Approve Mini-Grant Application for Casey Family Programs Funding to Support
Foster Youth in Transition and Authorize Department of Human Services to Submit
Electronically — Colorado Human Services Directors Association (CHSDA)
Whereas, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
Whereas, the Board has been presented with a Mini-Grant Application for Casey Family
Programs Funding to Support Foster Youth in Transition from the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Human Services, to the Colorado Human Services Directors
Association (CHSDA), with further terms and conditions being as stated in said
application, and
Whereas, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
Now, therefore, be it resolved by the Board of County Commissioners of Weld County,
Colorado, that the Mini-Grant Application for Casey Family Programs Funding to Support
Foster Youth in Transition from the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, to the Colorado Human Services Directors Association (CHSDA), be, and
hereby is, approved.
Be it further resolved by the Board that Julie Witkowski, Department of Human Services,
be, and hereby is, authorized to submit said application electronically.
The Board of County Commissioners of Weld County, Colorado, approved the above
and foregoing Resolution, on motion duly made and seconded, by the following vote on
the 10th day of December, A.D., 2025:

Brief description of the problem/issue: The Department is requesting approval to submit a mini-grant
application to the Colorado Human Services Directors Association (CHSDA). The grant, funded by
Casey Family Programs, provides up to $20,000 to support child welfare initiatives across the state.
The Department intends to apply under Priority Funding Area 3 to support Foster Youth in Transition
(FYIT). If awarded, the funds will be used to assist youth with housing application fees, security
deposits, cell phone costs, and transportation expenses—critical supports that enhance stability,
promote independence, and improve long-term outcomes for transitioning youth.
The grant application is due December 10, 2025.
What options exist for the Board?
• Approval of the CHSDA Mini-Grant Application.
• Deny approval of the CHSDA Mini-Grant Application.
Consequences: The Family Resource Division will not be able to apply for this grant
opportunity.
Impacts: The Family Resource Division will not receive grant funding to support Foster Youth
in Transition.
Costs (Current Fiscal Year/Ongoing or Subsequent Fiscal Years):
• Total Grant Request = $19,950.00
Recommendation:
• Approval to electronically submit the Mini-Grant Application. No signature is required.

CHSDA Mini-Grant Application
CHSDA is pleased to announce the following opportunities for mini-
grants to county departments of human services, utilizing a portion of
our Casey Family Programs funding for Calendar Year 2025. A total of up
to $20,000 will be awarded. Applications are due no later than 12/10/25
at 4pm. Joint applications are welcome! All awardees will be notified
before December 31, 2025 so funds can be transferred prior to the end
of the calendar year.
CHSDA and Casey Family Programs in 2025 are collaborating on child
welfare and CO's behavioral health continuum of care with an equity
lens. This funding opportunity targets the following associated funding
areas:
Priority Funding Area 1: Level-up meaningful integration of Lived Expert
populations into program and policy discussions at the county-level
and/or further county initiatives to advance equity in child welfare.
Example strategies include: Establish feedback loops where lived
experts see how their input is used to shape/improve practice and
policy; Provide equitable and reliable compensation and offer training
and mentorship so that lived experts have full access to technical
knowledge, system context, and tools to influence change; Invest in
leadership development pipelines for lived experts to move into formal
positions within agencies and/or policymaking bodies; Create equity-
focused metrics developed in partnership with lived experts to measure
whether their input is actually shaping outcomes.
Priority Funding Area 2: Advance child welfare safety, permanency, and
well-being outcomes in alignment with Colorado's Child & Family
Services Review (CFSR) measures and Child and Family Services Plan
(CFSP).
Priority Funding Area 3: Support any of the following populations:
relative/kinship, Foster Youth in Transition, and/or LGBTQ+ youth.
All counties awarded will be asked to submit updates on project
progress for reporting purposes.

Weld County Department of Human Services (WCDHS) is seeking funding support to assist
Foster Youth in Transition (FYIT)youth with housing application fees, security deposits,
phone and transportation expenses. Securing stable housing is a crucial investment which
will yield significant personal, social, and economic benefits. WCDHS is currently serving
32 young adults through the FYIT program, 17 of whom have independent housing, either
alone or with roommates. Several others are residing with family and saving in hopes of
obtaining independent housing, however, many do not have strong support from family or
friends to assist them with housing expenses. Obtaining stable housing is a critical first
step in achieving stability and will allow these individuals to proceed with additional steps
toward independence, such as education,job search, or addressing health needs.A safe,
consistent place to stay reduces stress, provides a secure environment and pushes young
people to develop essential life skills such as budgeting and time management.
Additionally, having consistent access to a phone and transportation will further increase
their ability to access employment or educational opportunities. WCDHS proposes to fund
phone expenses up to$100 per youth and transportation expenses up to$740 per youth.
Transportation expenses would include:
• Vehicle repairs/maintenance and insurance(with current driver's license and
registration)
• Fuel

With this assistance, and the support of their WCDHS Caseworker,youth will be better
positioned to begin building credit, developing healthy financial habits, and increasing
efforts toward joining the workforce or pursuing education. Since the FYIT program is
limited to the age of 21, the sooner youth develop these skills, the more successful they
will be in achieving independence and self-sufficiency.Youth who participate in the FYIT
program have an opportunity to break cycles of poverty and homelessness, ultimately
leading to a healthier community.

Average application fee of$25 x 6 youth=$150
Average security deposit fee of$1200 x 6=$7,200.00
Transportation up to$740 x 15 youth =$11,100
Phone expenses up to$100 x 15 youth =$1,500

On behalf of CHSDA, we are pleased to inform you that Weld has been selected as an award recipient for
a CHSDA mini-grant utilizing our Casey Family Programs funding!
The total amount we are able to award Weld is: $1,500.
This award is intended to support your county's efforts as outlined in your application (attached) to
support FYiT phone, housing application, and transportation expenses.
Because this is such a small amount and isn't the full amount you requested, please let us know if you
would prefer to decline the funding. We will require a simple reporting update on how funds are spent
in alignment with your proposed project.
If you would like to accept the funding, please send Emily and I details asap for where CHSDA
should send the check(payee name, mailing address, any other department information to receive
the funds). We have to have finance staff at Larimer County(our CHSDA treasurer) send out a check
tomorrow.
