Attached please two (2) copies of Amendment effective May 1, 2009 to December 31, 2009.
The amendment changes the weekly on -site Psychiatrist or Psychiatric Nurse Practitioner hours
from 5 hours per week to 2.5 hours per week. This decreased the monthly contract amount
which results in the Amendment.
Please have both Amendments fully signed and return one (1) copy back to me as soon as

AMENDMENT TO THE AGREEMENT FOR INMATE HEALTH SERVICES AT
WELD COUNTY, COLORADO
(Effective date May 1, 2009)
This is an Amendment to the "Health Services Agreement," effective January 1, 2006, (hereinafter
"AGREEMENT") between Correctional Healthcare Management, Inc. (hereinafter "CHM") and Weld County,
Colorado (hereinafter the "COUNTY"). Both parties agree that effective May 1, 2009 the following changes
shall be made to the AGREEMENT:
1. CHM shall provide 2.5 hours of Psychiatrist or Psychiatric Nurse Practitioner ("CHM Mental Health
Staff') services at the Weld County North Jail Complex. The Parties agree that COUNTY shall be
responsible for all intake and post admission mental health evaluations. CHM Mental Health Staff shall
provide treatment only upon referral from COUNTY. The Parties further agree that COUNTY shall
work cooperatively with CHM's Mental Health Staff to effectively carry out the terms and conditions of
this AMENDMENT.
2. From May 1, 2009 through December 31, 2009, the monthly amount to be paid by COUNTY to CHM is
$216,815.17 (Two -Hundred Sixteen Thousand, Eight Hundred Fifteen Dollars and 17/100).
Except for the provisions, amended by this document, all other provisions of the AGREEMENT shall remain in
full force and effect and unchanged.
IN WITNESS WHEREOF, the parties have caused this Amendment to be executed in their names or their
official acts by their respective representatives, each of whom is duly authorized to execute the same.
