The Weld County Board of Health met on November 25, 2025, at 9:00 a.m. in the second-floor
hearing room of the Weld County Department of Public Health and Environment and via Microsoft
Teams.
Those present included:
Board Members:Joyce Smock, Lonnie Ford, Michael Wailes, Linda Kautz and Bree Lukens.
Others:Alex Clemments, Environmental Health Specialist, Nick Trautner, Environmental Health
Manager and Ashtyn Kilber, Environmental Health Specialist, Weld County Department of Public
Health and environment.
APPROVAL OF AGENDA
The meeting was called to order at 9:00 AM. A motion was made by Lonnie Ford to approve the
agenda. Michael Wailes seconded the motion, and the motion was carried unanimously.
APPROVAL OF MINUTES
The board approved the minutes of the October 28, 2025, meeting. A motion was made to approve
by Michael Wailes,Joyce Smock seconded the motion,and the motion was carried unanimously.

A motion was made by Joyce Smock for approval of the renewals of Denali water solutions. Lonnie
Ford seconded the motion, and the motion was carried unanimously.
PRESENTATIONS: Kelly Martinez, Community Health Chronic Disease Prevention Manager,Weld
County Department of Public Health and Environment. Preventing Diabetes Together: Building
Strong Community Partnerships for Health.
Kelly talked about Diabetes in Weld County and how this is a major public health problem in Weld
County she mentioned that 1 in 9 residents(11.4%) reported being diagnosed by a health care
provider with diabetes at some point in their lives, while an additional 11.3%have been told they
are prediabetic.
2022 WCDPHE Community Health Survey revealed that the burden of diabetes is not evenly
distributed.A higher percentage of Hispanic/Latino Weld County residents reported they had
diabetes (15.9%)compared to White, Non-Hispanic/Latino residents (9.7%).
Some impacts on health due to diabetes are the major cause of blindness, kidney failure, heart
attacks, strokes and lower limb amputations.
She explained that the diabetes prevention program is an evidence-based year-long lifestyle change
program to help participants at risk for type 2 diabetes adopt sustainable, healthy lifestyle choices
and achieve weight loss. Hearth of Weld DPP Program employees in 2023,8 staff members became
trained and certified as CDC lifestyle coaches and began to offer the CDC's NDPP in 2025 this
program received full recognition which is the highest achievement possible and is reserved for
groups that have effectively delivered a high-quality, evidence-based program that meets all the
standards for CDC recognition.
Heart of Weld Community Partners include northern Colorado Health Alliance,Sunrise Community
Health Center, UCHealth, Weld County Government, City of Greeley and Greeley/Evans School
District 6.
STAFF DISCUSSION—Nick appreciated BOH members taking the time to volunteer BOH monthly
meetings in 2025 and hopes for a new& informative year 2026. He also mentioned that a position
is open and posted for Katie's job.
