between the Weld County Department of Human Services'
Family Educational Network's Head Start and Migrant and
Seasonal Head Start Programs and Jodi Little - To Be Placed
on Consent Agenda
Enclosed for Board approval is a written notice of termination of a Consultant Agreement
between the Department's Family Educational Network's Head Start and Migrant and
Seasonal Head Start programs and Jodi Little. Please place on Consent Agenda.
The Consultant has provided a written notice of termination of the Consultant Agreement to
be effective September 15, 2009. The Consultant provided Audiology Services to the Head
Start and Migrant and Seasonal Head Start programs.
The original date of the Agreement was for the period of June 19, 2009 through June 19,
2010, for a maximum amount of $8,000 for Head Start and $4,000 for Migrant and Seasonal
Head Start

This letter is to serve as written notice of termination of the consultant agreement
between myself, Jodi Little, and Family Educational Network of Weld County. Per the
agreement, this notice provides 30 days as specified.
Due to the nature of the contracted position, Audiology Service - hearing screening,
I will continue to fulfill my obligations through the end of the current Migrant Head
Start session.
