HomeMy WebLinkAbout20092660.tiffRESOLUTION RE: APPROVE SCOPE OF WORK FOR WOMEN'S WELLNESS CONNECTION INFRASTRUCTURE BUILDING GRANTS PROGRAM AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Scope of Work for the Women's Wellness Connection Infrastructure Building Grants Program from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, to the Colorado Department of Public Health and Environment, commencing upon full execution, with further terms and conditions being as stated in said Scope of Work, and WHEREAS, after review, the Board deems it advisable to approve said Scope of Work, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Scope of Work for the Women's Wellness Connection Infrastructure Building Grants Program from the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, to the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said Scope of Work. Wallace, MD, MPH, Director Department of Public Health and Environment —IV WA., 400—gea, DATE: September 21, 2009 SUBJECT: Infrastructure Building Grants Program Women's Wellness Connection (WWC) FY 2009-2010 Enclosed for Board review and approval is the Scope of Work as part of the Weld County Department of Public Health and Environment's application for funding from the Women's Wellness Connection Infrastructure Building Grants Program of Colorado Department of Public Health and Environment. Issuance of a purchase order will allow WCDPHE to continue the services of a .5 FTE WWC Community Outreach Specialist who provides case management, outreach, and enrollment services for Weld County women aged 40 years and older who qualify for breast and cervical cancer screenings under the program guidelines. The department is applying for funding in the amount of $24,995.28 for the time period beginning on the date the purchase order is issued and continuing through June 28, 2010. It is a state -run program of the National Breast and Cervical Cancer Early Detection Program (NBCCEDP), which is administered through the Centers for Disease Control (CDC). Services are available across Colorado. The objective of this Infrastructure Building Grants Program is to provide funding to newly contracted WWC providers so that infrastructure can be established which support services to women in a manner that meets will all state and federal requirements under NBCCEDP. Direct Beneficiaries: Direct beneficiaries of the grant will be organization that will serve women residing in the contractor's region who are unable to pay for breast or cervical screening services, the organization at which WWC services will be performed, and the community in which the agency resides. Project Goals: 1. To provide support to new contractor's in the WWC program as it establishes WWC program services its first active year. Funding and Timeline 1. Funding is a one-time opportunity and capped at 525,000. a. It is expected that the grant will provide funding towards establishment of a sustainable screening program after the grant has ended. b. A budget and budget narrative not to exceed $25,000 (templates attached) must be submitted prior to grant approval. 2. Work shall begin from date of fully executed purchase order through June 28, 2010. Project Specifies L Funding can be used for any or all of the following categories: a. Personnel necessary for the operation of the WWC program. i. WWC will pay for salary and benefits only. ii. Funding will not pay for: professional educational training. [Colposcopy training for medical providers is available under a separate WWC grant.] b. Equipment necessary to adequately' perform breast and cervical cancer screening. i. This may include equipment necessary to perform breast and cervical cancer screening, such as speculums, colposcopes or mammographic equipment. Request may also cover lap top computers. ii. Funding will not pay for: portable phones or personal handheld devices, patient educational materials, gifts, incentives or training models. [Educational materials are available free of charge at �� �� « ..... �<�,,, .0», .] c. Marketing of the WWC program in the contractor's community. i. This may include advertisements in local newsprint, flyers, event hosting or health fair sponsorship, electronic communications or any other form of media, advertising and public relations. [WWC has a marketing CD containing media material free of charge at wvvw.wwcitems.com and can supply at no cost additional print and radio ads upon request. Technical assistance will be provided at no cost by the program's media contractor upon request.] d. Other as approved by the WOMEN'S WELLNESS CONNECTION program. Deliverables: Under this arrangement, the contractor shall perform the following activities within the established timeframes. Within first 2 weeks of approval date of purchase order: 1. Establish and identify the WWC Coordinator, WWC eCaST Coordinator and Fiscal Payment Coordinator. 2. Develop a Work Plan that outlines how the contractor will ensure WWC services will become active within the first fiscal year. 3. For appropriate staff persons, apply for data access to the program's web -based program. 4. Host one conference call with WWC staff to review and approve work plan. Discussion will also cover review of the service delivery scope of work, support materials available and tools used in the program. Within 4 weeks of approval date of purchase order: 5. Host one conference call with WWC staff. Call must include discussion of subcontractors, management of abnormal results, enrollment process, and medical providers participating in program. Within 3 months of approval date of purchase order: 6. Attend state -sponsored trainings or demonstrate training of: i. Basic WWC Program Overview ii. eCaST Basics: data entry, reporting, reimbursement iii. WWC Contracts and Subcontracts iv. Medicaid process v. Quality Management (core indicators, clinical tracking and follow-up) vi. Cultural Competency vii. Medical policies and guidelines viii. Clinical web casts Within 7 months of approval date of purchase order: 7. Host at least 2 conference calls with WWC staff to review work plan, status of core performance indicators, fiscal reports and screening numbers to date. 8. Begin active screening services to women under the service delivery contract. By June 28, 2010: 9. Provide a completed and final work plan to the program. 10. Have contractor participate in an evaluation of the support and technical assistance provided to them under this grant. Reimbursement Project expenses will be reimbursed on a monthly basis for approved expenses incurred in the prior month. Reimbursement requests must be submitted monthly and are due on the 51h of each month. The state will not reimburse any costs incurred by the applicant prior to the issuance of a legally executed contract, task order, purchase order or other authorizing legal document. She will also input data into the e -cast system and assist Case Manager with follow up on clients with abnormal findings. Fringe benefits for A. Chairez for 11 months $5,808.74 Remaining one month salary will be covered by fee for service revenues. Operating Expenses: N/A Operating expenses will be supplied in -kind by WCDPHE. Outreach materials provided by WWC. Travel: $570.00 In state mileage Weld County rate $0.53 per mile. Travel to outreach events such as health fairs. Outreach activities in the Southwest Weld region.
