To Whom it May Concern:
Great Western Oil and Gas Company, LLC (GWOG), successor in interest to Apollo Operating, LLC, is
the Lessor of County Lease #2006-2816, recorded in the Weld County records at Reception No. 3430477.
In compliance with paragraph thirteen in the lease, enclosed you will find a copy of Declaration of Pooling
recorded at Reception No. 3614501, and an Affidavit of Extension of Oil and Gas Lease recorded at
Reception No. 3614502; both instruments being recorded with the Clerk and Recorder of Weld County,
Colorado. Said Declaration of Pooling gives notice as to the creation of a pooled unit consisting of the
S'/ SE''% of Section 31 -Township 7 North -Range 63 West of 6" P.M. GWOG anticipates further
development of the leasehold in the near future.

DECLARATION OF POOLING
KNOW ALL MEN BY THESE PRESENTS:
1. Canamex, LLC is the owner of the oil and gas leases ("Leases") set forth on Exhibit
A attached hereto covering the lands in Weld County, Colorado, more specifically
described on Exhibit A.
2. Each of the Leases contains provisions authorizing the Lessee to unitize or pool the
Leases and the lands covered thereby, in whole or in part, with other leases and
lands so as to create a unit or pool as maybe required to conform to applicable rules
and orders of the Colorado Oil and Gas Conservation Commission.
3. Acting under and by virtue of the pooling authority contained in the Leases, the
undersigned hereby pools, combines and designates the following lands (and the
leasehold, mineral and royalty rights thereunder) as a unit for the drilling and
production of oil, gas and associated liquid hydrocarbons from the Niobrara/Codell
formation(s):
Township 7 North, Range 63 West, 6`h P.M.
Section 31: S%SE%
IN WITNESS WHEREOF, this instrument is executed as of this 2n° day of April, 2009, but shall
be effective for all purposes as of the date of first production from the Heinze #34-31 well
located in the SW%SE% and the Heinze #44-31 well located in the SE%SE'A of said Section
31.

AFFIDAVIT OF EXTENSION OF OIL AND GAS LEASES
The undersigned, of lawful age, having been first duly sworn upon his oath, deposes and states:
1. That Canamex, LLC is the owner and holder of the oil and gas leases ("Leases") set
forth on Exhibit A attached hereto.
2. That Affiant is the Manager of Canamex, LLC.
3. That each of such Leases is for a definite term of years, days or months from the date
thereof and for so long thereafter as oil or gas is produced from the lands covered
thereby or land pooled or unitized therewith.
4. That under date of April 2, 2009 (but effective as of the date of first production
therefrom), the Leases were pooled to form a unit comprised of the S''%SE% of Section
31, Township 7 North, Range 63 West, 6th P.M. for the production of oil and gas from the
Niobrara and Codell formations.
5. That production of oil and gas has been obtained from the Heinze #34-31 and 44-31
Wells located in the SYhSE% of said Section 31.
6. That pursuant to Colorado Revised Statute §38-42-106 (1973, as amended), Keith Kress
as Manager of Canamex, LLC hereby claims and provided notice of the extension of
each of the Leases identified on said Exhibit A.
