RE: APPROVE NINE PURCHASE OF SERVICES AGREEMENTS WITH VARIOUS
PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with nine Purchase of Services Agreements
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Aging, and the following providers, commencing upon full execution, and ending June 30,
2010, with further terms and conditions being as stated in said agreements, and
1. Erie Community Center
2. Evans Senior Center
3. Fort Lupton Recreation Center
4. Greeley Senior Activity Center
5. Johnstown Senior Center
6. Mead Senior Center
7. Milliken Senior Center
8. Tri-Town Senior Center
9. Platteville Senior Center
WHEREAS, after review, the Board deems it advisable to approve said agreements,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the nine Purchase of Services Agreements between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Human Services, Area Agency on Aging, and the above listed
providers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreements.

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, made and entered into this 23rd day of September, 2009 by and between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Human Services' Area Agency on Aging, hereinafter referred to as "WCAAA" and
the Erie Community Center hereinafter referred to as "Erie".
WITNESSETH:
WHEREAS, WCAAA develops and administers the Area Plan for a comprehensive and coordinated system of
services, and
WHEREAS, WCAAA considers senior centers a vital link in the county's system of services to older adults,
and
WHEREAS, Erie is a senior center providing recreational, nutritional, and educational opportunities for older
adults, and
WHEREAS, Erie proposes to implement a "Step Up To Health" program to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The WCAAA hereby agrees to award grant monies to Erie in the sum of $500.00.
2. Erie agrees to sponsor a walking program to provide motivation, support and encouragement to seniors
by 1) selling daily step goals using pedometers and an optional FitLinxx tracking system and 2) free
admission to monthly health seminars; by marketing through Active Net, flyers, weekly bulletins,
monthly calendars, e -boards and word of mouth.
3. Erie agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 10th of each month. Erie understands that the grant of $500.00 may be used to pay
for the cost of the project only.
4. Erie understands this is a one time only award and the terms of this agreement are valid through June
30, 2010.

the Evans Senior Center hereinafter referred to as "Evans".
WITNESSETH:
WHEREAS, WCAAA develops and administers the Area Plan for a comprehensive and coordinated system of
services, and
WHEREAS, WCAAA considers senior centers a vital link in the county's system of services to older adults,
and
WHEREAS, Evans is a senior center providing recreational, nutritional, and educational opportunities for older
adults, and
WHEREAS, Evans proposes to implement a "Bones, Bladder, and Balance" program to reach out to area
seniors;
NOW THEREFORE, it is mutually agreed:
1. The WCAAA hereby agrees to award grant monies to Evans in the sum of $500.00.
2. Evans agrees to conduct classes held by a physical therapist that emphasize exercise and address the
concerns of osteoporosis, bladder control and balance experienced by older women.
3. Evans agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 101h of each month. Evans understands that the grant of $500.00 may be used to pay
for the cost of the project only.
4. Evans understands this is a one time only award and the terms of this agreement are valid through
June 30, 2010.

2. Fort Lupton agrees to sponsor A Matter of Balance class and lead a Walking to Wellness project on the
recreation trails by recruiting with advertisements and brochures and awarding participants who
complete the classes.
3. Fort Lupton agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 10th of each month. Fort Lupton understands that the grant of $500.00 may be used
to pay for the cost of the project only.
4. Fort Lupton understands this is a one time only award and the terms of this agreement are valid
through June 30, 2010.

WHEREAS, Greeley is a senior center providing recreational, nutritional, and educational opportunities for

WHEREAS, Greeley proposes to implement a "It's In Your Hands" program to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The WCAAA hereby agrees to award grant monies to Greeley in the sum of $500.00.
2. Greeley agrees to provide portable hand sanitation stations; offer classes on proper hand washing and
the importance of preventing the spread of germs; and provide educational information on various flu
viruses by posting information in the monthly newsletter, daily announcements during lunch and
distributing educational materials.
3. Greeley agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 10th of each month. Greeley understands that the grant of $500.00 may be used to
pay for the cost of the project only.
4. Greeley understands this is a one time only award and the terms of this agreement are valid through
June 30, 2010.

the Johnstown Senior Center hereinafter referred to as "Johnstown".
WITNESSETH:
WHEREAS, WCAAA develops and administers the Area Plan for a comprehensive and coordinated system of
services, and
WHEREAS, WCAAA considers senior centers a vital link in the county's system of services to older adults,
and
WHEREAS, Johnstown is a senior center providing recreational, nutritional, and educational opportunities for
older adults, and
WHEREAS, Johnstown proposes to implement a "Diabetic Care Program" to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The WCAAA hereby agrees to award grant monies to Johnstown in the sum of $500.00.
2. Johnstown agrees to sponsor educational classes and exercises as recommended by the Center for
Disease Control by a certified instructor. Classes and exercises will cover food preparation, nutritional
guidelines, testing procedures, health screenings, time management, reminder systems, etc.
3. Johnstown agrees to provide transportation to and from exercise activities: stadiums, malls, parks, etc.
4. Johnstown agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 10th of each month. Johnstown understands that the grant of $500.00 may be used
to pay for the cost of the project only.
5. Johnstown understands this is a one time only award and the terms of this agreement are valid through
June 30, 2010.

1. The WCAAA hereby agrees to award grant monies to Mead in the sum of $500.00.
2. Mead agrees to purchase a Wii system, interactive games, and warranty to promote physical activity to
seniors by scheduling game time.
3. Mead agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 10th of each month. Mead understands that the grant of $500.00 may be used to pay
for the cost of the project only.
4. Mead understands this is a one time only award and the terms of this agreement are valid through June
30, 2010.

WHEREAS, Milliken proposes to implement a "Senior Yoga and Body Sculpting" program to reach out to area
seniors;
NOW THEREFORE, it is mutually agreed:
1 The WCAAA hereby agrees to award grant monies to Milliken in the sum of $500.00.
2. Milliken agrees to sponsor the cost of Senior Yoga and Body Sculpting classes held at Thompson
Rivers Parks and Recreation. Milliken agrees to provide transportation by volunteers for seniors who
cannot provide or obtain access to transportation services.
3. Milliken agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 10th of each month. Milliken understands that the grant of $500.00 may be used to
pay for the cost of the project only.
4. Milliken understands this is a one time only award and the terms of this agreement are valid through
June 30, 2010.

WHEREAS, Tri Town proposes to implement a "Dance a Smile On" program to reach out to area seniors;
NOW THEREFORE, it is mutually agreed:
1. The WCAAA hereby agrees to award grant monies to Tri Town in the sum of $500.00.
2. Tri Town agrees to use flyers and news articles distributed throughout the community to promote and
recruit seniors to participate in various dance classes. Tri Town agrees to hire knowledgeable
instructors for each type of dance class offered and utilize volunteers to promote the classes as well as
class registration.
3. Tri Town agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 101h of each month. Tri Town understands that the grant of $500.00 may be used to
pay for the cost of the project only.
4. Tri Town understands this is a one time only award and the terms of this agreement are valid through
June 30, 2010.

NOW THEREFORE, it is mutually agreed:
1. The WCAAA hereby agrees to award grant monies to Platteville in the sum of $500.00.
2. Platteville agrees to sponsor the cost of scholarships for a senior yoga classes instructed by a yoga
instructor. The sponsorship includes a six week program with yoga and floor mats that encourages
older adults to live healthier and exercise more.
3. Platteville agrees to document the results of the project on a monthly basis and complete all required
paperwork by the 10th of each month. Platteville understands that the grant of $500.00 may be used to
pay for the cost of the project only.
4. Platteville understands this is a one time only award and the terms of this agreement are valid through
June 30, 2010.
