RE: APPROVE MEMORANDUM OF UNDERSTANDING FOR HEAD START AND
MIGRANT/SEASONAL HEAD START PROGRAMS AND AUTHORIZE CHAIR TO SIGN -
ENVISION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Memorandum of Understanding for the
Head Start and Migrant/Seasonal Head Start Programs between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, Family Educational Network of Weld County (FENWC), and
Envision, commencing August 15, 2009, and ending August 14, 2010, with further terms and
conditions being as stated in said Memorandum of Understanding, and
WHEREAS, after review, the Board deems it advisable to approve said Memorandum of
Understanding, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Memorandum of Understanding for the Head Start and
Migrant/Seasonal Head Start Programs between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Family Educational Network of Weld County (FENWC), and Envision be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said Memorandum of Understanding.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 19th day of August, A.D., 2009, nunc pro tunc August 15, 2009.

Memorandum of Understanding -between the Weld County

Enclosed for Board Approval is the Memorandum of Understanding between the
Department's Head Start and Migrant and Seasonal Head Start programs and Envision,
CCB/EIC in Weld County. This was presented at the Board's August 17, 2009, Work
Session.
The purpose of this Memorandum of Understanding is to implement, foster, and facilitate
an effective and collaborative system of early intervention between Envision and the
Family Educational Network of Weld County (FENWC), to define and clarify the roles,
responsibilities and timelines of each agency and assure families have timely, compre-
hensive and easy access to the early intervention supports and services as they relate to
Envision and FENWC eligible children.

MEMORANDUM OF UNDERSTANDING BETWEEN
ENVISION, CCB/EIC IN WELD COUNTY AND
FENWC HEAD START AND MIGRANT AND SEASONAL HEAD START
This Agreement is between Envision, Creative Support for People with Developmental
Disabilities (hereinafter referred to as Envision), the Community Centered Board (CCB) and
Early Intervention Colorado (EIC) in Weld County, and the Weld County Department of Human
Services' Family Educational Network Head Start and Migrant and Seasonal Head Start
(hereinafter referred to as FENWC), for the period of August 15, 2009 through August 14, 2010.
Purpose
The purpose of this Agreement is to implement, foster and facilitate an effective and
collaborative system of early intervention between Envision and FENWC, to define and
clarify the roles, responsibilities and timelines of each agency and assure families have
timely, comprehensive and easy access to the early intervention supports and services as
they relate to Envision and FENWC eligible children. This system will be in compliance
with Part C and Part B 300.125 of the Public Law 105-17, Individuals with Disabilities
Education Act (IDEA), Colorado Rules for the Administration of the Exceptional
Children's Education Act, the Division for Developmental Disabilities Early Intervention,
and the Head Start Program Performance Standards on Services for Children with
Disabilities (45 C.F.R. 1308).
II. Partnership Requirement
Head Start programs are mandated to have formal partnership agreements with local
agencies in regard to identifying and serving pre -kindergarten children. Local level
agreements should define relationships and specify areas of collaboration which are
most appropriate to local situations.
III. The intent of this Agreement is to:
• Define potential services and activities to be provided by each Agency.
• Support cooperative communication and share leadership responsibilities at the local
level to ensure that available resources are utilized in the most effective manner.
• Facilitate the referral and service delivery process.
IV. Responsibilities of FENWC
1. Provide public information dissemination and access to programs for families contacting
Head Start or Migrant/Seasonal Head Start programs.
2. Recruit, enroll, and serve eligible Regional Head Start children ages 3 — 5 and Migrant and
Seasonal Head Start children ages 6 weeks through 5 years whose families wish to place
them in FENWC Head Start centers, including working parents.
3. Screen all FENWC enrolled children, within 45 days for Regional Head Start and within
two weeks for Migrant and Seasonal Head Start of enrollment, for potential health
problems and /or developmental delays. FENWC will refer children ages 6 weeks
through 35 months for whom screening indicates potential developmental delay or
disability to Envision's Early Intervention Colorado (EIC) office, with appropriate
permission from the families. FENWC will participate in Child Find activities under Part
C of IDEA, including but not limited to, participation in IFSP development meetings,
annual reviews, and transition planning.
4. Find resources in the community for Envision and FENWC eligible children and their
families as needed, including services to meet the needs of working parents.
5. Work collaboratively with Envision to develop and implement cooperative staff training
opportunities including educational activities, curricular objectives and/or instruction to
increase knowledge and ability to provide quality services to Envision and FENWC
eligible children. This may include such topics as academic content standards,
instructional methods, curricula, and social and emotional development, provided by
FENWC and/or the technical assistance center, and sharing the use of facilities and other
program elements.
6. When requested by the Service Coordinator, assist Envision with communication and
parent outreach for scheduling meetings and to support smooth transitions from Part C to
Part B IDEA support services.
V. Responsibilities of Envision as ETC
1. Provide referral information and referral documents to all primary referral sources.
2. Within 3 days of receipt of a referral for a child ages 0 to 36 months, refer on to the
appropriate Child Find Administrative Unit to schedule a multidisciplinary evaluation
with the family within 45 days of referral. Envision will assign a service coordinator
within 3 business days of receipt of the referral. The service coordinator will complete
required paperwork with the family, provide appropriate notices, and assist in gathering
information for use during the evaluation and to inform the IFSP.
3. With information gathered during the multidisciplinary evaluation and from medical
sources, when applicable, determine the child's eligibility for Early Intervention services.
Assure that parents are aware of their rights if a child is found to be not eligible based
upon the results of the evaluation.
4. Refer eligible children to FENWC and other community agencies for possible services, as
appropriate.
5. Attend and assist the parents to participate in the multidisciplinary evaluation with Child
Find. Refer families on for further evaluations if needed (i.e., medical evaluations).

