RE: APPROVE EIGHT PURCHASE OF SERVICES AGREEMENTS FOR OUTREACH
ACTIVITIES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with eight Purchase of Services Agreements
for Outreach Activities between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Agency, and the various Senior Centers, as follows, commencing December 1, 2008,
and ending June 30, 2009, with further terms and conditions being as stated in said agreements,
and
Evans Senior Center
Fort Lupton Community Center
Greeley Senior Activity Center
Johnstown Senior Center
Mead Senior Center
Pierce Senior Center
Platteville Community Center
Tri-Town Senior Center
WHEREAS, after review, the Board deems it advisable to approve said agreements, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the eight Purchase of Services Agreements for Outreach Activities between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, Area Agency on Agency, and the
various Senior Centers as listed above be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreements.

Purchase of Services Agreements between the Weld County
Department of Human Services and Eight (8) Service
Providers
Enclosed for Board approval are Purchase of Services Agreements between the Weld County
Department of Human Services and Eight (8) Service Providers. These Agreements were presented at
the Board's December 15, 2008, Work Session.
The Service Providers are:
1. Evans Senior Center
2. Fort Lupton Community Center
3. Greeley Senior Activity Center
4. Johnstown Senior Center
5. Mead Senior Center
6. Pierce Senior Center
7. Platteville Community Center
8. Tri-Town Senior Center
The major provisions of these Agreements are as follows:
1. These Agreements are the Loneliness Agreements in the amount of $500.00 each.
2. These grants help the centers to run small outreach programs that address issues around loneliness and
isolation.
3. The term of these Agreements is a one time only award and the terms are valid through June 30,
2009.

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this 1st day of December, 2008 by and between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of
Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Evans Senior Center hereinafter
referred to as "Evans."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services, and
WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Evans" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Evans" proposes to maintain the "Evans Senor Outreach" program to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to "Evans" in the sum of $500.00
2. "Evans" agrees to maintain an outreach volunteer committee that will coordinate all senior outreach efforts. The
volunteers will make contact via phone and/or face to face to encourage participation at the senior center. Seniors
will receive information concerning ongoing and special events through a variety of mailing and advertising.
3. "Evans" agrees to document the results of this outreach effort on a monthly bases and complete all required
paperwork by the 10th of each month. "Evans" understands that the grant of $500.00 may be used to pay for the cost
of the outreach program only.
4. "Evans" understands this is a one time only award and the terms of this agreement are valid through June 30, 2009.

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this 1st day of December, 2008 by and between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of
Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Fort Lupton Community Center
hereinafter referred to as "Fort Lupton."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services, and
WHEREAS, "AAA" considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Fort Lupton" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Fort Lupton" proposes to implement the "Loving Boundaries Outreach" program in order to connect with
area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to "Fort Lupton" in the sum of $500.00.
2. "Fort Lupton" agrees to implement the "Loving Boundaries Outreach" program. The program will focus on
connecting with community seniors through special events, programs and scholarships for low income and isolated
seniors. Transportation will be provided to seniors who can not access events on their own.
3. "Ft Lupton" agrees to document the results of the outreach efforts monthly and complete all required paperwork by
the 10th of each month. "Ft. Lupton" understands that the grant of $500.00 may be used only to pay for the cost of
the outreach program.
4. "Fort Lupton" understands this is a one time only award and the terms of the agreement are valid through June 30,
2009.

WHEREAS, "Greeley" proposes to implement a "Staying Active and Social in 2008/09"program to reach out to area
seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to "Greeley" in the sum of $500.00.
2. "Greeley" agrees to send flyers to low income isolated seniors inviting them to apply for an Exercise Scholarship that
would cover the cost or a portion of the cost for the Senior Shape -up and other exercises classes. This class promotes
physical fitness and positive social interaction. Transportation will be provided by the volunteers for seniors who
can not provide or obtain access to transportation services.
3. "Greeley" agrees to document the results of this outreach effort monthly and complete all required paperwork by the
10th of each month. "Greeley" understands that the grant of $500.00 may be used only to pay for the cost of the
outreach program.
4 "Greeley" understands this is a one time only award and the terms of the agreement are valid through June 30, 2009.

Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human
Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Johnstown Senior Center hereinafter referred to as
"Johnstown."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of
services, and
WHEREAS, "AAA" considers senior centers a vital link to seniors who are lonely and isolated, and
WHEREAS, "Johnstown" is a program providing recreational, nutritional, and educational opportunities for senior
citizens, and
WHEREAS, "Johnstown" proposes to implement and maintain the "Senior Outreach Project." to reach out to
seniors in the Johnstown area to involve them in the social activities at the Johnstown Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to "Johnstown" in the sum of $500.00.
2. "Johnstown" agrees to implement and maintain the "Senior Outreach Project" that would involve the
maintenance of a volunteer committee that will continue to reach out to isolated or homebound seniors via
monthly mailing of flyers/calendars/newsletters and phone and one on one outreach. Special programs such as
ice cream socials and other social outreach events will be implemented and/or maintained. Local media
options will be used to advertise events and provide service information.
3. "Johnstown" agrees to document the results of this outreach effort monthly and complete all required
paperwork by the 10'" of the month. "Johnstown" understands that the grant of $500.00 maybe used to pay
for the cost of the outreach program and to advance the overall "Johnstown" senior program.
4. "Johnstown" understands this is a one time only award and the terms of this agreement are valid through June
30, 2009.

Center hereinafter referred to as "Mead."
WITNESSETH:
WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of
services, and
WHEREAS, "AAA" considers senior centers a vital link to seniors who are lonely and isolated, and
WHEREAS, "Mead" is a program providing recreational, nutritional, and educational opportunities for senior
citizens, and
WHEREAS, "Mead" proposes to implement and maintain the "You are not Forgotten" program to reach out to
seniors in the Mead and Can areas to involve them in the social activities at the Mead Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to the "Mead" in the sum of $500.00.
2. "Mead" agrees to maintain the "You are not Forgotten" program by recruiting volunteers who will contact
homebound seniors and new seniors to the area, to increase phone calls and mailings, develop special events
and to help seniors with transportation, when possible.
3. "Mead" agrees to document the results of this outreach effort monthly 'and complete all required paperwork
by the le of each month. "Mead" understands that the grant of $500.00 may be used only to pay for the cost
of the outreach program
4. "Mead" understands this is a one time only incentive award and the terms of this agreement are valid through
June 30, 2009.

WHEREAS, "Pierce" is a program providing recreational, nutritional, and educational opportunities for senior citizens,
and
WHEREAS, "Pierce" proposes to implement the "Old Time Jam" Project to reach out to isolated seniors in their
community to involve them in the social activities at the Pierce Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to "Pierce" in the sum of $500.00.
2. "Pierce" agrees to develop and maintain the "Old Time Jam" project. This project will use a both community
volunteers and senior center volunteers to encourage broader senior participation at the senior center. Outreach
efforts will include advertizing events, phone contacts, flowers, cards and special projects.
3. "Pierce" agrees to document the results of this outreach effort monthly and complete all required paperwork by the
10th of each month. "Pierce" understands that the grant of $500.00 may be used only to pay for the cost of the
outreach program.
4. "Pierce" understands this is a one time only award and the terms of this agreement are valid through June 30, 2009.

WHEREAS, "Platteville " proposes to maintain the "Platteville Loneliness Outreach " program to reach out to area isolated
seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to "Platteville" in the sum of $500.00.
2. "Platteville" agrees to maintain the "Platteville Loneliness Outreach" program that will include volunteer
outreach efforts that will include identifying seniors in the community who are at risk of loneliness and
isolation and encourage participation in senior activities.
3. "Platteville" agrees to document the results of this outreach effort monthly and complete all required paperwork by the
10th of each month. "Platteville" understands that the grant of $500.00 may be used only to pay for the cost of the
outreach program.
4. "Platteville" understands this is a one time only award and the terms of the agreement are valid through
June 30, 2009.

WHEREAS, "Tri-Town" proposes to implement a "The Connection" project to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" hereby agrees to award grant monies to "Tri-Town" in the sum of $500.00.
2. "Tri-Town" agrees to implement and maintain the "The Connection" project. The project will use a volunteer
committee that will reach out to homebound and isolated seniors and encourage senior center involvement and/or
provide home visits to homebound seniors and volunteer transportation services. Volunteers will also send cards,
letter, brochures and flyers to community seniors.
3. "Tri-Town" agrees to document the results efforts monthly and complete all required paperwork by the 10th of the
month. "Tri-Town" understands that the $500.00 may be used to pay for the cost of the outreach program and to
advance the Tri-Town Senior Program.
4. "Tri-Town" understands this is a one time only award and the terms of this agreement are valid through June 30,
2009.
