RE: APPROVE AMENDMENTS TO FOUR PURCHASE OF SERVICES AGREEMENTS WITH
VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with four Amendments to Purchase of Services
Agreements between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on
Aging, and the following providers, commencing February 1, 2009, and ending June 30, 2009, with
further terms and conditions being as stated in said amendments, and
1. Meals on Wheels
2. North Range Behavioral Health
3. Eldergarden
4. Catholic Charities
WHEREAS, after review, the Board deems it advisable to approve said amendments,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the four Amendments to Purchase of Services Agreements between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, Area Agency on Aging, and the above
listed providers be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.

Enclosed for Board Approval are the Amendments to Purchase of Services Agreements
between the Department and Various Providers. The Department's Area Agency on Aging
has funding that has not been spent and needs to expend by June 30, 2009, by increasing

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of February, 2009 by and between the Weld County
Department of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and Meals on Wheels,
hereinafter referred to as "MOW"
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase adult day services from "MOW", and
WHEREAS, the "AAA" and "MOW" are desirous of amending the original agreement of July 1, 2008;
NOW THEREFORE, it is mutually agreed:
I. The "AAA" will increase the current contract from $47,960 (Older American Act and Older Coloradoan funds) to
$57,960, reimbursing "MOW" $3.10 per meal served. "MOW" agrees to provide an additional 3225 units of services
for 50 seniors.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from February 1, 2009 through June 30, 2009. This Amendment will be
terminated immediately in the event of loss of funding.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of February, 2009 by and between the Weld County
Department of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and Catholic Charities,
hereinafter referred to as "CC"
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase adult day services from "CC", and
WHEREAS, the "AAA" and "CC" are desirous of amending the original agreement of July 1, 2008;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will increase the current contract from $27.225 (Older American Act and Older Coloradoan funds) to
$37,225, reimbursing "CC" the cost of direct one on one outreach services. "CC" agrees to provide an additional 300
units of services for 100 seniors.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from February 1, 2009 through June 30, 2009. This Amendment will be
terminated immediately in the event of loss of funding.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of February, 2009 by and between the Weld County
Department of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and Eldergarden, hereinafter
referred to as "Eldergarden"
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase adult day services from "Eldergarden", and
WHEREAS, the "AAA" and "Eldergarden" are desirous of amending the original agreement of July I, 2008;
NOW THEREFORE, it is mutually agreed:
I. The "AAA" will increase the current contract from $30,250 (Older American Act and Older Coloradoan funds) to
$55,250, reimbursing "Eldergarden" $12.00 per hour of adult care services. Eldergarden agrees to provide an
additional 2 083 units of services for I I seniors.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from February 1, 2009 through June 30, 2009. This Amendment will be
terminated immediately in the event of loss of funding.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of February, 2009 by and between the Weld County
Department of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and North Range Behavioral
Health, hereinafter referred to as "NRBH"
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase adult day services from "NRBH", and
WHEREAS, the "AAA" and "NRBH" are desirous of amending the original agreement of July 1, 2008;
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will increase the current contract from $39,930 (Older American Act and Older Coloradoan funds) to
$49,930, reimbursing "NRBH" at an hourly rate of $19.70. "NRBH" agrees to provide an additional 507 units of
services for 120 seniors.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this
Amendment.
3. The terms of this Amendment extend from February 1, 2009 through June 30, 2009. This Amendment will be
terminated immediately in the event of loss of funding.
