RE: Firestone Eight (8th) Annexation and Inclusion into Carbon Valley Park and
Recreation District
Pursuant to the Town of Firestone ("the Town") annexation ordinance, a property that seeks to
annex into the Town shall also seek inclusion into all applicable special districts. In addition, Carbon
Valley Park and Recreation District ("Rec District") has an agreement with the Town wherein all annexed
properties are required to seek inclusion into the Rec District. Therefore, when applying for annexation
into the Town you must also submit a petition for inclusion into the Rec District.
To encourage immediate compliance with the annexation ordinance, enclosed is an inclusion
packet with all Rec District requirements. The packet contains instructions and forms to help you
successfully and legally petition into the Rec District. A petition for inclusion form must be completed
and executed before a notary. Further, a current legal description of the property must be attached to the
petition as "Exhibit A." The Rec District recommends you maintain a complete copy of the inclusion
packet for your personal files, before returning all original documents to us for processing. The Rec
District will absorb any costs associated with processing the inclusion and a public hearing shall be
scheduled.
The Rec District requests that you respond with the petition for inclusion at the same time you
submit your application for annexation. Please do not hesitate to contact me directly with questions or

INSTRUCTIONS FOR INCLUSION OF REAL PROPERTY INTO
CARBON VALLEY PARK AND RECREATION DISTRICT
1. A petition for inclusion must be executed before a notary public by 100% of the
owners of the real property to be included into the District.
2. An accurate legal description of the real property to be included into the District
must be attached to the fully executed petition for inclusion.
3. The fully executed petition for inclusion must be filed with the District at the
following address:

4. Upon receipt of the petition for inclusion, the Board of Directors of the District
shall set a public hearing at which the Board will consider the petition. Notice of
public hearing will be published one time prior to the hearing.
5. At such public hearing, upon approval of the petition, the Board will issue an
Order of Inclusion, which shall be filed with the Weld County District Court with
a request that the District Court Judge issue a final Order of Inclusion.
6. Upon issuance of the Order of Inclusion by the Weld County District Court Judge,
the Order will be recorded with the Weld County Clerk and Recorder and the
inclusion shall be effected.
7. Copies of the recorded Order of Inclusion shall be sent to the Division of Local
Government within the Department of Local Affairs and to the Weld County
Assessor.
