RE: APPROVE AMENDMENTS TO TWO CHILD PROTECTION AGREEMENTS FOR
SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with two Amendments to Child Protection
Agreements for Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human Services,
and the following providers, commencing upon full execution, with further terms and conditions
being as stated in said amendments, and
1. Griffith Centers for Children - Colorado Springs
2. Griffith Centers for Children - Larkspur
WHEREAS, after review, the Board deems it advisable to approve said amendments,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the two Amendments to Child Protection Agreements for Services between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, and the above listed providers be,
and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.

Amendments Child Protection Agreements for Services
between the Weld County Department of Human Services
and Various Providers
Enclosed for Board Approval are Amendments to Child Protection Agreements for
Services between the Department and Various Providers. The amendments are to adjust
the Educational Services rate in each contract to align with the rate set forth by CDE. The
Department usually does not pay out on educational services associated with Day
Treatment as that is picked up by the school districts; however, there is one case in which
the case worker has indicated the school district is not paying this fee. The major
provisions for these Agreements are:

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
GRIFFITH CENTERS FOR CHILDREN - COLORADO SPRINGS CAMPUS
This Amendment, made and entered into the/D-- day of March 2009, by and between the Board of
County Commissioners, on behalf of the Weld County Department of Human services, hereinafter referred
to as "Department," and Griffith Centers for Children — Colorado Springs Campus, hereinafter referred to
as the "Contractor."
WITNESSETH
WHEREAS, the parties desire to modify their current Agreement (PY-08-09-CORE-0168) in
order to adjust the educational rate per the Colorado Department of Education; and
WHEREAS, Human Services has budgeted, appropriated, and otherwise made available; and
WHEREAS, required approval, clearance, and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE. in consideration of the premises, the parties hereto covenant and agree as
follow:
1. Exhibit B, Payment Schedule, Item 2, is hereby revised to reflect the following:
$60.16/Day (Educational Services)
This Amendment, together with the original Child Protection Agreement for Services, constitutes
the entire understanding among the parties with respect to the subject matter hereof, and may not be
changed or modified except as stated in the Child Protection Agreement for Services.
