Subject: Conquest Disposal Service Conquest -4 — 3rd Quarter Inspection 2009
Dear Mr. Goddard:
On August 11, 2009, the Weld County Dept. of Public Health & Environment conducted a routine
inspection of the disposal well operated by Conquest Disposal Service, located at 22625 Weld County
Road 64, Weld County, Colorado. The purpose of the inspection was to assess the facility's compliance
with the Approved Site Specific Development Plan and Use by Special Review Permit Number•729
t(USR=729)xand applicable Weld County Code.
Based on recent Air Pollutant Emission Notice (APEN) submittals from like facilities, this department has
reason to believe your facility may need to report air emissions of Volatile Organic Compounds.
Development Standards 11 & 14 of USR-729 requires the facility to comply with any required air
emissions permits and to comply with the laws, standards, rules and regulations of the Air Quality
Control Commission. Section 23-2-250 of the Weld County Code requires the operation of this use to
comply with the air quality regulations promulgated by the Air Quality Control Commission.
Please file an APEN with the APCD and copy the submittal to this Department. The Department will
follow-up with you regarding this during the next quarterly inspection. A copy of the inspection checklist

inspection of the disposal well operated by Conquest Disposal Service, located at 22625 Weld County
Road 64, Weld County, Colorado. The purpose of the inspection was to assess the facility's compliance
with the Approved Site Specific Development Plan and Use by Special Review Permit Number 729

On January 8, 2009, the Weld County Dept. of Public Health & Environment conducted a
routine inspection of the disposal well operated by Conquest Disposal Service, located at 22625
Weld County Road 64, Weld County, Colorado. The purpose of the inspection was to assess the
facility's compliance ith-the Approved Site Specific Development Plan and Use by Special
Review Permit Numb r 729 d applicable Weld County Code. Based on the inspection, the
facility was in compliance pending the removal of portable storage tanks from the facility.
A copy of the inspection checklist and an e-mail from Conquest regarding the portable storage

Good Morning! As per your request I'm letting you know we have set 22 temporary frac tanks
at different locations in order to help with the Cl well being down during drilling our
new well. In the future I will e-mail you and let you know when we bring these tanks on
location.
