RE: APPROVE AGREEMENT FOR DELIVERY OF HEALTHIER LIVING COLORADO
PROGRAM AND AUTHORIZE CHAIR TO SIGN - CONSORTIUM FOR OLDER ADULT
WELLNESS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Agreement for Delivery of the Healthier
Living Colorado Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and the
Consortium for Older Adult Wellness, commencing November 16, 2009, and ending June 30, 2010,
with further terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Agreement for Delivery of the Healthier Living Colorado Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and the
Consortium for Older Adult Wellness be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 14th day of December, A.D., 2009, nunc pro tunc November 16, 2009.

Agreement between the Weld County Department of
Human Services' Area Agency on Aging and the
Consortium for Older Adult Wellness Regarding Delivery
of Healthier Living Colorado, Using Amendment 35 Grant
Funding
Enclosed for Board approval is an Agreement between the Department's Area Agency on Aging
and the Consortium for Older Adult Wellness Regarding Delivery of Healthier Living Colorado,
Using Amendment 35 Grant Funding. This Agreement was presented at the Board's October 26,
2009, Work Session.
Several months ago the Weld County Commissioners and the Department of Human Services'
Area Agency on Aging division agreed to participate with other partnering agencies in an
Amendment 35 grant application. We were notified a month ago that the grant was approved and
we now have the final Memorandum of Understanding which outlines the tasks that must be
completed in order to access the approved funds.
We will be providing four (4) Chronic Disease classes throughout the county and we also will
have training funds that can be used to train additional volunteer or staff trainers so that we are
able to offer additional classes with the Weld community.
The funding dollars not to exceed $10,000.00 will be available November 16, 2009 through June
30, 2010. Our wellness programs within the Area Agency on Aging will continue to be provided
after Amendment 35 grant money have been expended by using other appropriate Federal dollars.

Agreement between the Consortium for Older Adult Wellness and
Weld County Area Agency on Aging Regarding Delivery of
Healthier Living ColoradoTM
(aka K. Lorig Chronic Disease Self Management Program)
Using Amendment 35 Grant Funding
We, Weld County Area Agency on Aging, agree to the following:
❑ We understand that we will be operating under a license held by the Consortium for Older Adult Wellness (COAW). COAW
is responsible for ensuring that all partner agencies comply with rules and regulations regarding instruction and
administration of the Healthier Living Coloradoml program in accordance with the Stanford licensing requirements. We
understand that any violation of the following agreement may result in termination of our partnership with COAW and the
Regional Coordinating Agencies. If this occurs, we will no longer operate under said license and we will discontinue the
Healthier Living ColoradoTM/Chronic Disease Self -Management workshops. We may be required to reimburse
COAW/Regional Coordinating Agencies for all costs incurred including training, materials and implementation funds.
❑ We agree to conduct workshops in accordance with "The Chronic Disease Self- Management Program Leaders Manual". We
will not create derivatives of the program. We will not reproduce or distribute material derived or adapted from the program.
❑ We understand that we will receive technical support for training and implementation of the Healthier Living Colorado
programs from COAW. We will be responsible for any additional supplies or expenses.
❑ We will provide locations, dates and times of our classes, before they begin, to the Regional Coordinator for inclusion in
COAW's statewide marketing efforts.
❑ We understand that the funding is not to exceed $10,000 and must all be spent prior to June 30, 2010.
❑ We understand that the end of funding for this project is June 30, 2010 and the responsibilities under this grant also
conclude on June 30, 2010.
❑ We agree that this funding may be used toward the training through COAW of up to 5 lay leaders at the rate of $900 per
trainee.
Li We will hold up to 5 series of classes from November 16, 2009 —June 30, 2010. The first class will be scheduled within 3
months of the signing of this agreement.
❑ The agency will be reimbursed for implementation of classes that are directly connected to a family medicine clinic in the
Patient Centered Medical Home Project. These funds may be requested by completing the attached voucher.
❑ Acceptable items for reimbursement include lay -leader stipends in the amount of $300 per leader, per six week session.
❑ Documentation and/or receipts will be required to be submitted.
❑ The request for reimbursement must be submitted monthly. All receipts must be submitted in the month the funds were
spent.
❑ We agree to have fidelity checks done on all of our classes in accordance with COAW policies and procedures and will pay
COAW $150 per fidelity visit conducted.
❑ We will submit a class roster and pre/post test class data to our Regional Coordinator for each class. We agree to send the
class roster and pre test class data within 10 business days of the first class of the session. We agree to send the class roster
and post test class data within 10 business days of the last class of the session.

❑ We agree to submit a class roster and follow-up data for each class 6 months after the conclusion of the class. Preferably,
data can be collected at a 6 -month, post -class reunion. The 6 -month post data may also be collected by mail or phone. We
will send the class roster and 6 -month post class data to the Regional Coordinator within 15 business days of the 6 -month
anniversary of the first day of the class.
❑ We agree to send all media generated from our classes and all marketing materials used to our Regional Coordinator.
❑ We understand if we have any questions regarding the implementation of this program, or any related topics, we can contact
our Regional Coordinator, or the HASS Manager.
