HomeMy WebLinkAbout20090039.tiffRESOLUTION RE: APPROVE FOUR PURCHASE OF SERVICES AGREEMENTS FOR OPERATIONAL ACTIVITIES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with four Purchase of Services Agreements for Operational Activities between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Agency, and the various Senior Centers, as follows, commencing December 1, 2008, and ending June 30, 2009, with further terms and conditions being as stated in said agreements, and Senior Citizens of Nunn Hill N Park Senior Citizen Program Lochbuie After Glows Pierce Senior Center WHEREAS, after review, the Board deems it advisable to approve said agreements, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the four Purchase of Services Agreements for Operational Activities between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Agency, and the various Senior Centers as listed above be, and hereby are, approved. Griego, Director, Human Service' ommiss ep ers Purchase of Services Agreements between the Weld County Department of Human Services and Four (4) Service Providers Enclosed for Board approval are Purchase of Services Agreements between the Weld County Department of Human Services and Four (4) Service Providers. These Agreements were presented at the Board's December 15, 2008, Work Session. The Service Providers are: 1. Hill N Park Senior Citizens Program 2. Lochbuie Senior Citizens Program/After Glows 3. Senior Citizens of Nunn 4. Pierce Senior Citizens The major provisions of these Agreements are as follows: I . These Agreements are the Operational Agreements in the amount of $500.00 each. 2. These grants help the facilities pay basic operational costs such as heating, electricity, insurance, etc. This helps assure that our senior nutrition program will continue to be able to operate in these centers. 3. The term of these Agreements is a one time only award and the terms are valid through June 30, 2009. If you have any questions, give me a call at extension 6510. 2009-0039 Purchase of Services Agreement THIS AGREEMENT, made and entered into this 1 s` day of December, 2008 by and between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Hill N Park Senior Citizen Program hereinafter referred to as "Hill N Park." WITNESSETH: WHEREAS, "AAA" develops and administers the Area Plan for a comprehensive and coordinated system of services, and WHEREAS, "AAA" considers small rural senior centers a vital link in the county's system of services to older adults, and WHEREAS, "Hill N Park" is a senior center providing recreational, nutritional, and educational opportunities for senior citizens; NOW THEREFORE, it is mutually agreed: 1. The "AAA" hereby agrees to award grant monies (under the Older Americans Act) in the sum of $500 to "Hill N Park" 2. "Hill N Park" agrees to expend the grant monies to pay for operating expenses for the center to include: electricity, gas, telephone, insurance, office supplies, postage, and other minor operating expenses. 3. "Hill N Park" agrees to maintain documentation of the above expenditures and to seek prior approval from the Area Agency on Aging if it is needed to expend a portion of this grant on expenditures not listed in number (2) above. 4. "Hill N Park" understands that the terms of the agreement are valid through June 30, 2009 IN WITNESS WHEREOF, the parties hereunto have caused this Agreement to be duly executed as of the day and year first hereinabove set forth.