6. Coordinate the development of the IFSP for children with disabilities and/or
significant developmental delays based on evaluation information. The IFSP
interdisciplinary team, which includes the parents/guardians of the child or an
educational surrogate, FENWC interdisciplinary staff, Envision Service Coordinator, and
Early Intervention Services Providers, will participate in the development of the IFSP.
When possible, the IFSP meetings will be held at a location and time convenient for all of
the interdisciplinary team members. Copies of the IFSP will be disseminated by the
Envision Service Coordinator to the parents in the preferred language of the family, and
copies in English will be distributed to FENWC and other IDT members with permission
from the parents. To the extent possible, services identified on the IFSP will be delivered
in an environment that is natural for the family and is part of their everyday routines,
activities and places.
7. Locate providers and coordinate provision of early intervention/Part C services to
identified FENWC children and families according to the IFSP in accordance with the
wishes of the family.
8. Maintain accurate data tracking of all children referred and data regarding all required
timelines as indicated in the Community Contract Management System (CCMS), and as
required by the Office of Special Education Programs (OSEP), U.S. Department of
Education. When available, Envision Service Coordination will obtain reports from
CCMS to assist with quality control and correction when needed.
9. Maintain documentation for in -kind purposes.
10. Work collaboratively with FENWC to develop and implement cooperative staff training
opportunities to increase knowledge and ability to provide quality services to program
eligible children.
VI. COUNTING AND REPORTING
Envision and FENWC will have systems and timelines for counting and reporting
children.
• Envision will utilize the CCMS system to track data required by OSEP, to include
numbers of children receiving EI services, number of children exiting EI programs and
reason for exit, and numbers of EI services received. Child count data is compiled twice
yearly.
• FENWC uses Child Plus data base and the Mental Health/Disabilities Specialist tracks
from concern to completion.
VII. REVIEW and TRANSITION PLANNING
In an effort to provide smooth transitions from Part C to Part B IDEA programs providing

support and services the following will occur:
1. Written notification of IFSP meetings will be provided to parents 10 days prior to the
scheduled meeting, unless waived by the parents. IFSPs will be reviewed at least every six
months or more often if needed or requested by the family, and a complete IFSP will be
developed annually. The Service Coordinator is responsible for assuring the IFSP
document is conducted in and translated into the family's native language, if necessary.
2. The Envision ETC Service Coordinator will begin the transition planning process with the
child's family at the IFSP meeting or review closest to the child's second birthday, or as
soon as possible if the child's eligibility for early intervention services is determined after
the child is two years of age, providing the family with transition information. The service
coordinator will review, with the family, changes in services that may occur when the
child turns three, including information on the Part B evaluation and eligibility criteria.
3. Upon written consent from the family, the service coordinator will notify the Local
Education Agency (LEA) of all children receiving early intervention services nine to six
months prior to the child's third birthday, if not earlier. Notification is accomplished
through written documentation to the LEA from Envision and the monthly Transition
Meeting held at Envision with EIC Service Coordinators and LEA Child Find
representatives. Transition plans will be developed no later than 90 days prior to the
child's 3`d birthday. When possible, eligibility for Part B services will be determined prior
to the Transition staffing. However, it is understood that the LEAs are not required to
determine Part B eligibility until much closer to the child's 3r° birthday, and may do so just
prior to an IEP. All IFSP team members, including FENWC, will be invited to attend the
Transition meeting..
4. Information to parents regarding the differences in service delivery will be provided jointly
during the Transition Staffing of the child from FENWC to the public school setting.
5. CAPTA, in conjunction with Part C of IDEA 2004, requires that children under the age of
three are referred for early intervention services if the child
• is involved in a substantiated case of child abuse or neglect, or
• is identified as affected by illegal substance abuse, or withdrawal symptoms
resulting from prenatal drug exposure and when screening of the child indicates
the presence of a developmental delay.
• Consent from parents is required if parental rights have not been terminated.
VIII. CONFIDENTIALITY
Consent for Release of Information Forms that will assist FENWC and Envision in
providing optimal services will be generated and signed as each child is identified in
accordance with the Family Education Privacy Rights Act and the Health Insurance
Portability and Accountability Act.

IX. MODIFICATION OF AGREEMENT
1. All modifications to this agreement shall be in writing and signed by both parties.
X. ASSIGNMENTS
1. This Agreement shall be binding upon the parties hereto, their successors, theirs, and legal
representatives.
IN WITNESS WHEREOF, the parties hereunto have caused this Agreement to be duly executed
as of the day and year first hereinabove set forth.
