RE: PHYSICAL, VISION CARE AND DENTAL CARE SERVICES CONTRACT BETWEEN WELD
COUNTY AND THE EAST SIDE HEALTH CLINIC.
WHEREAS, the East Side Health Clinic has submitted a contract to the
Board of County Commissioners, Weld County, Colorado, acting as the Commun-
ity Action Agency on behalf of the Department of Human Resources-Weld County
Head Start Program, copy of which is attached hereto and made a part hereof
by reference, and
WHEREAS, after careful review and consideration of the contract, the
Board believes it to be in the best interest of the County to accept the
same.
NOW, THEREFORE, BE IT RESOLVED, by the Board of County Commissioners,
Weld County, Colorado, that the hereinabove mentioned contract as submitted
by the East Side Health Clinic be, and the same is hereby approved and ac-
cepted for signature by the Board.
The above and foregoing Resolution was, on motion duly made and seconded,

AGREEMENT
THIS AGREEMENT is made and enteredinto at Greeley, Colorado, this

COLORADO, acting as the Community Action Agency hereinafter referred to
as the AGENCY, on behalf of the Weld County Department of Human Resources-
Head Start Prpgram, and the EAST SIDE HEALTH CLINIC, hereinafter referred
to as the "CONTRACTOR," both of Weld County,
Witnesseth That:
The AGENCY and the CONTRACTOR do mutually agree as follows :
1. The CONTRACTOR shall, in a satisfactory and proper manner as deter-
mined by the AGENCY, perform the following:
A. Dental Care service to include:
1. Dental prophylaxis and instruction in self care
and hygiene procedures.
2. Application of topical fluoride.
3. Restoration of decayed primary and permanent teeth.
4. Pulp therapy for primary and permanent teeth.
5. Services required for the relief of pain or infection.
The hereinabove services provided will be:
60 children-10 of which are to be paid by medicaid. 50 are to
be paid by the AGENCY. ($25.00 x 50 = $1,250.00).
2. The CONTRACTOR shall commence performance on this Contract on the
20th day of October, 1975, and shall complete performance to the satisfaction
of the AGENCY no later than the 19th day of December, 1975.
Dental Screening and partial treatment will be complete by December
31, 1975. Total care will be completed by May 31, 1976.
3. THE CONTRACTOR shall maintain such records and accounts including
property,personnel and financial records as are deemed necessary to THE
AGENCY or the Director of HEW to assure a proper accounting for al.l project
funds, both Federal and Non Federal shares. These records will be made avail-
able for audit purposes to THE AGENCY, the HEW, or Comptroller General of the
United States or any authorized representative, and will be retained for
three years after the expiration of this Contract unless permission to des-
troy them is granted by both the AGENCY and the Director of HEW.
4. Services provided for under this Agreement shall be compensated for
THE AGENCY in the following manner:
Billing for services provided will be made as of December 31 , 1975.
5. It is expressly understood and agreed that in no event will the total
amount to be paid by the AGENCY to the CONTRACTOR under this Agreement exceed
One Thousand TWo Hundred Fifty Dollars ($1,250.00) , for full and complete
satisfactory performance. The CONTRACTOR will not be obligated to render ser-
vices in excess of this amount.
6. This Agreement is subject to and incorporates the attached Part II,
"Terms and Conditions Governing Contracts Between Community Action Program
Agency and Contractor for Professional or Technical Services to a Community
Action Program."
7. The CONTRACTOR agrees to assist the AGENCY in complying with all the
"Conditions Governing under Section 204 and 205 of the Economic Opportunity
Act of 1964."
8. IN WITNESS WHEREOF, the AGENCY and the CONTRACTOR have executed this
Agreement as of the date first above written.
