IN THE MATTER OF APPROVAL OF LETTER OF AGREEMENT BETWEEN WELD
COUNTY HEALTH DEPARTMENT AND MARY STEIN FOR SPEECH THERAPY
SERVICES.
•
BE IT ORDAINED BY THE BOARD OF COUNTY COMMISSIONERS OF WELD
COUNTY, COLORADO:
WHEREAS, the Weld County Health Department has presented
to the Board of County Commissioners of Weld County, Colorado a
letter of agreement between said Weld County Health Department
and Mary Stein, Speech Therapist, for speech therapy services.

A copy of said letter of agreement is attached hereto and
incorporated herein by reference at this point, and
WHEREAS, the Board of County Commissioners of Weld County,
Colorado deems it advisable and in the best interests of Weld
County to approve said letter of agreement.
NOW, THEREFORE, BE IT ORDAINED by the Board of County Com-
missioners of Weld County, Colorado that the letter of agreement
between the Weld County Health Department and Mary Stein for
speech therapy services be, and hereby is, approved.
BE IT FURTHER ORDAINED by the Board of County Commissioners
of Weld County, Colorado that an emergency exists which requires
that this Ordinance be enacted forthwith in that Ordinance proce-
dures would prevent said letter of agreement from being executed
due to the lack of time, and therefore, this Ordinance is declared
to be an emergency Ordinance under the provisions of Section 3-14
of the Weld County Home Rule Charter.
The above and foregoing Ordinance No. 46 was , on motion duly
made and seconded, adopted by the following vote on the 13th day
of December, A.D. , 1978 , nunc pro tunc October 1, 1978 .

LETTER OF AGREEMENT
between
HOME HEALTH CARE SERVICES OF WELD COUNTY HEALTH DEPARTMENT
and the
SPEECH THERAPIST
September 20, 1978
The following policy or procedure items are agreed upon:
The speech therapist has been granted a certificate of clinical competence in
speech pathology by the American Speech and Hearing Association.
The speech therapist's duties shall be as follows:
1. Cooperation in the evaluation of patients with speech, hearing or
language disorders.
2. On the basis of orders by the physician, who indicates anticipated
goals and is responsible for general medical direction of such service
as part of the total care of the patient, determines and recommends
appropriate speech and hearing services.
3. Provides rehabilitative services for speech, hearing and language disorders.
4. Records and reports to the physician the patient's reaction to treatment
and any changes in the patient's condition.
5. Instructs other health team personnel and family members in methods of
assisting the patient to improve and correct speech, hearing and language
disabilities.
6. The speech therapist agrees to supply speech therapy on a part-time basis
for patients referred for therapy by the Home Health Care Service in
accordance with a physician's order.
7. Services to be determined at the following rate:
a. Fifteen dollars ($15.00) per visit.
b. Fourteen cents (l4c) per mile.

8. The speech therapist will submit a timely daily report of activities,
montlhy summary of travel expenses and such other reports and records as
directed by the nursing division.
9. Evaluation by immediate supervisor will be conducted annually on the
anniversary date of employment.
10. It is understood and agreed that either party may terminate this agreement at
any time provided, not less than sixty (60) days prior to the date of
termination, notice in writing shall have been given the opposite party.
