RE: APPROVE AMENDMENT TO ADDENDUM FOR AGREEMENT TO PURCHASE OUT OF
HOME PLACEMENT SERVICES AND AUTHORIZE CHAIR TO SIGN - SHILOH HOUSE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Addendum for the
Agreement to Purchase Out of Home Placement Services between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, and the Shiloh House, commencing July 1, 2008, and ending
June 30, 2009, with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Amendment to the Addendum for the Agreement to Purchase Out of
Home Placement Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human Services,
and the Shiloh House be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 23rd day of February, A.D., 2009, nunc pro tunc July 1, 2008.

Amendment to the Weld County Addendum between the
Weld County Department of Human Services and Shiloh
House
Enclosed for Board approval is an Amendment to the Weld County Addendum between the
Weld County Department of Human Services and Shiloh House. This Amendment was
reviewed at the Board's February 10, 2009, Work Session.
The major provisions of this Amendment are as follows:
1. The County desires to modify their current Agreement (PY08-2586) in order to decrease
the number of guaranteed beds as outlined in Paragraph 1(A).
2. As of March 15, 2009, Contractor will provide six (6) guaranteed Therapeutic Residential
Child Care Facility (TRCCF) beds, located at 745 Highway 119, Longmont, CO, for the
exclusive use of the County.
3. County agrees to pay $175.59 per day for each guaranteed bed that is not filled by a
County youth.
4. The term of this Agreement is July 1, 2008 through June 30, 2009.

To that certain Agreement to the Weld County Addendum between Shiloh House and the
Weld County Department of Human Services for the period from
July 1, 2008 through June 30, 2009. q t

This Amendment made and entered into the /Cc day of , 2009, by and between the Board of I1 30
County Commissioners, on behalf of the Weld County Department of Human Services, hereinafter referred to as
"County," and Shiloh House, Provider ID#1539982, hereinafter referred to as the "Contractor."
WITNESSETH
WHEREAS, the County desires to modify their current Agreement (PY08-2586) in order to decrease the
number of guaranteed beds as outlined in Paragraph 1(A);
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as follow:
1. As of March 15, 2009, Contractor will provide six (6) guaranteed Therapeutic Residential Child
Care Facility (TRCCF) beds, located at 745 Highway 119, Longmont, CO, for the exclusive use of
the County. County agrees to pay $175.59 per day for each guaranteed bed that is not filled by a
County youth.
This Amendment, together with the original Agreement to Purchase Out -of -Home Placement Services and the Weld
County Addendum, constitutes the entire understanding among the parties with respect to the subject matter hereof,
and may not be changed or modified except as stated in the Agreement to Purchase Out -of -Home Placement
Services.
