtenatce according to these Terms and Condtans providing al labor, parts, and nun-crosumdole tens
necessary to repair the equipment. Consumable items such as: PC Drums, Developer Units, Fuser Rollers, Fuser Units, Imaging Units, Separator part„ pick /feed roles, or any other part
identfied by the mmufatuer as consumable tens shall be rail wed HOV Service' at the manufataa's remmrnerded intrvas or as needed and invoiced to the customer a current HOV
Se -vices pricing, Supplies are nd cwaed under this type of agreement

autgnzation. HOV Services adims necessitated by and trough the shove wit be undertaken by NOV Services only on the Customers approval of estimated additional charges and the
Customers Ageement to pay the anal charges incur red
4.5 Premium System Support and Maintenance Agreement HOV Services will make available b the Customer, al application software reve ens and assodatd documentation as they became
available from the manufacturer, paned the Customer is using the most recent or currenl release of the softvare prior to the efedive dated flit agreement as wet as, labor to install those

8. HOV Services may cancel this agreement within 30 days of wtten notice fa the blowing reasons 1) t to equipment a covacd components are moved fromthe location specified on page 1 of this
PMA. 2) If the equ pmant or covered canpments are son, leased or transferred to another pay. 3) 1 the equpment a covered mrnpornents am operated by any pat), net auhorized by the customer. The
agreement is based upon the egppment and cweral components toeing operabd nomtay by the customer. Any misuse or excessive use d the covered corrponetts that is not recommended by HOV
Services a the manufacturer, may Aso slow HOV Seams to cancel this PMA through when notice

10 At he end of the contract penal, HOV Services may change pies, tens and conditions fa the new contract period due b unforeseen maker dianges, avanbilty of parts, et. beyond HOV Service's
control. Customer shall receive a minimum of tidy, (30) days written ndice to the renews of this agreement. Should any lapse of maintenance coverage occur, NOV Services reserves the right to invoice
the Customer for any time the components were not covered under this ageement
