DEPARTMENT: WELD COUNTY PUBLIC WORKS DATE: February 26. 2009 PERSON REQUESTING: Josh Holbrook, Weld County Public Works. Brief description of the problem/issue: Change Order No. 1 - The original agreement for Professional Services under the On -Call Geotechnical contract could extended from June 18th, 2008 to June 17th, 2009 as permitted by the contract. This extension would be in the second year of the four year term limit. The 2007 rate Schedule of the original contract would be replaced with the attached Exhibit A, 2009 Rate Schedule. The increase does not exceed the Denver Boulder CPI per the contract requirements. Work Order No. 7 — This work order is set up to cover the testing for the 2009 construction projects that our lab dose not have the capabilities to perform. We have selected CTL Thompson to perform theses services under the On -Call Geotechnical Contract. I Weld County On -Call Geotechnical Testing Service Date: Project: Weld County On -Call Geotechnical testing generally consists on ensuring materials used during construction meet contract specifications for embankment, gravel, concrete and asphalt. Owner: Weld County Public Works Department, PO Box 758, Greeley, Colorado 80632 Consultant: CTL Thompson, Inc. 351 Linden Street Unit 140, Fort Collins, Colorado 80524 The following change is hereby made to the Contract Documents: Contract Time Extension: The original Agreement for Professional Services is extended from June 18th, 2008 to June 17th, 2009 as permitted by the contract.
