February 15, 2009 the following entity has submitted a signed Agreement for the Sampling
and Analysis of Drinking Water.

Stone Mountain Lodge
We are forwarding this information according to Resolution #2001-3421 dated December 17,
2001. All fees are in accordance with the fees established in Weld County Code, Appendix 5-G.
However some agreements reflect a discount based on volume.
