RE: APPROVE AMENDMENT TO PURCHASE OF SERVICES AGREEMENT FOR BUILDING
HEALTHY MARRIAGES PROGRAM AND AUTHORIZE CHAIR TO SIGN - COMMUNITY
MEDIATION PROJECT, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Purchase of Services
Agreement for the Building Healthy Marriages Program between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, and the Community Mediation Project, Inc., commencing
October 1, 2008, and ending September 30, 2009, with further terms and conditions being as
stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to the Purchase of Services Agreement for the Building
Healthy Marriages Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human Services,
and the Community Mediation Project, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 28th day of October, A.D., 2009, nunc pro tunc October 1, 2008.

Amendment to the Purchase of Services Agreement for
Services between the Weld County Department of Human
Services' Building Healthy Marriages and Community
Mediation Project, Inc.
Enclosed for Board approval is an Amendment to the Purchase of Services Agreement for
Services between the Department and Community Mediation Project, Inc. This Agreement was
presented at the Board's October 26, 2009, Work Session.
Building Healthy Marriages exceeded its target numbers by 151°/> in year three (October 1, 2008
through September 30, 2009). This has required an increase in finds required for the educational
portion of the Building Healthy Marriages Program of $14,338. The funds will be paid for by the
Federal Healthy Marriage Demonstration Grant dollars.

AMENDMENT
PURCHASE OF SERVICES AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT
OF HUMAN SERVICE AND COMMUNITY MEDIATION PROJECT
This AMENDMENT to Purchase of Services Agreement for Services made and entered into thisr day
of October, 2009, by and between the Board of County Commissioners, on behalf of the Weld County
Department of Human Services' recipients of the Healthy Marriages Demonstration Grant, hereinafter
referred to as "Human Services," and Craig Conrad on behalf of Community Mediation Project, Inc,
hereinafter referred to as "CMP."
WITNESSETH
ESSETH
WHEREAS, Human Services and CMP entered into a Purchase of Services Agreement for Services
on October 2008, for the period of October 1, 2008 through September 30, 2009, to provide services to
individuals participating in the Building Healthy Marriages Program (BHM).
WHEREAS, the monies provided under the agreement is not sufficient for the term of the Agreement to
serve the educational of Building Healthy Marriage participants.
WHEREAS, Human Services desires to adjust its payment schedule to respond to the increased number
of individuals participating in BHM.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follows:
1. Exhibit A, Payment Schedule, Funding and Method of Payment, shall be amended as follows:
Human Services agrees to reimburse CMP in consideration for the work and services performed, a total
amount not to exceed Fourteen thousand, three hundred, thirty eight dollars ($14,338). This will
continue to be funded by the Healthy Marriages Demonstration Federal Grant dollars.
2. This Amendment, together with the original Purchase of Services Agreement for Services
constitutes the entire understanding among the parties with respect to the subject matter hereof, and
may not be changed or modified except as stated in the Purchase of Services Agreement.
