Thank you for having Banner Occupational Health Worksite Wellness at North Colorado Medical Center
provide Lunch 'n Learns, Bone Density Screenings, Skin Cancer Screenings, Glucose Finger Sticks, Colo
Kits, First Aid, PSA Screenings, Health Assessments and Chair Massage for employees at Weld County.
We look forward to working with you on these events as well as your future health and wellness needs.
The following services and rates shall be provided to your company effective March 6, 2009 through
Decemeber 31, 2009. It is understood that either party may terminate this Letter of Agreement at anytime
without cause or penalty upon 30 days prior written notice.

Bone Density Screening
Osteoporosis is a disease that causes your bones to gradually weaken and become fragile. With the
help of low-cost ultrasound screenings, we are able to determine if you are at risk for developing
osteoporosis or osteopenia (an early form of osteoporosis). Early detection is key in the prevention
and treatment of osteoporosis.
Cost: $25/per person

Skin Cancer Screenings
Melanoma is the most serious form of skin cancer. If diagnosed and removed early, it is almost
100 percent curable. Skin cancer screenings should be part of everyone's preventive health care.
Our qualified medical personnel can provide this service as part of a company health fair or as a
stand-alone service.
Cost: $165 per hour

Colo Kits were provided on 2/26/09:
Colorectal Kits
Occult blood testing is the first line of defense for anyone age 40 and over in the fight against
colorectal cancer. 99% of all cases of colon and rectal cancers in the United States occur in people
over 40. The test can be taken in the privacy of your own home and is simple, clean and effective.
Cost: $10/person

Heart Saver First Aid
This class utilizes the American Heart Association comprehensive approach to medical and
traumatic injuries. Topics covered include fractures, bleeding, shock, allergic reactions, seizures,
diabetic emergencies, and environmental emergencies. Upon completion of the course, participants
receive a two year First Aid completion card.
Cost: $35/person

Prostate Specific Antigen (PSA) Blood Screening
This is a blood screening for men. The American Urological Association and the Prostate Cancer
Education Council recommend annual prostate screenings for all men age 50 and over (earlier if
positive family history).
Cost: $25 per person
Other Services Provided:
Health Assessment
Health Assessments include: Blood Pressure, Body Composition, Hip and Waist Measurements,
Step Test, Sit and Reach, and Sit-ups and Push-ups. It takes approximately 30 minutes per person
to completer the fitness assessment.
Cost: $25/person, date, time and location TBA upon request
Chair Massage
A Certified Massage Therapist will be provided for employees who choose to have a 10 minute
chair massage. Employees are massaged in their work attire and areas can include back, neck,
arms, and hands.
Cost: $65/hr, date, time and location TBA upon request
Weld County is covering the cost and/or collecting money for any of the events or screenings
scheduled above.
• Billing: Billing for all screenings will be sent directly to the company.

** Please note: It is up to the employer to collect money from individual employees who are
participating in these events. We require one check from the employer for services rendered.

Either party may terminate this letter at anytime without cause or penalty upon thirty (30) days prior
written notice to the other party. Unless specifically responded to by Weld County in writing within ten
(10) business days, all services provided by Banner will be subject to the terms of this letter. We look
forward to meeting your occupational health needs. Please remember that other services may be added
